Objective To evaluate the efficacy of ultrasound-guided transversus abdominis plane (TAP) block for postoperative analgesia in patients undergoing inguinal hernia repair.Methods Forty ASA Ⅰ or Ⅱ patients,aged 18-79 yr,with body mass index ＜ 30 kg/m2,scheduled for elective unilateral inguinal hernia repair combined spinal-epidural anesthesia,were randomly divided into 2 groups (n =20 each):control group (group C) and ultrasound-guided TAP block group (group B).The ultrasound-guided TAP block was performed at the end of surgery and 20 ml of 0.375 ％ ropivacaine was injected in group B,while the equal volume of normal saline was given in group C.Tramadol was injected intravenously when VAS score ≥ 4 after surgery.VAS scores at rest and during activity were recorded at 4,6,24 and 48 h after surgery.The warm block plane on the blocked side was measured at 24 and 48 h after surgery.The overall satisfaction on analgesia was scored and the time when the patients passed the flatus was recorded.TAP block-related side effects were recorded.Results Four patients required tramadol in group C,while no patients required rescue analgesic in group B.Compared with group C,VAS scores were significantly decreased,the overall satisfaction scores were increased (P ＜ 0.05 or 0.01),and no significant change was found in the time when the patients passed the flatus in group B (P ＞ 0.05).The rate of warm plane block on the blocked side was 80％ at 24 h after surgery and there was not warm block plane in patients at 48 h after surgery in group B.There was not warm block plane in patients at 24 and 48 h after surgery in group C.TAP block-related side effects were not found in group B.Conclusion The efficacy of ultrasound-guided TAP block for postoperative analgesia is better in patients undergoing inguinal hernia repair and the safety is higher.

The PBL Teaching Method has implemented in the department of gynecology and obstetrics of Fudan University since 2004.This new method cultivates the students'ability to ask and answer the questions to collaborate,to comiderate problems conpletely and the ability to consult information and solve problems quickly.Teachers and department should improve the teaching and manage level.

In this paper, a new imaging method to determine the position of blood effusions or edemas in the deep position of the brain by using electric current flowing through the brain is presented. Its principle is that the existence of non-normal matters in the brain cause a disturbance to boundary potentials, while a low frenqency current flows through the brain. In terms of the measurement for the change value of boundary potentials a reconstruction for the blood effusion or the edema in the deep position of the brain can be done.

Objective To analyze the dermatology journals indexed by SCI‐E and papers published in recent 5 years ,to pro‐vide advices on periodical selection and contribution .Methods We mainly adopted the bibliometrics methods and word frequency a‐nalysis method to analyze 10 targets in 62 SCI‐E dermatology journals .Results Main publishing nationality were United States , England ,Denmark and Germany ,publishing language was English ,main publishing cycle period was bimonthly journals ,the highest impact factor was 6 .372 .The theme of published content was relatively concentrated ,reviewers′duration was fast and reasonable , 80 .7% submission of the journal was easy .Main type of literature published in recent 5 years is article ,review ,editorial and letter . 58 SCI‐E dermatology journals accept articles from China .Conclusion We suggest that the authors should firstly check the name of dermatology and cross‐disciplinary journals indexed by SCI‐E ,and evaluate the theme of published content ,rate of publishing article contributed from China ,difficulty degree for contribution ,reviewers′duration ,publishing cycle and impact factor when contribution articles .

Objective: To investigate the characteristics of the breakthrough cases of mumps in Ningbo City, Zhejiang Province from 2018 to 2023, so as to provide insights into improving prevention and control measures for mumps. Methods: Data of mumps cases and mumps containing vaccine (MuCV) vaccination in Ningbo City from 2018 to 2023 were collected through the Infectious Disease Reporting Information System of Chinese Disease Prevention and Control Information System and Zhejiang Immunization Planning Information System. The population distribution characteristics and MuCV immunization history of mumps breakthrough cases were described. The impacts of the final immunization age and immunization interval on the age of onset were analyzed. Results: A total of 6 643 mumps cases were reported in Ningbo City from 2018 to 2023, with an average incidence rate of 11.72/105. There were 5 142 breakthrough cases (77.40%), including 3 173 males (61.71%) and 1 969 females (38.29%). The median age of onset was 6.00 (interquartile range, 4.00) years. There were 2 487 cases in preschool children (48.37%) and 2 232 cases in students (43.41%). There were 4 736 one-dose breakthrough cases (92.10%) and 406 two-dose breakthrough cases (7.90%). The proportion of two-dose breakthrough cases among all mumps cases increased from 1.00% in 2018 to 25.32% in 2023. Among individuals born after December 2018, the median age of onset of two-dose breakthrough cases was 3.00 (interquartile range, 1.00) years, which was older than that of one-dose breakthrough cases at 1.00 (interquartile range, 2.00) year (P<0.05). The ages of onset of mumps breakthrough cases differed significantly with varying final immunization ages and immunization intervals (both P<0.05). Conclusions The breakthrough cases of mumps in Ningbo City from 2018 to 2023 were mainly males, preschool children and students. The proportion of two-dose breakthrough cases increased, and the age of onset delayed.

Objectives To investigate the effects of nerve nutrient factors and inhibitory factors on neuronal regen? eration and axonal reconstruction at ischemic neocortical penumbra after focal cerebral ischemia in rats. Methods The rat model of middle cerebral artery ischemia was established using suture-occluded method. The pathological morpholo? gy of brain tissue was examined by using HE staining. The expression levels of GAP-43 SYN, nutrient factor (BDNF VEGF Ang1) and inhibitory factor (NogoA NogoR RhoA) were determined by using Western blotting technique. Results The number of neurons in ischemic penumbra was significantly decreased in model group (P<0.01) than in sham-operat? ed group. The expression levels of GAP-43 were significantly decreased (P<0.05) while the expression levels of NogoA NgR and RhoA in the thalamus were significantly increased (P<0.05) in model group than in sham-operated group. The expression levels of SYN and nutrient factors (BDNF VEGF Ang1) were not different between the model group and sham-operated group. Conclusion The increase in nerve inhibitory factors may contribute to the down-regulation of neu?rogenesis at ischemic neocortical penumbra after focal cerebral ischemia in rats.

Objective To explore the clinical significance of early biomarkers in neonatal asphyxia diagnosis with myocardial damage by detection of ischemia modified albumin in neonatal serum(IMA) and glycogen phosphorylase isoenzyme BB(GPBB) for screening sensitive markers with direct myocardial injury.Methods 166 neonates were selected in our hospital as the research object,in which 136 cases with myocardial injury dividend into the experimental group and 30 cases into the control group.The experimental group were divided into mild group and severe group according to the degree of asphyxia.All the children were tested for GPBB and IMA 1 h after admission.Results The levels of GPBB in neonatal asphyxia myocardial injury group and severe asphyxia group were significantly higher than those in the control group,the difference was statistically significant(P<0.05).The sensitivity of GPBB in diagnosis of asphyxia was higher than that of IMA,the difference was statistically significant(P<0.05).Conclusion The degree of asphyxia is closely related to serum IMA,GPBB level in neonatal asphyxia complicated with myocardial injury.The sensitivity and specificity of GPBB in diagnosis asphyxia is better than IMA in children complicated with myocardial damage.

Objective:To investigate the feasibility and accuracy of osmometer in the osmolarity measurement of pirenoxinun sodi-um eye drops. Methods:An osmometer was used to measure the osmolarity of pirenoxinun sodium eye drops, and the stability of the osmolality was studied. Results:The osmolarity of pirenoxinun sodium eye drops with different formula conformed to the requirement of ophthalmic preparations. The osmolality showed no significant change at the ambient temperature, indicating the stability of the prepa-ration. Conclusion:The method is simple, rapid and practical, which can be used as the osmolarity measurement method for pirenoxi-nun sodium eye drops.

Objective To explore the significance of combined detection of tumor markers cancer embryo antigen (CEA) ,alpha fetoprotein (AFP ) ,carbohydrate antigen 19-9 (CA19-9 ) and carbohydrate antigen 72-4 (CA72-4 ) in digestive tract tumors . Methods The electrochemical luminescence method was adopted to detected the CEA ,AFP ,CA19-9 and CA72-4 levels in 106 cases of digestive tract malignant tumor confirmed by pathological examination (malignant tumor group) ,110 cases of digestive tract be-nign diseases (benign disease group) and 60 persons undergoing the healthy physical examination (control group) in our hospital from January to December 2015 .The differences were compared among various groups and the positive detection rates of 4 markers in the patients with digestive tract tumors were compared .Results The serum levels of CEA ,AFP ,CA19-9 and CA72-4 levels in the malignant tumor group were significantly higher than those in the benign disease group and control group(P<0 .05);the benign disease group and normal control group had no statistically significant difference (P>0 .05) .In the single indicator detection of ser-um CEA ,AFP ,CA19-9 and CA72-4 ,AFP had the highest detection rate in hepatocellular carcinoma (HCC) ,which was 74 .19% ;the positive detection rate of CA 72-4 in gastric cancer was highest ,which was 60 .71% ;the positive detection rate of CA 19-9 in pan-creatic cancer was highest ,which was 75 .00% ;the positive detection rate of CEA was not high without specificity in various tumor diseases .The positive detection rate of 4-item combined detection was significantly higher than that of single item detection (P<0 .05) .Conclusion The detection of tumor markers CEA ,AFP ,CA19-9 and CA72-4 is conducive to the identification of tumor types ,meanwhile they can improve the detection rate of gastrointestinal cancer and conduces to early diagnosis and early treatment for the patients .
