2.Continuous Infusion of Midazolam for Short-term Sedation in Critically III Patients.

BACKGROUND: The mechanically ventilated patient's stay in the emergency department (ED) is brief but ventilatory management and intervention must be optimal. Agitation, anxiety, distress, and discomfort are common complications far the ventilated patient. So, sedation is necessary to reduce these complications. METHODS: Patients were monitored and mechanically ventilated. Midazolam was intravenously administered as loading dose(3-5mg), followed by continuous infusion at a rate of 0.06mg/kg/hr, titrated to achieve a predetermined sedation level (Ramsay's sedation score: 2-5 points, Glasgow coma score scathe : 8-11 points). RESULTS: After onset of predetermined sedation, systolic and diastolic blood pressure and heart rate remained stable during continuous infusion when compared with those of the initial state. Arterial blood gas and peak inspiratory pressure remained unchanged but respiratory rate was decreased to allowable ranges. CONCLUSION: A short-term continuous infusion of midazolam was a safe and erective method for sedation of ventilated patients in ED.

3.Biometric Analysis of the Normal Aged Eye with Photodocumentation Method.

The biometric analysis of the normal aged eye with photodocumented method (using EAS-lOOO Scheimpflug camera) was performed in 109 eyes. We measured anterior chamber depth, corneal thickness, corneal radius, lens thickness and biometric lens analysis according to the age. The lens thickness increased annually (O.O1mm) with age and the anterior chamber depth reversely decreased. The biometric analysis can provide normal critariae values about the effect of anticatar acto us drugs in long-term follow up study and the relationships between normal aged lens and highly risk groups in the cataract prevalence study.

5.Hypocholesterolemia and neurotic symptoms.

BACKGROUND: An association between low serum cholesterol concentrations and depressive symtoms has ever been found in several studies. The objective of this study is to evaluate neurotic symptoms in hypocholesterolemia. METHODS: In the course of two months (from June to August, 1998), 81 subjects were selected among the people who screened in adult disease survey of one university hospital. As the control group, 80 subjects were selected from a healthy population. In the study population, we excluded people who had medical history of hypertension, diabetes mellitus, ischemic heart disease, chronic liver disease and history of any medication to lipid metabolism and other psychotic disease history. Neurotic symptoms were measured by Symptom Checklist for Minor Psychiatric Disorders (SCL-MPD) questionnaire. RESULTS: There was no difference between the distribution of age and sex in the hypocholesterolemia groups. As the level of the education and kind of employment, no significant difference between two groups. Hypocholesterolemia groups scored significantly higher than conrol group in some questionnaire of SCL-MPD. The results of depression scale (p<0.01) and anxiety, hypochondriasis scale (p<0.05) were significant. The men of hypocholesterolemia groups have significant relation to hypochondriasis scale (p<0.001) and the women to anxiety scale (p<0.05). CONCLUSION: Though in some scale of neurotic symptoms, the association between hypocholesterolemia and neurotic symptoms was found in our study. There need to be a further study on the correlation of hypocholesterolemia and neurotic symptoms.

6.Amniotic Membrane Transplantation in Perforation or Impending Perforation of Cornea.

To evaluate the efficacy of amniotic membrane ransplantation(AMT) in the cornea that was perforated or impending perforation due to recalcitrant corneal ulcer or trauma, we performed AMT on 1 case of acid burn, 2 cases of thermal burn, 1 case of corneoscleral staphyloma with bullae due to previous ocular trauma, 2 cases of keratolysis as a graft rejection after keratoplasty, 3 cases of corneal ulcer. We used one or several layers of amniotic membrane(AM) that preserved in DMEM solution and 10% glycerin(1:1) under-70 degrees C or lyophylized form. As a result, the corneal perforations were prevented and the stable ocular surface were achieved. However, in case of neurotrophic keratopathy, epithelial rebreakdown was occurred after AMT, eventually epithelalization was fulfilled. Therefore, AMT may be a effective alternative method for treating corneal perforation or impending perforation that are refractory to conventional treatment.

8.Orbital floor and infraorbital rim reconstruction with vascularized calvarial bone flap.

A number of methods have been introduced for support the orbital floor following a maxillectomy without orbital exenteration or severe facial trauma. These methods including skin graft and muscular sling provided the unsatisfactory results, like as diplopia, orbital ptosis, enophthalmos and severe facial deformity. Therefore the bone and soft tissue reconstructions using microvascular free flaps were performed recently by many surgeons, but long time operation, donor site morbidity, postoperative large scar, and ptosis of the flap were pointed out as disadvantages of free flap reconstruction. Vascularized calvarial bone flap, a modified method of free calvarial bone graft, was adequate for reconstruction of the orbital floor and the infraorbital rim as a horizontal buttress, especially in case of poor vascular bed and postradiated state. The authors introduced the vascularized calvarial bone flap for the orbital floor and the infraorbital rim reconstruction in 3 cases of maxillectomy, and could be obtained satisfactory results aesthetically and functionally.

9.Ultrastructure of the Experimentally Induced Cataract in Porcine Crystalline Lens.

One of the purposes of this experiment is to observe the structure of crystalline lens with cataract, which is formed artificially, using the light microscope and electron microscope. The other is to observe the differences of structural variations in the cataract developed inside body. Twelve eyes of six pigs were used for this experiment. Two of them are normal crystalline lens, five are cystalline lens in distilled water, and the rest are in balanced salt solution through intact or ruptured capsule. We examined the time of cataract formation and compared the ultrastructural changes. Ruptured capsule and high osmotic pressure difference induced more rapid opacity. In case of distilled water, the capsule is maintained but it is very difficult to distinguish between epithelium and lens fibers. Also, there is a severe crack in the lens fibers. In electron microscopic, as the cataract progresses, the osmotic swelling becomes more prominent. One of the striking changes was a marked intercellular cyst formation. Lens cells became irregular in size and density and were extensively vacuolated. The swelling of the lens cells continuously induced large intracellular vacuoles and liquefied the cytoplasmic protein. In conclusion, the structural change of cataract, which was seen by an electron microscope, resulted from change in osmolarity from previously announced in vivo experiment and structural change resulted from this experiment are similar.

10.The Effect of The IOL Position Studied by Using Scheimpflug Camera to The Postop Astigmatism.

Management of surgically induced astigmatism is the major problem for surgeons implanting intraocular lenses. Besides corneal astigmatism, the fixation status of the intraocular lens(IOL) may contribute to postoperative astigmatism. This study was undertaken to analyze whether the variable factors such as fixation status, IOL type, and capsulotomy method affect tilting and decen tration of IOL. The tilting angle and decentration of the IOL were measured by image-processing technique using computerized Scheimpflug camera. The average tilting angle was 4.30 +/- 2.21 degree. The average decentration was 0.44 +/- 0.36mm from the corneal center. Based on these data, the astigmatic error induced by the tilting and/or decentration of the implanted IOL was calculated as within 0.1 diopter. Haptic fixation and designs were statistically significant for IOL posit ion but tilting and decentration were not significantly associated with capsulotomy method, agegroup, sex, and postoperative day.
