1.Clinical Study of Borderline Malignant Tumor of Ovary.

Borderline malignant ovarian tumor are characterized as neoplasrns exhibiting cellular priliferative changes greater than those seen in the benign form, but not showing destnictive invasion of the ovarian strama. We reviewed 14 cases of borderline ovarian tumor who had been diagnosed and treated at Kyung Pook university hospital from 1987 to 1992. We nnalysed the clinical features, treatment and prognosis which were compared to 24 cases of the epithelial ovarian cancer at the same period The most common histological thpe of boderline tumnr was mucinous type (1 l cases, 79%), the next one was serous type (3 cases, 21%). In FIGO stage of Borderline tumor, 11 caseS weve stage I a, 1 with stage II c, 1 with stage gb and 1 with stage lV, and the FIGO stage of 24 cases of ovarian cancer was 8 cases with stage I a, 1 with stage I b, 2 with stage I c, 1 with II c, 2 with stage III a, 1 with stage III b, 7 with stage III c and 2 with stage 1V. The mean age of borderline and malignant tumor was 37.1 and 47.6 years.. 3 cases were being pregnant on the operatian. In the 14 cases of borderline turnor, 7 cases were managed by conservative operation, but 1 case recurred 4 years later so extirpative operation and pastoperative chemotherapy were given. The other 4 cases were managed by extirpative operation but postoperative chemotherapy was given in only 1 patient. Follow up ranged from 7 to 72 months. 12 cases were alive. 2 cases died, one was stage III b, the other was atage IV. This atudy suggeat, that it is neceasary to follow up carefully even the early stage of botderline tumor and manage as oyarian carcinama in the advanced stage.

4.CT Differentiation of Malignant and Inflammatory Lesion Involving Cecum.

PURPOSE: To evaluate the morphological differences between malignant and inflammatory lesions that arise from the cecal or pericecal region on CT by analyzing not only the mass itself but also the changes of surrounding structures. SUBJECTS AND METHODS: We reviewed CT scans of 38 cases of cecal lesions confirmed by pathology(16 malignant lesions and 22 inflammatory lesions). The analytical points were :the changes of bowel wall mass, the changes of surrounding structures, strands of retroperitoneal fat, pericecal fluid collection, and regional lymphadenopathy. RESULTS: The malignant bowel wall thickening(18.0mm) was thicker than inflammatory one(ll.4mm)(p < 0.001). Concentric bowel wall thickening was seen in 87.5%(14/16) of malignant lesions and 36%(8/22) of the inflammatory lesions. The pericecal fat stranding was circumferential in 84%(16/22) of inflammation and eccentric in 64%(916) of malignancy(p < 0.01). Pericolic fat infiltration was more extensive in inflammatory lesions(p < 0.005). The strands of retroperitoneal fat were more frequently found in inflammatory lesions(p < 0.05). The pericecal fluid collection was seen in 55%(12/22) of inflammatory lesions and none of malignant lesions. There was no difference in the presence of pericecal lymphadenopathy between the two groups. CONCLUSION: Malignant cecal masses have thicker and concentric bowel wall thickening, and narrower and eccentric pericolic fat infiltration. On the other hand, inflammatory masses have relatively thinner and eccentric bowel wall thickening, and more extensive and circumferential pericolic fat infiltration sometimes accompanied by abnormal fluid collection.

5.Voiding Pattern and Incontinence after Orthotopic Ileal Neobladder in Male Patients.

PURPOSE: To report the voiding pattern and continence stati in male patients having undergone a radical cystectomy and orthotopic bladder substitution. MATERIALS AND MATHODS: Between March 2000 and June 2005, 20 men with advanced bladder cancer had a radical cystoprostatectomy and urinary diversion via an ileal orthotopic bladder substitution. The voiding pattern and continence stati were assessed using patient interviews, frequency- volume charts and pad tests at 1, 3, 6, 9 and 12 months after catheter removal. RESULTS: The mean age and follow-up were 60.5 years (41-72) and 34.1 months (12-69), respectively. Neobladder substitution was performed with an ileum in all patients. 17 patients had reconstruction with a Studer neobladder and 3 with a W-pouch Hautmann. All patients had transitional cell carcinoma (TCC) and their tumor stages ranged from T1G3 to T4aN1M0. The frequency of voiding ranged from 4 to 12 times during the day, and 0 to 5 times during the night, with a mean maximum neobladder capacity of 336ml (200-620) and maximum flow rate of 24.6ml/sec (12-46) 12 months after removal of the catheter. No patient required intermittent self-catheterization. All-time continence rates were 15, 30, 65 and 85%, and all-time incontinence rates were 55, 25, 10 and 5% at 1, 3, 6 and 9 months after catheter removal, respectively. CONCLUSIONS: Orthotopic bladder substitution has an excellent functional outcome over time, resulting in high daytime and nighttime continence rates within 9 months of catheter removal.

6.Health Effects of Air Particle Produced in a Waste Treatment Plant on Residents Living in an Official Residence of Air Base.

BACKGROUND: This study was conducted to reveal the health effects of air pollution on the residents living near waste treatment plant. METHODS: We sampled 190 residents as the hogh exposed group, 192 residents as the moderate group, and 139 residents as the low exposed group according to the geographical distances from the waste treatment plant. Self-administered questionnaires for prevalence of respiratory disease, irritation symptoms of mucous membrane (eye, nose, thoat, skin, and respiratory tract), gastrointestinal symptoms, other nonspecific symptoms were conducted. We measured the concentrations of total suspened partcles, Zn, Cu, Cr, Ni, Pb, Cd and As in 3 exposed area. RESULT: Airborne concentrations of total suspended particles measured in exposed area were 485, 365, and 332 micro gram/m(3) which excees the limit value. Airborne concentrations of Zn, Cu, Cr, Ni, Pb, Cd and As were not excees the limit value. The prevalence of respiratory disease of high exposed group (16.8%) was significantly higher than that of moderate group (3.6%) and low exposed group (5.0%). The sysptom score of irrtation sysptoms of mucous membrane, gastrointestinal sysptoms, and other nonspecific sysptoms were increased with exposed level. In multiple linear regression, exposure level was related to all sysptom scores significantly. CONCLUSION: These results suggest that the air pollution of waste treatment plant would influence prevalence of respiratory disease, induce irritation of mucous membrane, gastrointestinal symptoms, and other nonspecific sysptoms.

7.Risk Factors of Ileus following Radical Cystectomy.

PURPOSE: Ileus is the most common complication following radical cystectomy. Ileus causes prolonged fasting, significant patient discomfort and prolongation of the hospital stay. We retrospectively examined the risk factors for ileus following radical cystectomy MATERIALS AND METHODS: Between January, 2000 and June, 2007, 59 patients underwent radical cystectomy with urinary diversion at our institution. 30 patients underwent creation of an ileal conduit and 29 patients underwent orthotopic bladder substitution with using ileum. Ileus was defined as the persistent absence of flatus on postoperative day 7 or the generalized findings of ileus on simple abdominal film. The potential risk factors were analyzed between the ileus and non ileus groups, including the patient's age and gender, the American Society of Anesthesiologists score, the preoperative serum albumin level, prior abdominal surgery, the operative time, combined operation, the estimated blood loss, the type of urinary diversion, use of N2O, use of intestinal stapler, complications, the ICU stay and the pathologic stage and grade. RESULTS: 24%(14/59) of patients had postoperative ileus. The difference of the ASA score between the two groups was statistically significant (p=0.03). Patients with ileus(67.5+/-7.2) were older than the non-ileus patients(63.0+/-9.4)(p=0.103). Ileus was more frequent in men(29.5%, 13/ 44) than in women(6.6%, 1/15)(p=0.09). CONCLUSIONS: Ileus following radical cystectomy was more frequent in patients with a high ASA score. Older aged men tend to have ileus after radical cystectomy.

8.Epidemiological study of clonorchiasis and metagonimiasis along the Geum-gang (River) in Okcheon-gun (County), Korea.

The endemic status of clonorchiasis and metagonimiasis along the Geum-gang (River) in Okcheon-gun (County) in Korea was examined. From February to December 2000, stools of total 1,081 inhabitants living in 5 villages were examined. Each stool specimen was examined by both the cellophane thick smear method and the formalin-ether sedimentation technique. Egg-positive cases were further analyzed by Stoll's egg-counting technique, and praziquantel was administered to positive cases. The egg-positive rates for Clonorchis sinensis and Metagonimus species were 9.3% and 5.5%, respectively, and the double infection rate was 3.5%. The numbers of eggs per gram (EPG) of feces of C. sinensis and Metagonimus sp. were 918 +/-1.463 and 711+/-947, respectively. The egg-positive rates for C. sinensis and Metagonimus sp. in the riverside area were 14.2% and 8.4%, respectively, which were significantly higher than those of the inland area (3.2% and 1.7%, respectively). The egg-positive rates of C. sinensis and Metagonimus sp. in males (16.7% and 10.0%) were significantly higher than those of females (3.5% and 1.8%). However, there were no significant differences of EPG values between localities and sexes. The prevalence of clonorchiasis and metagonimiasis in this survey was significantly lower than that in the previous reports. However, there is still a high prevalence of infection with C. sinensis and Metagonimus sp. in this region, especially in the riverside area.

9.Prostate Specific Membrane Antigen Expression in Renal Cell Carcinoma and Transitional Cell Carcinoma.

PURPOSE: Prostate specific membrane antigen (PSMA) has been used in molecular staging of prostatic cancer and known as a more sensitive marker than PSA (prostate specific antigen). Recently it has been known that PSMA is expressed in various tissue including kidney, bladder, testis, duodenum, breast, colon, etc. We performed RT-PCR (reverse transcriptase-polymerase chain reaction) assay for PSMA to evalute whether PSMA is expressed in renal cell carcinoma and bladder transitional cell carcinoma or not. MATERIALS AND METHODS: RNA was extracted from renal cell carcinoma cell lines (Caki-1, Caki-2, 7860, A498, ACHN, and 769-P), peripheral blood of metastatic renal cell carcinoma patients (7), peripheral blood of metastatic bladder transitional cell carcinoma patients (8), normal urothelium, bladder transitional cell carcinoma cell lines (RT4, 5637, HT1376, T24, TccSup) and prostatic cancer cell lines (LNCaP, DuCaP). These RNAs were analyzed by nested RT-PCR assay for the expression of PSMA. 196 bp nested PCR product was excised from the gel, cloned, and sequenced. RESULTS: PSMA mRNA was expressed in Caki-1, peripheral blood of metastatic renal cell ca (4/7, P=0.006), peripheral blood of metastatic blader trnasitional cell carcinoma patient (3/8, P=0.032), HT1376, LNCaP, and DuCaP. Automated sequencing analysis showed that all sequences were identical with PSMA mRNA sequence. CONCLUSIONS: PSMA is expressed in renal cell carcinoma and bladder transitional cell carcinoma besides prostatic cancer. This result suggests the possibility that PSMA expression can be a new biomarker for the renal cell carcinoma and bladder transitional cell carcinoma.
