1.MR Imaging of the Early Rheumatoid Arthritis: Usefulness of Contrast Enhanced Fat Suppressed SPGR Imaging.

PURPOSE: To evaluate value of post-contrast 3-Dimensional fat suppressed Spoiled GRASS (FS SPGR) in detectiong subtle bony erosion and tenosynovitis of hands and wrists due to early rheumatoid arthritis. MATERIALS AND METHODS: Fourteen MR iraagings of the hands and wrists were performed in 7 early rheu- matoid arthritis without any abnormalities in plain radiography and in 7 healthy volunteers. All subjects underwent MR sequence of coronal 3D FS SPGR with and without contrast enhancornear in 1.5T MR unit. We evaluated the number of the bony erosion and tenosynovitis respectively in pre- and post-contrast FS SPGR images. RESULTS: The abnormal enhancing areas were not demonstrated in 7 healthy volunteers. Seven patients had 25 bony erosions in pre-contrast FS SPGR and 52 bony erosions with tenosynovitis (n=10) in post-contrast FS SPGR. Enhancing joint spaces were shown in 8 cases CONCLUSION: Post-contrast FS SPGR was better than pre-contrast FS SPGR in the evaluation of early rheumaotid arthritis and is valuable as a baseline study.

3.Clinical Analysis of 8 Cases of Chronic Carbon Disulfide Poisoning in Workers Engaged in the Viscose Rayon Industry.

The authors have experinced 8 cases of chronic carbon disulfide poisoning in workers engaged in the viacase rayon industry during the period between March 1987 and September 1988. Cases of carbon disulfide poisoning has not been reported in Korea till then. Carbon disulfide is primarily a neurotoxic poison, therefore those symptoms indicating central and peripheral nervous damage are very important. And it has toxic effects on the various organs such as skin, eye, ear, and respiratory, cardiovascular, gastrointestinal, hepatic, genitourinary, hematological, and endocrine system. Occupational histories were obtained, and clinical findings and laboratory findings were analysed on all cases. 1. Seven (7) cases had been exposed to carbon disulfide for more than 10 years, and 1 case has been exposed for 6 years. 2. Most common symptoms were neurological symptoms such as gait disturbance, extremity weakness, paresthesia, and speech disturbance. 3. Involvement of central nervous system was noted in 7 cases through abnormal findings of electroencephalography, brain computerized tomography, neurological signs, and psychological test. Needle electromyography and nerve conduction study showed polyneuropathy in all cases. Ophthalmological abnormalities were found in 7 cases, mainly retinal microaneurysms and peripheral constriction of the visual field. Renal damage appeared in 4 cases, increase of serum cholesterol and/or triglyceride level appeared in 3 cases, and dercrease of erythrocyte and hemoglobin appeared in 2 cases. And hypertenison appeared in 2 cases, and diabetes appeared in 1 case. One(1) case who has been exposed tn carbon disulfide for 6 years showed polyneuropathy and peripheral constriction of the visual field.

6.Concurrent Validity of the Seven-Item BBS-3P with Other Clinical Measures of Balance in a Sample of Stroke Patients.

PURPOSE: The aim of this study was to investigate if the 7–item Berg balance scale (BBS) 3–point, which is a short form of the BBS (SFBBS), has compatible psychometric properties in comparison with the original BBS, and also to study the concurrent validity using a 10-meter walk test (10mWT) and a timed up and go test (TUG), which are widely used with SFBBS in clinical settings. METHODS: A total of 255 patients who had experienced stroke participated in this cross-sectional study. We used results obtained from 188 patients who completed both 10mWT and TUG. The three levels in the center of the BBS were collapsed to a single level (i.e.,0-2-4) to form the SFBBS. The concurrent validity was assessed by computing the Spearman coefficients for correlation among outcome measures and in between each outcome measure and the SFBBS. As there were four outcomes, the corrected p-value for significant correlation was 0.013 (0.05/4). RESULTS: Spearman coefficients for correlations and evaluation instruments for concurrent validity revealed significantly high validity for both of SFBBS and BBS (r=0.944). 10mWT and TUG were −0.749 and −0.770 respectively, which are in the high margin and are statistically significant (p>0.000). CONCLUSION: SFBBS has sound psychometric properties for evaluating patients with stroke. Thus, we recommend the use of SFBBS in both clinical and research settings.

7.Acute Shunt Malfunction after Cesarean Section Delivery: A Case Report.

Shunt malfunctions that require surgical intervention during pregnancy and the postpartum period are rare. Furthermore, no study has reported on an acute shunt malfunction immediately after cesarean section. Here, we describe the case of a 32-yr-old woman who became drowsy 12 hr after cesarean section delivery of her second child. She had a ventriculoperitoneal shunt placed to treat hydrocephalus associated with meningitis at 26 yr of age. Marked ventriculomegaly was seen on brain computed tomography and her consciousness recovered temporarily after aspirating cerebrospinal fluid from the flushing device. At surgery, the distal catheter tip was plugged by a blood clot. We believe that the blood spilled over during the cesarean section. The clogged catheter end was simply cut off and the remaining catheter was repositioned in the peritoneal cavity. Her consciousness recovered fully.

9.Evaluation of the Effect of Various Root Planing Instruments on the Root Surfaces.

The purpose of this study was to evaluate the effects of periodontal curet and various rotating instruments on the root surfaces. Thirty-five extracted teeth with advanced periodontal disease were used. They was root planed with periodontal curet, periodontal Perio-Clean bur, periodontal Roto-Perio bur, resin polishing ET bur, and resin polishing diamond fissure bur. To find dentinal tubule orifices on the root surface, tetracycline HC1 solution was applied to the one tooth of treated each group. Then, root surfaces were investigated using scanning electron microscope. Amount of loss of cementum was evaluated by loss of tooth substance index. The results were as follows. 1. Groups treated with periodontal curet and Perio-Clean bur showed irregular surface and concavities. Concavities seemed to be lacunae of cementocyte. Other groups treated with Roto-Perio bur, resin polishing ET bur, and resin polishing diamond fissure bur showed partially opened dentinal tubule orifice. 2. Groups treated with periodontal curet and Perio-Clean bur and tetracycline HC1 showed irregular surface. No dentinal tubule orifice was seen. Other groups treated with Roto-Perio bur, resin polishing ET bur, and resin polishing diamond fissure bur and tetracycline HCI showed dentinal tubule orifice with various shape and size. 3. Loss of tooth substance indices were compared between groups. There was no statistically difference between periodontal curet and Perio-Clean bur groups. There were statistically differences between periodontal curet and Roto-Perio bur, ET bur, and diamond fissure bur groups. As a result of this study, groups treated with Roto-Perio bur, resin polishing ET bur, and resin polishmg diamond fissure bur showed more cementum removed than groups treated with periodontal curet and Perio-Clean bur. Therefore, in a conventional treatment for periodontal disease, it was recommended that periodontal curet or Perio-Clean bur should he used. In a treatment for regeneration of periodontal tissue, it was recommended that Roto-Perio bur, resin polishing ET bur, or resin polishing diamond fissure bur should be used

10.Histologic Study on the Effect of Two Types of Bovine Bone Powder in Extraction Socket of Beagle Dogs.

Several extraction cases with advanced bone loss as a result of periodontal disease, root or labial bone fracture, extensive caries, and periapical lesions occur esthetic, functional problems and severe bone loss. Therefore, to treat these cases used several surgical methods and socket preservation among this therapies have been evaluated simple, effective and good prognosis in the implant placement. Socket preservation therapy have been used with barrier membranes or/and graft materials. Deproteinized bovine bone mineral have been evaluated ideal grafting materials. Recently, calcium-phosphate thin film coated bovine bone powders were developed in our country, but the study for these material wasn't reported. When two types of xenograft materials were implanted in extraction sockets of Beagle dogs, the effects of these were analyzed after 4 weeks and 8 weeks histological views. The results of this study were as follows. 1.In control groups, 4 weeks after implantation, the extraction sockets were filled with connective tissue which has dilated vessels and epithelial growth. And after 8 weeks, irregular connective bundles were observed. But new bone formation was not seen. 2.In Bio-Oss groups, epithelial growth was not seen and bone powder was covered with connective tissue fiber. New bone formation was found around the interproximal bone. There was no special change seen after 8 weeks, connective tissue fibers became more regular, and bone growth near bone powder was not made well. 3.In Ca-P BBP groups, epithelial cells didn't grow in the extraction sockets, there was a lot of new bone made around the bone powder after 8 weeks, new bone around bone powder was replaced with mature bone. It is thought that bone powder grafting into the extraction sockets is very useful for conservation of ridge, and Ca-P BBP is more effective in bone formation than Bio-Oss.
