2.The Effect of Pantethine(Pantomin(R)) on Serum Cholesterol and Triglyceride in Hyperlipidemic Patients.

The incidence of atherosclerosis in Korea seems to be much increased due to diet change after national development. The pathogenesis of atherosclerosis is not clarified and there are many hypothesis but the most recent and reliable hypothesis is the ratio of HDL-cholesterol per total cholesterol. Under the basis of this hypothesis there have been much trials to administer the agents which has effect on lipid metabolism, so we tried pantethine on 30 patients who visited Han-Yang University Hospital Internal medicine Department, and the result as follows; 1) The mean age of study population was 50.1 years of age, mean body weight was 62kg, and mean height was 160.8cm. male was 17, and female 10. 2) The associated disease of study population was <19 with> cardiovascular disease, in 19, gastrointestinal disease 3, obesity in 1 and others in 4. 3) The undesirable effect of the drug was found on 3 patients; that is, constipation on 1 patient, dizziness on 1 patient, and skin eruption on 1 patient. 4) The mean serum lipids before and after pantethine administration(levels) are as following table. In conclusion, it seems that the effect of the drug which decreases the serum lipids is mild at initial but more increasing as the time goes by and constant, and we experience little side reaction except mild dizziness, constipation and skin eruption.

4.Clinical characteristics and Effect of Partial Fasciectomy in Dupuytren's Disease of the Korean

From January 1982 to December 1991, we experienced 96 hands of 66 patients with Dupuytren's disease in Seoul National University Hospital. We performed retrospective analysis of these 96 hands of 66 patients and the results were as follows. Among 66 patients, 60 patients (91%) were male and 51 patients (77%) were in sixth or seventh decade, the average age 58.1 years. Occupations of 66 patients were manual laborer(18 patients, 28%), farmer(15 patients, 22%), sportsman(4 patients, 6%) and deskworker(29 patients, 44%). Among the diseases such as diabetes mellitus, alcoholics, epilepsy known as associated with Dupuytren's disease, diabetes mellitus was more common in our cases, whereas alcoholics and epilepsy were relatively rare. There was no case in which family history was present. Both hands were involved in 30 patients, right hand only in 30 patients, and left hand only in 6 patients. Right hand was involved in 60 patients(91%), whereas left hand was involved in 36 patients(55%). The case in which only the 4th finger was involved was most common(42 hands, 44%). The 4th finger was affected most frequently(81 hands, 84%). In diabetic patients, the middle finger was involved more frequently than in non-diabetic patients(p < 0.05). Most of patients(83 hands, 86%) had symptoms of flexion contracture, nodule or contracted cord simultaneously. Surgical treatment was per- formed in 32 hands of 22 patients, with the results of excellent in 24 hands(75%), good in 6 hands(19%) and no recurrence, in average follow-up 3.6 years(2.6 years to 5.0 years). In conclusion, it might be thought that Dupuytren's disease in which surgical treatment was required was relatively rare in Korean and even if necessary, partial fasciectomy resulted in rather good result.

5.Percutaneous Embolization of the Internal Spermatic Vein for the Treatment of Childhood and Adolescent Varicocele.

Proper management of childhood and adolescent varicocele remains still controversial. In general, all methods for varicocele ablation for adults are available and include open surgical ligation, laparoscopic ligation, or percutaneous ablation. We evaluated the usefulness of percutaneous embolization of internal spermatic vein in boys with varicocele. Between February 1988 to May 1996, 48 boys of 53 varicoceles underwent percut aneous embolization. In 49 cases, the embolization were technically possible and in the other 4 cases, we could not access the internal spermatic vein because of vasospasm and venous abnormality. The results were successful in 40 cases (81.6%) and failed in 9 cases (18.4%), including 4 (8.2%) of persistent and 5 (10.2%) of recurred cases. There were no major complications except 3 cases of extravasation of contrast material. Percutaneous embolization may be recommended as a primary treatment for childhood and adolescent varicocele.

6.The Efficacy of 99mTc-DMSA SPECT in Diagnosis of Renal Scarringn of Vesicoureteral Reflux in Children.

PURPOSE: The diagnosis of renal scarring is very important for the pediatric patient because it has been associated with the later development of hypertension, renal failure, and end-stage kidney disease. Recent studies indicate that the sensitivity and specificity of the99mTc-DMSA single photon emission computerized tomography (SPECT) is better than IVP, USG, and 99mTC-DMSA Planar ThG Purpose Of this Study iS tO Compare 2 types Of 9%nTh DMSA renal scan(planar and SPECT) and IVP/USG in the diagnosis of renal scars. MATERIALS AND METHOD: We reviewed retrospectively the data of 49 children(72 renal units) with vesicoureteral reflux(VUR) who underwent renal cortical imaging with IVP/USG from January 1987 to August 1996. Patient ages ranged from 3 months to 14 years with a mean age 3.7 years. There were 17 boys and 12 girls(44 venal units) with 99mTc-DMSA planar with IVP/USG, and 12 boys and 8 girls(28 renal units) with 99mTc-DMSA SPECT with IVP/USG. RESULTS: The 99mTc-DMSA planar imaging identified 94.7% sensitivities while 99mTC-DMSA SPECT imaging identified 96.3% Sensitivities. And the IVP/USG identified 75.3% sensitivities. CONCLUSIONS: Recently, the 99mTc-DMSA renal scan has replaced IVP/USG in detecting renal scarring. And we show that 99mTc-DMSA SPECT studies are superior to IVP/USG, 99mTc-DMSA planar studies in detecting renal scarring. We believe that the role of 99mTc-DMSA SPECT will become increasingly important in detecting renal scarring of VUR.

7.6 Cases of Single Ectopic Ureter in Children.

PURPOSE: Single ectopic ureter is ralatively rare disease. We reviewed 6 cases of single ectopic ureter in children from 1989 and 1997. MATERIALS AND METHODS: Herein 5 females and 1 male are described with single ectopic ureter associated with a ipsilateral small, hydronephrotic or dysplastic kidney. Six patients varied in age from one month through 11 years. Male to female ratio was 1 to 5. RESULTS: The chief complaints were total incontinence in 4 and urinary tract infection in 1. One patient had a prenatally diagnosed hydronephrosis with ultrasonography. Ectopic ureteral orifice were identified at the vagina in 4, at the bladder neck in 1 and posterior urethra in 1 patient. The treatment methods were nephrectomy in 4, transurethral incision of ureterocele in 1 and bilateral ureteroneocystostomy in 1. CONCLUSIONS: The careful history taking, physical examination and the corresponding radiologic studies are essential in diagnosis of single ectopic ureter. And proper operative method may provide excellent results.

8.Suspension Suture Materials and Suture Carrier in Modified Gittes Bladder Neck Suspension.

We reviewed 42 cases (mean age 48.3+/-7.6 years, range 30 to 63 years) who underwent the modified Gittes bladder neck suspension for female stress urinary incontinence between January 1990 and May 1995 to evaluate the efficacy of two different suspension suture materials and new suspension suture carrier. At first we used the No. 1-0 Prolene as suspension suture in 24 cases (group I) and secondly No. 1-0 Ethibond in 18 cases (group II) with Ostycut bone biopsy needle (Angiomed) as suture carrier Following results were obtained. 1. The patient's ages ranged from 30 to 63 years (mean age 48.3+/-7.6 years), and most patients were multiparous with an average of 3.3+/-1.2 deliveries and we classified the patients according to Blaivas classification and there was statistically no difference of variables in both groups except the parity. 2. The mean values of operative time, postoperative days on catheter, and postoperative hospitalization were 65.5+/-11.4 minutes, 6.9+/-1.6 days, 7.3+/-1.4 days respectively. 3. The postoperative complications were transient urinary retention 8 cases (21.1%), vaginitis or cystitis 5 cases (11.9%), wound bleeding 1 case (2.4%) and deviation of urinary steam 1 case (2.4%). 4. The success rate was 83.3% in the group I, 94.4% in group II with minimum followup of 12 months (mean 24.6+/-5.0 months) but the latter wasn't significantly higher than the former statistically. The overall success rate was 88.1%. 5. The Ostycut bone biopsy needle as suspension suture carrier was simple and safe in the bladder neck suspension treatment. With above results, we recommend the No. 1-0 Ethibond as suspension suture material rather than the Prolene in bladder neck suspension and Ostycut bone biopsy needle as suspension suture carrier.

10.Gait Analysis after Total Knee Arthroplasty.

In order to identify the correlations between clinical results and quantitative data of gait analysis, we analyzed the results of 20 cases of total knee joint replacement arthroplasty in 15 patients with degenerative arthritis. We also evaluated the gait analysis of ten age-matched healthy candidates as a control group. Mean follow-up periods were 30 months. Clinical results included post-operative HSS (Hospital for Special Surgery) knee rating scores and the changes of the tibiofemoral angles. The three dimensional gait analysis included clinical assessment, video-taping, three dimensional kinematics and kinetics. The three dimensional kinematics were obtained using a 5 camera VICON system, and the three dimensional kinetic data was collected using two AMTI force plates. There was no statistical difference in linear parameters between the patient and control group. In patients group, however, double support time decreased as the HSS score increased, and range of knee motion and maximum knee flexion increased in accordance with the increase of pain score. Kinematic data of the patients group revealed that some parameters, such as knee flexion during loading response, knee flexion in swing phase, and knee varus during swing phase, were decreased. On the other hand, internal rotation of the knee from initial contact to initial swing was increased when compared with that of control group. There was no significant correlation between the degrees of tibiofemoral angle and coronal plane moment in the patients group. In three cases which showed mild varus instability post-operatively, knee flexion during loading response decreased and valgus moment in midstance increased as compared with the cases without instability. We believe that three dimensional gait analysis will be a good modality for evaluation of the results after total knee arthroplasty. With further accumulation of long term. follow-up data of gait analysis, we might be able to predict the long term results of total knee arthroplasty including possibility of loosening.
