4.Evaluation for the Accuracy of the SRK/T Formula in PCL Implanted Patients(II): Optimizing, A-Constant.

To evaluate the efficacy of optimizing of A-constant, the error in prediction was analyzed in three groups of eyes after posterior chamber lens implantation ; 102 eyes with bag placed, 66 eyes with simple sulcus places, and 25 eyes with sulcus placed with transscleral fixation suture. We compared the predictive accuracy of the SRK/T formula using optimized A-constant (bag-placed, sulcus-placed) derived from the 300 eyes with Holladay and SRK/T using original surgeon factor and A-constant. The SRK/T formula with optimized A-constant has a better mean relative, absolut, and standard error and has a better range of IOL prediction error, which was more accurate than other formulas without optimizing. In this study it is our recommendation that each surgeon use a different optimized A-constants for sulcus or bag placement to enhance the predictability of the postoperative refraction.

7.Surgical Repair of the Kidney after Blunt Renal Injury Using a Vicryl Mesh an Experimental Study.

Eighteen experimentally induced blunt and sharp renal injury in rabbits were treated either with an alloplastic renal capsule made of Vicryl mesh or with through and through chromic catgut sutures or with no suture as controls. A simple apparatus was used to cause uniform renal injury and the device was modification of an AO Kuntscher nail extractor. The Vicryl mesh capsule was made by ourselves with #2 and #4 Vicryl. The postoperative healing course were followed up until 6 weeks and isotope renogram showed good renal function in 15 cases. However, when compared to the controls, the kidney which had been repaired with Vicryl mesh contained considerably less scar and showed neither perirenal fibrosis or atrophy of the parenchyma in the vicinity of the capsule. Our preliminary results seem to confirm that simple and rapid surgical treatment of intermediate degree renal injuries is possible using the alloplastic Vicryl mesh capsule. The method may be also suitable for reconstruction of the parenchyma following nephrotomies, such as for removal of staghorn calculi.

8.Isolated avuision fracture of the tibial attachment of the posterior cruciate ligament

Five patients with isolated avulsion fracture of the tibial attachment of the posterior cruciate ligament were treated in our hospital from September, 1975 to May, 1978. Among the five patients. one was treated conservatively and the others by surgical repair using catgut suture, cancellous bone screw fixation and staple fixation. Four patients were followed from 4 to 10 months poatoperatively. Satisfactory results were obtained in four patients (one was loat in follow up).

9.Survival of An Avulsed Auricle Attached by A Tenuous Subcutaneous Pedicle.

Two avulsed ears attached by very narrow subcutaneous pedicles were repaired with only cutaneous sutures and showed complete survival. The results showed that the entire auricle could be fully vascularized by just a single small vascular branch. This clinical result is compatible with our previous findings in anatomical study concerning arterial supply of the auricle.
