1.Application and exploration of the pervasive teaching method in medical humanity education

ObjectiveTo explore an effective educational method of medical humanity to improve the medical humanistic quality for nursing students and to lay a basis for the culture of medical personnel with high-quality.MethodsTwo classes of nursing students in grade 2010 were selected randomly and divided into the control group ( CG,n=65 ) and the experimental group ( EG,n=62 ).Personnel training project of our school was applied in the control group including the teaching of compulsory basis medical humanity courses:medical psychology,interpersonal communication,medical ethics and laws from the first semester to the third semester after entering the school.The pervasive teaching method was used in the EG.The basic content of humanistic quality,education was integrated systematically into the teaching of every course,class and extra-curricular activity in a planned way.The check on the topic of etiquette,verbal expression and communication and psychological quality ( the full mark of each topic was 10 points ) was conducted in the third semester in the two groups.The average points of the three topics were graded as the students' final mark of this course.T-test analysis on students' mark of each subject was used.Results The result of t-test showed that the marks of etiquette and psychological quality in EG were obviously higher than those in CG ( P<0.05 ),however,no statistic difference in speech communication and expression was observed.ConclusionThe pervasive teaching method is beneficial to students concerning the accepting,understanding,digesting and absorbing of medical humanity knowledge can it can effectively enhance the humanistic quality of medical students.
2.The Effect of Uvulopalatopharyngoplasty on Voice in Patients with OSAHS

Objective To study the impact of changes in the pharynx on the vocal functions and resonance functions to evaluate the effects of UPPP on voice and in order to provide a reference for the selection of UPPP sur‐gery population .Methods The 30 male adult patients who underwent UPPP surgery were recorded pre-surgery ,1-week post-surgery and 3-month post-surgery speech .Then the multi-dimensional voice program (MDVP) , linear prediction coding (LPC) ,the Nasometer Ⅱ(model 6450) were used to evaluate the vocal functions ,the oral resonance functions and the nasal resonance function .Results F0 ,jitter ,shimmer ,and NHR were unchanged from the pre-surgery to the post-surgery condition .One week after the surgery ,F1 ,F2 of /a/,/i/and /u/were un‐changed (P>0 .05) .Three months after surgery ,F1 ,F2 of the vowel /a/and F1 of the vowel /u/were remained essentially unchanged (P>0 .05);while the frequency of F1 ,F2 of the vowel /a/and of F2 of the vowel /u/were significantly higher compared to pre-surgery values (P<0 .05) .One week after the surgery ,the nasalance scores of the oral sentence ,the oranasal sentence ,and the nasal sentence were unchanged (P>0 .05) .Three months after surgery ,the nasalance scores of the oral sentence and the oranasal sentence were unchanged (P>0 .05);while the nasalance scores of the nasal sentence were significantly higher compared to pre-surgery values (P<0 .05) .Conclu‐sion UPPP doses not affect the vocal function .But it might have a certain effect on resonance function ,demonstra‐ted by the change of several formant frequency and nasalance .UPPP in all professional voice users should be cau‐tious .Such patients who want to undergo UPPP must be informed of this potential modification of the voice after sursery .

Triptolide(TP)is a diterpenoid epoxide purified from a Chinese herb Tripterygiumwilfordii Hook. f.. lt has been shown to have multiple pharmacological activities,such as anti-tumor,anti-inflammatory,immune-suppressive effects. TP can reduce airway hyperresponsiveness,decrease the proliferation of airway smooth muscle,and produce potential therapeutic effect in airway remodeling. The effect of TP on allergic asthma may be related to its inhibition of maturation of dendritic cells,rebalance of the function of Th1 and Th2,reduction of the secretion of inflammatory factors,and decrease of the infiltration of eosinophils.
4.Effects of Tri-ortho-cresyl Phosphate on Mitochondrial Membrane Permeability in Hen's Nerve Tissue

Objective To study the effects of tri-ortho-cresyl phosphate(TOCP) on mitochondrial membrane permeability in hen's nerve tissues and investigate the mechanism of the organophosphorus ester-induced delayed neurotoxicity(OPIDN).Methods Adult Roman hens were randomly divided into four groups,including three treated groups and one control group(24 in each group).The hens in the experimental groups were treated with TOCP by gavage at the single dosages of 185,375 and 750 mg/kg respectively.TOCP was dissolved in corn oil and administered at 0.65 ml/kg.The control hens received an equivalent volume of corn oil by gavage.The hens were sacrificed on the 1st,5th,15th and 21st day after treatment and the cerebrum,cerebellum,spinal cord were dissected and homogenized in ice bath.The mitochondria in these nerve tissues were extracted to determine the changes of the membrane permeability and membrane potential.Results Compared with the control group,no significant increase of the mitochondrial membrane permeability in the cerebrum was observed in treated groups.In the cerebellum,the membrane permeability in the 185 mg/kg group had no significant changes,while in the 375 and 750 mg/kg groups it increased significantly on the 1st and 5th day after TOCP treatment(P
5.Mechanism of microRNA-15a-5p in enhancing the sensitivity of glioma cells to temozolomide by modulating RUNX1

Objective To investigate the effects of microRNA-15a-5p (miR-15a-5p) on the proliferation, invasion and migration of and its mechanism of sensitivity of glioma cells to temozolomide (TMZ). Methods Glioma cell lines H4 and SHG-44, normal astrocytes HA1800 and TMZ-resistant H4 cells were selected. H4 cells were transfected with miR-NC or miR-15a-5p mimics and recorded as the miR-NC group and the miR-15a-5p group, respectively. The cells of miR-15a-5p group was further transfected with Vector or OE-RUNX1 plasmids and treated with 400 μmoL/L TMZ, and recorded as miR-15a-5p+Vector group, miR-15a-5p+RUNX1 group, TMZ+Vector group and TMZ+RUNX1 group. Additionally, the miR-NC and miR-15a-5p groups were treated with 400 μmoL/L TMZ, and recorded as TMZ+miR-NC group and TMZ+miR-15a-5p group, respectively. Untreated cells served as the Control group. Cell proliferation was assessed using the Methyl Thiazolyl Tetrazolium (MTT) assay. Invasion and migration were evaluated by Transwell assays.
6.Construction of Δ0948 complementary strain of Streptococcus suis 2 and its effect on hemolysin secretion and virulence

Objective:To construct Streptococcus suis type 2 Δ0948 complementary strain and verify its effect on suilysin (SLY) secretion and virulence. Methods:The SSU05_0948 gene sequence with promoter was amplified by PCR and ligated to pAT18 vector to construct complementary strain and verify its expression through Western blot. Growth curve was drawn to compare the growth of complementary strain against the wild-type strain and mutant strain in different periods. CD1 mice challenge model was used to verify whether complementary strain could restore the virulence of mutant. SLY hemolytic activity and Western blot were compared the effect of complementary strain and wild-type strain and mutant strain on SLY protein secretion at different time points.Results:The complementary strain was successfully constructed, but the expression of SSU05_0948 was lower than the wild-type strain. The growth rate of the complementary strain was significantly slower than the wild-type strain and mutant strain in the logarithmic growth phase, but the same in the platform phase. The CD1 mice challenge model showed the complementary strain could basically restore the virulence of the mutant strain. The hemolytic activity of SLY and Western blot showed that SSU05_0948 could inhibit the secretion of SLY protein in the early and middle logarithmic phase, but did not affect the secretion of SLY in the late logarithmic and platform phase, while the complementary strain could restore the secretion of SLY protein.Conclusions:The complementary strain CΔ0948 of Streptococcus suis can restore the virulence of mutant strain Δ0948, and SSU05_0948 affects the virulence of Δ0948, which provides a new idea for the prevention and treatment of Streptococcus suis.
7.Prevalence of extended-spectrum beta-lactamases-producing Escherichia coli and Klebsiella pneumoniae in community-onset bloodstream infections from county hospitals of Zhejiang Province

Objective To study the epidemiology and genotypes of extended-spectrum beta-lactamases (ESBL)-producing Escherichia coli (EC) and Klebsiella pneumoniae (KP) that caused community-onset bloodstream infections (COBSI) in 9 county hospitals of Zhejiang Province.Methods This is a multi-center, prospective, observational study.The cases and isolates with COBSI caused by EC and KP were consecutively collected from 9 county hospitals in Zhejiang Province between 1st March 2014 and 30th April 2015.The double disk diffusion method was used to confirm the production of ESBL.The ESBL genotypes were determined by polymerase chain reaction(PCR) amplification and sequence analysis.Multi-locus Sequence Typing (MLST) was used to analyze the homology of ESBL-producing isolates.Minimal inhibitory concentration (MIC) of frequently used drugs for ESBL-producing isolates was determined by in-vitro antimicrobial susceptibility tests.Results During the study period, a total of 172 cases with COBSI were collected and 171 cases were eligible, among which 126 were caused by EC and 45 were caused by KP.The overall prevalence of ESBL was 34.5% (59/171),and the prevalence of ESBL-EC and-KP was 41.3% (52/126) and 15.6% (7/45), respectively.CTX-M-type ESBL accounted for 96.6% (57/59) of all the ESBLs-producing isolates, and the most common type was CTX-M-14 (27.1%, 16/59), followed by CTX-M-55 (22%, 13/59).MLST analyses revealed significant genetic diversity among ESBL-EC and-KP.The most prevalent ST of ESBL-EC was ST131 (23.1%).In addition to carbapenems, cefoperazone/sulbactam, piperacillin/tazobactam, moxalactam, amikacin and fosfomycin also showed good in-vitro activity against ESBL-EC and-KP.Conclusions The prevalence of ESBL in EC and KP is high in 9 county hospitals of Zhejiang Province, and the most common genotypes are CTX-M-14 and CTX-M-55.The detection rate of ESBL in EC is higher than in KP.It could be considered adequate empirical therapy according to the results of antimicrobial susceptibility tests.Carbapenems, cefoperazone/sulbactam, piperacillin/tazobactam, moxalactam, amikacin and fosfomycin have good in-vitro activity against ESBL-EC and-KP.
9.Evaluation of single leaflet replacement of bovine pericardium treatment aortic valve insufficiency using echocardiography

Objective To evaluate the value of single leaflet replacement of bovine pericardium in the treatment of aortic valve insufficiency using transthoracic echocardiography(TTE) and transesophageal echocardiography(TEE).Methods TTE and TEE examination were performed for 25 cases pre-operation,postpump intraoperative and 3-6 months after the operation,to judge the structure and the regurgitation of the aortic valve.And the inside diameter of left atrium(LA)and left ventricle(LV)in the same plane and the same phase were also been measured.Results The degree of aortic regurgitation(AR)before operation were significantly different with that postpump intraoperative(P<0.001)evaluated by TEE.There was no difference in the degree of AR by postpump intraoperative TEE and those of AR by TTE 3-6 months after surgery(P>0.1).The inside diameter of LA[(3.80±1.37)em]and LV[(5.75±1.32)cm]before repair were larger than those 3~6 months after surgery[LA(3.19±0.90)cm,LV(4.72±1.19)cm,P<0.001].Conclusions The replacement of single aortic valve with bovine pericardium to treat aortic valve insufficiency is a good alternative for patients with satisfactory short-term results,but long-term results are not clear.
10.Expression and significance of SP-A in nasal mucosa of allergic rhinitis and nasal polyp

Objective: Surfactant protein A (SP-A) is protein that appears to play an important role in mammalian first-line host defense. The objective of this study was to immunolocalize SP-A in human sinonasal tissue. Method:Eleven cases of allergic rhinitis, fifteen cases of polyp and seven cases of normal middle turbinate were studied with immunohistochemistry and immunofluorescence method to detect the expression of SP-A. Result:The expression of SP-A in allergic rhinitis and polyp were dramatically higher than that in controls(P<0.05), and there was no remarkable difference in the expression of SP-A between allergic rhinitis and polyp(P>0.05). The result demonstrated that SP-A was positivly correlated with eosinophils within the basement membrane of epitheli-um(R=0.81,0.55). In the result of immunofluorescence, there was significantly higher expression SP-A in nasal mucosa of allergic rhinitis and nasal polyp than that in control group(P<0.05). Conclusion:SP-A is likely to play key roles in the inflammatory reaction process of allergic rhinitis and polyp. Its secretion in the upper airway indicates that future studies may allow manipulation of this protein and development of novel treatments for sinonasal pathology.
