Objective To study the effect of matrine on the cell cycle and DNA content of fibroblasts derived from hypertrophic scarring tissues. Methods The fibroblasts were derived from 3~5 generations of the cultured hypertrophic scarring tissues. After the cells were treated with different concentration of matrine, the population doubling time was calculated and the cell cycle and DNA content were measured with flow cytometry. The fibroblasts treated with normal saline were as control. Results Compared with the control group, the distributions in cell cycle showed that the percentage of G 2 M and S phase cells decreased, and the percentage of G 0 G 1 phase cells increased significantly ( P
2.Treatment of open fracture of tibial shaft with external fixation at first stage combined with intr-amedullary nail internal fixation at second stage

Objective To evaluate the effect of external fixation at first stage combined with intr-amedullary nail internal fixation at second stage on treating open fracture of tibial shaft. Methods A total of 80 patients with open fracture of tibial shaft treated in our hospital from April 2016 to April 2017 were selected as the subjects. According to the treatment methods,they were divided as control group and obser-vation group with 40 cases in each group. The control group was given intramedullary nail internal fixation and the observation group was given external fixation at first stage combined with intramedullary nail inter-nal fixation at second stage. The general operation condition of the two groups was observed and the frac-ture reduction,healing and Rasmussen score were compared between the two groups. The difference of the circumference and pain degree between the two groups after 1,3 and 7 days were compared. Results The operative excellence rate of the observation group was significantly higher than that of the control group, and there was no difference in circumference and pain between the two groups after 1 day. The circumfer-ence and pain in the observation group was lower than that of the control group after 7 days postoperative-ly,and there was no difference in the Rasmussen score between the two groups after 1 month. The Rasmus-sen scores in the observation group were higher than those in the control group after 3 and 6 months post-operatively. The healing time and the time of callus formation in the observation group were shorter than those in the control group. Conclusion External fixation at first stage combined with intramedullary nail-ing internal fixation at second stage has better therapeutic effect on treating open fracture of tibial shaft.

OBJECTIVE:To investigate the analgesic and hypnotic effects of oral isoflurane(Iso)and sevoflurane(Sev). METHODS:180 mice were randomly divided into 18 groups(n=10). It included in hot-plate test(n=60),writhing test(n= 60),i.e. NS group,Iso1 group(1 mL?kg-1),Iso2 group(2 mL?kg-1),Iso3 group(3 mL?kg-1),Sev1 group(5 mL?kg-1),Sev2 group(10 mL?kg-1). Another six groups were included in sleeping test,i.e. NS group,Iso1 group(6 mL?kg-1),Iso2 group(8 mL?kg-1),Iso3 group(10 mL?kg-1),Sev1 group(20 mL?kg-1),Sev2 group(40 mL?kg-1). Neurogenic bowel dysfunction (NBD) manifested as constipation and fecal incontinence often occurs after spinal cord injury (SCI).NBD affects patients' quality of life and is an urgent clinical problem to be solved.The mechanism of NBD is related to central and autonomic nervous system dysfunction,intestinal nervous system dysfunction,changes in intestinal microorganism composition and abnormal content of neurotransmitters.The evaluation method of NBD is mainly based on scoring and imaging,which lacks unified criteria,and the treatment method for NBD is the combination of traditional Chinese and Western medicine.The author summarizes the mechanism,evaluation method,treatment and nursing of NBD in order to provide new insight into these aspects to improve clinical efficacy.

OBJECTIVETo investigate the effects of Matrine on apoptosis of fibroblasts and the expression of apoptotic modulation related protein in the hypertrophic scar.
METHODSHypertrophic scar was produced on the ear of 24 New Zealand white rabbits, which were employed as the model, and were randomly and equally divided into control (CC) and Matrine (M) groups (12 in each group). Matrine (50 g/L) was injected into the ear scar in M group and with normal saline in C group once every four days. At 2, 4, 6, 8 and 12 weeks after the injection, the apoptotic fibroblast count in the scar was determined by TUNEL method, and the expressions of apoptosis related modulation proteins p53, bcl-2, bax were detected by immunohistochemistry method.
RESULTSThe apoptotic fibroblast count was much larger in M group than that in C group at all test time points (P < 0.05). Furthermore, the bax expression was increased and that of p53 and bcl-2 was decreased significantly in M group. In adding, the scar became flat in M group.
CONCLUSIONMatrine might obviously enhance the fibroblast apoptosis in rabbit ear hypertrophic scar, and up-regulate the expression of apoptosis related modulation protein bax and down-regulate the expression of p53 and Bcl-2.

6.Effect of minimally invasive plate osteosynthesis combined with locking compression plate fixation on postoperativefracture healing and serum levels of nerve growth factor, vascular cell adhesion molecule and alkaline phosphatase in patients with limb comminuted fracture

Objective To investigate the effect of minimally invasive plate osteosynthesis combined with locking compression plate fixation on postoperative fracture healing and serum levels of and serum nerve growth factor (NGF),vascular cell adhesion molecule (s VCAM),alkaline phosphatase (ALP) in patients with limb comminuted fracture.Methods From January 2014 to May 2016,seventy-four patients with limb comminuted fracture in the Eighth People's Hospital of Shanghai were enrolled in this study and randomly divided into the control group (37 cases) and the study group (37 cases).The traditional incision and reduction fixation were used in the control group.The study group was treated with locking compression plate internal fixation combined with minimally invasive plate bone grafting technique.The patients were followed up for 1 ~ 4 months after operation.The clinical efficacy,perioperative indicators (operation time,intraoperative blood loss,fracture healing time),serum (nerve growth factor,vascular cell adhesion molecule,alkaline phosphatase) levels change at the time of admission and after first D and complications rate were statistically compared.Results The total effective rate of the study group was 97.30% (36/37),higher than that in the control group (78.38% (29/37)),and the difference was statistically significant (x2 =4.554,P<0.05).The operation time of the study group was (84.76 ± 11.08) min,longer than that of the control group ((71.89 ± 10.54) min),the intraoperative blood loss of the study group ((108.52 ± 21.33) ml) was less than that of the control group ((245.30±24.61) ml),and the fracture healing time of the study group ((9.04 ± 1.81) weeks) was shorter than that of the control group ((12.79 ±2.02) weeks),and the difference was statistically significant (t =5.119,t =25.547,t =8.410,P< 0.05).The levels of serum NGF,s VCAM and ALP in the two groups after operation were higher than those before operation,and the levels of serum NGF ((1.33±0.19) ng / L) and ALP ((312.04±31.07) U/L) in the study group were higher than those in the control group ((0.85 ± 0.22) ng/L,(181.28±30.45) U/L),and the level of serum s VCAM ((502.18±40.07) μg/L) was lower than that in the control group ((579.28 ± 41.15) μg/L),and the difference was statistically significant (t =10.044,18.283,8.165,P<0.05).The incidence of complications in the study group was 8.11% (3/37),lower than that in the control group (27.03% (10/37)),the difference was statistically significant (x2 =4.573,P < 0.05).Conclusion Minimally invasive plate osteosynthesis combined with locking compression plate fixation in the treatment of limb comminuted fracture can shorten the healing time of the fracture,improve the treatment effect,improve the levels of serum NGF,s,VCAM and ALP,and it have lower complication rate with higher safety.
7.Early period information need and its influencing factors of craniocerebral injury in patients′relatives

Objective To study the information need in early period and its influencing factors of craniocerebral injury patients′relatives. Methods A total of 180 patients′relatives from three general hospitals in Wenzhou were selected by stratified random sampling. Their information need was investigated by an original scale. The results were analyzed by statistical methods. Results The score of the scale was (21. 28 ± 2. 27). Within the dimensions, the highest and the lowest scores were (3. 56 ± 0. 31) and (2. 61 ± 0. 48), which were injury state and hospital rules, respectively. The relatives of patients who had been in hospital for less than three days had more information need on injury state, therapeutic measure, examination method, family cooperation and hospital rules than those of patients who had been in hospital for more than three days. The differences were statistically significant (P < 0. 05). The relatives of patients with severe craniocerebral injury had more information need on injury state, therapeutic measure, examination method, doctor and nurse competency, treatment cost and family cooperation than those of patients with mild and moderate craniocerebral injury. The differences were statistically significant (P < 0. 05). The relatives with high education degree had more information need on therapeutic measure, doctor and nurse competency and family cooperation than those with low education degree. The differences were significant ( P < 0. 05 ). The relatives′ satisfaction with the information provided by doctors and nurses was positively related to their satisfaction with total work of doctors and nurses (r=0. 495, P<0. 01). Conclusions The relatives of patients with craniocerebral injury have much information need. Patients′length of hospital stay and injury degree, and relatives′education degree may be the influencing factors of information need. Providing the relatives with the information need can improve their satisfaction with total work of doctors and nurses.
