1.Extraction and Separation of Gingerols and Assaying of [6]-gingerol

Objective To extract and separate crude gingerols and to determine the content of [6]-gingerol in crude gingerols.Methods Crude gingerols were separated from the ginger supercritical-CO2 extracts by silica gel dry column chromatography with solvent system of diethyl ether-n-hexane(7:3).The content of [6]-gingerol in crude gingerols was determined by HPLC.Results [6]-gingerol content in the prepared crude gingerols by silica gel dry column chromatography arrived 52.87 %(m/m).[6]-gingerol had a good linearity in the range of 0.512~ 3.075 ? g,r=0.999 9,and the average recovery was 99.19 %,RSD=1.58 % .Conclusion Silica gel dry column chromatography can be used to quickly,effectively prepare crude gingerols,in which [6]-gingerol content is high,and can supply enough material for further research.The liquid chromatographic analysis of [6]-gingerol is simple,reliable,reproducible and can be used for the quality control of crude gingerols.
2.Factors affecting malnutrition among elderly patients with type 2diabetes mellitus

Objective:
To investigate the factors affecting the development of malnutrition among the elderly patients with type 2 diabetes mellitus (T2DM), so as to provide insights into the prevention of malnutrition among the elderly patients with T2DM.
Methods:
Elderly T2DM patients at ages of 60 years and older admitted to Hangzhou First People's Hospital affiliated to Zhejiang University School of Medicine from June 2022 to June 2023 were recruited. Patients' demographics, number of medicines administered and comorbidities were collected using questionnaire surveys, and glycosylated hemoglobin (HbA1c), serum albumin (ALB) and hemoglobin levels were collected from medical records. The nutrition status was assessed with the new version of the Mini Nutritional Assessment Short Form (MNA-SF). Factors affecting the development of malnutrition were among the elderly T2DM patients identified using a multivariable logistic regression model.
Results:
A total of 382 elderly T2DM patients were enrolled, including 226 men (59.16%) and 156 women (40.84%) and with a mean age of (70.06±8.41) years. The prevalence of malnutrition was 22.25% among participants. Multivariable logistic regression analysis identified age (70 to 79 years, OR=1.261, 95%CI: 1.007-1.158; 80 years and older, OR=3.285, 95%CI: 1.618-6.662), three and more types of comorbidities (OR=2.790, 95%CI: 1.254-5.191), use of multiple medicines (OR=3.501, 95%CI: 1.841-6.658), sleep disorders (OR=1.613, 95%CI: 1.120-2.322), regular exercises (OR=0.280, 95%CI: 0.115-0.614), serum ALB of 35 g/L and greater (OR=0.272, 95%CI: 0.102-0.560) and HbA1c of 7% and higher (OR=2.914, 95%CI: 1.445-5.881) as factors affecting the development of malnutrition among elderly patients with T2DM.
Conclusions
The risk of malnutrition is associated with age, types of comorbidities, use of multiple medicines, sleep disorders, regular exercise, HbA1c and ALB among the elderly patients with T2DM. Periodical nutrition screening and intensified health education are recommended for T2DM patients.

In recent years,antiviral therapy for chronic hepatitis B infected patients has achieved great development along with the invention of nucleos (t) ide,nucleos (t) ide analogues,interferon α and pegylated interferon α.The development of antiviral medicine also proposes new demands for the clinical diagnosis and therefore promotes the development of laboratory diagnostic techniques in the detection of chronic hepatitis B.In this review,we focused on the clinical application in the quantitive detection of HBV DNA,and its significance on clinical evaluation,treatment options,follow-up and prognosis in antiviral therapy.
4.Determination of [6]-Gingerol in Rat Plasma by HPLC-UV

Objective To develop a HPLC-UV method for the determination of [6]-gingerol in the rat plasma.Methods [6]-Gingerol was extracted from the rat plasma by using liquid-liquid extraction,then was separated on a Hypersil C18 column(250 mm?4.6 mm,5 ?m).The mobile phase was acetonitrile and water(45:55,v/v) with a flow-rate of 1.0 mL/min.The eluate from the HPLC column was monitored by the spectrophotometric detector at 280 nm.The injection volume was 20 ?L.[6]-Gingerol was identified based on its retention time compared with the reference standard.Results The linear calibration curve was obtained in the concentration range of 0.25~5.0 ?g/mL.The low quantitative limit was 0.1 ?g /mL.The RSD of inter-day was
5.Application of ultrasound-guided interscalenus brachial plexus Mock for emergency surgery

Objective To investigate the feasibility of ultrasound-guided interscalenus brachial plexus block used for the emergency surgery.Method From August to December 2007 80 patients from the Integrated Traditional Chinese and Western Medicine Hospital of Wenzhou,ASA Ⅰ to Ⅲ,age 18 to 68 years old,weight 45 to 75 kg,without deformity on the shoulder or neck,without limb paresthesia,without contraindications of interscalenus brachial plexus block,were scheduled for emergency surgery of upper limb.They were randomly divided into two groups:interscalenus block guided by ultrasound group(group U,re =40)and nerve stimulator group(group N,re = 40).Patients of both groups received 20 mL mixture of 0.75%ropivacaine and 2%lidocaine.The onset time,and the analgesic efficacy of axillary nerve,lateral antebrachial cutaneous nerve,radial nerve,median nerve,median cutaneous nerve of arm,median antebrachial cutaneous nerve and ulnar nerve,and duration of anesthesia were observed.Statistical analysis was performed by t test,rank sum test and X2 test.Results The onset time of analgesia after nerve block was shorter in group U than that in group N(P<0.01).The rate of perfect analgesia after nerve block in group U was higher than that in group N(100%vs.87.5%,X2 = 5.267,P = 0.027).The duration of anesthesia lasted in group U was(378 ± 151)minutes and that in group N was(365 ± 163)minutes(t= 0.363,P = 0.718).ConclusionsCompared with the method of nerve block guided by nerve stimulator,the ultrasound-guided interscalenus block offers faster onset and better analgesic efficacy.It is an ideal technique of brachial plexus block for the emergency surgery.

40 normal persons were subjected to the air caloric test, 10 of whom also to the Hallpike water caloric test. The results showed no substantial differences between the air and water caloric stimulation. The normal values were obtained. There were no differences between right and left labyrinthes. The warm stimulation was more intense than the cold. Speed of slow component is the most important parameter of electronystagmograph.
7.Effect of valproic acid on astrocyte proliferation around the central canal in rats following spinal cord injury

Objective To investigate the effect and mechanism of valproic acid in preventing astrocyte proliferation around the central canal of rats following spinal cord injury.Methods Forty-five Wister rats were divided into normal control group (n =5),injury group (n =20) and treatment group (n =20) according to random number table.Animal models of acute spinal cord injury were produced at T10 using Allen' s method by dropping a 10 g weight from a 15 mm height.Rats in treatment group received intraperitoneal injection of valproic acid (300 mg · kg-1 · d-1 in two divided doses) at 30 minutes postinjury.Instead,rats in injury group were injected with an equal volume of saline in the same way.Hindlimb function was evaluated using BBB scoring system at 1,3,7,and 14 days postinjury.Astrocytes proliferation around central canal and expression of glial fibrous acid protein (GFAP) were examined.Results In normal control group,few astrocytes around spinal central canal and a low expression of GFAP were detected.In injury group,astrocytes began to increase at 24 hours postinjury; fluorescence intensity for GFAP was 24.6 ± 3.6 at 24 hours,reached a peak of 69.2 ± 6.4 at 3 days,maintained a high level of 56.7 ± 5.6 at 7 days,and reduced to 35.4 ± 4.3 at 14 days,a level that remained higher than that in normal control group (11.2 ± 1.6).Whereas in treatment group at 3 and 7 days,astrocyte proliferation around spinal central canal was lower than that in injury group; GFAP expressions (47.8 ± 5.3 and 42.2 ± 6.7) were lower than those in injury group (F =177.6,P < 0.05).At 3,7,and 14 days,BBB scores in treatment group (7.80 ± 0.83,12.00 ± 1.58,and 16.60 ± 1.12 respectively) were significantly higher than those in injury group (4.60 ± 0.54,6.65 ± 0.67,and 9.40 ± 1.14 respectively) (F =1 113.6,P < 0.05).Conclusion After spinal cord injury,valproic acid reduces astrocyte proliferation around central canal via inhibiting GFAP expression to promote functional recovery.
8.PEDIGREE ANALYSIS OF OTOSCLEROSTIC FAMILIES

This paper presented the hereditary pattern of otosclerosis in families. Among 1500 cases there were 13 patients, belonging to 6 families, and 20 ears were surgically treated. There was often a marked similarity in clinical features of affected siblings within the same family.The clinical characteristics are as following: (1) The age at inset of hearing loss seems similar. There is a difference of 1 or 2 years in two families znc 3 to 10 years in the other four. (2) The side of first hearing loss is the same in affected individuals of 6 families. (3) The propositi are females in 5 families (83.3%). (4) Auditory curve of hearing loss is similar in 4 familiers, and different in the other two. (5) The footplate lesions are similar in macroscopic features, thin footplate in two fimalies and thick footplate in the other four.
9.Study on the working noise in BYPC and the effects caused by working noise on the workers' vestibular and auditory function

Objective: In order to observe the kinds and intensity of the working noise of Yansan Petrochemical Co. and the effects caused by the working noise on the workers' vestibular and auditory function. Method :The intensity and frequency of the working noise were recorded by exactolnoisemeter in the workshop. One hundred and seventeen workers were tested in routine pure-tone tested method. The SPVN and ABR were tested within fifty-one workers of all. Result:The working noise of Yansan Petrochemical Co. belongs to the broad band and steady noise. The intensity of the working noise were during 85.7~104.0 dB (A) and the main frequency were during 1~8 kHz. About 59 percent workers who exposed to the working noise had hearing loss. The most hearing-loss were in the high frequency. The hearing-loss of speech frequency were slight. Workers who have more six years standing have obviously increased hearing-loss than the workers who have less five years standing. There were significant differences the ABR thresholds and wave-interval between the tested and controlled groups. The SPVN and CP were abnormal in more than 17.4 percent workers with hearing loss. Conclusion: The working noise of Yansan Petrochemical Co. belongs to the broad band and steady noise. Working noise can lead to workers' hearing loss of certain degrees who exposed in the noise for a long time. Obvious correlation was not defined between the hearing-loss and the abnormal vestibular response group.
10.Roles of ERKs and intracellular free calcium in cardiomyocyte hypertrophic response induced by endothelin-1

AIM: To study the roles and mechanisms of ERKs and intracellular free calcium in cardiomyocyte hypertrophic response induced by endothelin-1(ET-1). METHODS: (1) Neonatal rat cardiomyocyte hypertrophic response was assayed by measuring cell surface area and protein content; (2) ERKs activity was determined by Whatman Paper Filter method; (3) Intracellular free calcium concentration ([Ca2+]i) was measured using Fura-2/AM as a fluorescent indicator. RESULTS: (1) ET-1 could increase total protein production, surface area, ERKs activity and [Ca2+]i in cultured cardiomyocyte in dose-dependent manner at concentrations ranging from 10-9 to 10-7 mol/L. And this effect could be abolished by BQ123, an antagonist of ETA receptor, partly inhibited by PTX, but not by BQ788, an antagonist of ETB receptor.(2)The activation of ERKs and the increase of [Ca2+]i induced by ET-1 were obviously inhibited by PD98059, a selective ERKs kinase inhibitor, and nifedipine, a calcium channel blocker, respectively. Both antagonists partially inhibited ET-1-stimulated cardiomyocyte hypertrophic response. (3) Staurosporine, a selective PKC inhibitor, could inhibit ET-1-stimulated cardiomyocyte hypertrophic response and increase of [Ca2+]i, but not affect the activation of ERKs. CONCLUSION: Cardiomyocyte hypertrophic response induced by ET-1 is mediated by ETA receptor coupled to PTX-sensitive G-protein, which involves at least two signalling pathways: PKC-mediated increase of [Ca2+]i , and PKC-independent activation of ERKs.
