1.The diagnostic observation on thinprep cytologic test and Pap smear in cervical disease

Objective To observe the diagnostic value of thinprep cytologic test(TCT) and Pap smear in cervical disease.Methods The cervix cytological examination 7600 women were divided into two groups.The TCT was applied to the observation group(3800 cases).The Pap smear was applied to the control group (3800 cases).Results The positive rate of cervical intraepithelial neoplasia was higher in the observation group(17.26%,656/3800)than that in the control group(12.24%,465/3800)(P<0.01).The coincident rate with histopathology was higher in the observation group(96.34%,632/656)than that in the control group(87.53%,407/465)(P<0.05).But the positive rate of trichomonad was not obviously significant between the two groups(P>0.05).Conclusion The TCT is better than Pap smear,which is worthy of being recommended in diagnosis of cervical disease.
2.Effect of ginseng saponin on spinal neurons in relation to level of nitrogen monoxide

BACKGROUND: Researches suggest that ginseng saponin (GS) has protective effect on central nerve, but the effect on spinal nerves is reported rarely.OBJECTIVE: To investigate the relationship between effect of GS on spinal nerve and level of nitrogen monoxide (NO) and its mechanism.DESIGN: Randomized controlled animal study.SETTING: Military Hyperbaric Oxygen Center of Navy General Hospital of Chinese PLA.MATERIALS: The experiment was completed at Clinical Anatomy Institute (National Key Laboratory) of the First Military Medical University of Chinese PLA in 2000. Forty SD foetus rats with 15-day conception were selected.METHODS: Study Ⅰ: Embryo-spinal nerve cells of SD rats were separated, extracted and modeled with DMEM/F12 culture medium. On the fourth day of inoculated culture, axon of spinal nerve (simulation of peripheral nerve injury) was damaged with scarification method in injury group, but that in non-injury group was not treated. 150 μL cell culture medium and 100 mg/L Griess solution were mixed at 0, 0.5 1, 1.5, 2, 2.5, and 3 hours after injury respectively. Absorbency (A) was assayed with Σ960 (λ=570 nm) enzyme-linked immunoadsordent assay (ELISA) symbolic device 10 min-utes after reaction at room temperature. Study Ⅱ: Embryo-spinal nerve cells of SD rats were separated and extracted. Those in the experimental group were treated with GS + DMEM/F12 culture medium, but with DMEM/F12 culture medium in the control group. A value was assayed with the same method.MAIN OUTCOME MEASURES:① Relationship between injury of spinal neurons and level of NO;② Relationship between protective effect of GS and level of NO.RESULTS:① Relationship between injury of spinal neurons and level of NO: In the injury group, NO secretion was increased after injury of spinal neurons, reached peak 2 hours later, and decreased 3 hours later. There was significant difference as compared 0.5 hour with 0 hour (P < 0.01),and also there was significant difference as compared 2 hourswith 0 hour (P < 0.01).② Relationship between protective effect of GS and level of NO: In the control group, A value was increased with time passing, reached peak 2 hours later, and decreased 3 hours later; but A value in the experimental group was not changed generally. There was significant difference between the two groups at 2-hour point (P < 0.01).CONCLUSION: NO liberation is increased after peripheral nerve injury.GS can inhibit NO liberation so as to protect peripheral nerve.
3.Concurrent chemoradiotherapy with paclitaxel and nedaplatin followed by consolidation chemotherapy in locally advanced squamous cell carcinoma of the uterine cervix

Background and purpose:Cisplatin-based concurrent chemoradiotherapy(CCRT) has become the standard treatment for locally advanced cervical cancer(LACC) . But no marked survival benefi t was found for stage Ⅲ-ⅣA patients,and the high toxicity of cisplatin warrants evaluation of other chemotherapeutic agents to increase effi cacy and decrease toxicity. This study was done to evaluate the effi cacy and toxicity of paclitaxel(TXL) and nedaplatin(NDP) administered concurrently with radiotherapy followed by consolidation chemotherapy in the treatment of locally advanced squamous cell cervical carcinoma(LASCC) . Methods:29 Patients with LASCC(FIGO stage ⅡB-ⅢB) were enrolled into this study from February 5,2007 through December 10,2007. The median age was 48 years old(35-64) . Radiotherapy included external beam radiotherapy to pelvic(stageⅡB 45Gy,stage Ⅲ 50 Gy) and 192Ir brachytherapy(stageⅡB 50 Gy,stage Ⅲ 35Gy) . TXL 35 mg/m2 and NDP 20 mg/m2 were given every week for 6 weeks concurrently with radiotherapy. After one month of CCRT,TXL 135 mg/m2 and NDP 60mg/m2 were administered every 3 weeks for 4 cycles. Results:All patients completed CCRT. For the consolidation chemotherapy,25 patients received 4 cycles,2 patients 3 cycles and 2 patients 2 cycles. The clinical complete response rate was 89.7%(95%CI,78.6%-100%) ,partial response rate 10.3%(95%CI,0-21.4%) After a median follow-up of 14 months(range,8.7-18.8 months) ,the disease-free and overall survival rates were 86.2%(95% CI,73.7%-98.7%) and 96.7%(95% CI 90.2%-100%) ,respectively. One patient died due to disease progression.During CCRT,Grade 3 leukopenia occurred in 3.6%(6/169) of the cycles,Grade 3 diarrhea 0.6%(1/169) ;During consolidation chemotherapy,Grade 3 leukopenia and neutropenia occurred in 8.7%(9/104) of the cycles. There were 2 patients who developed Grade 3 late rectum toxicity. Conclusion:CCRT with paclitaxel and nedaplatin followed by consolidation chemotherapy is well tolerated and effective in patients with LASCC. A prospective randomized trial to compare this treatment strategy with standard CCRT seems to be worthwhile.
4.Expression of potassium channel Kv1.6 in rat hippocampus after status epilepticus

Objective To investigate the expression and distribution of Kv1.6 in rat hippocampus after status epilepticus (SE) induced by lithium and pilocarpine to explore the relationship between Kv1.6 and epileptogenesis.Methods Forty-eight healthy male Sprague-Dawley rats were randomly divided into experiment group and control group.The rats in each group were randomly assigned into 6-hour,1-day,2-day and 3-day subgroups (n =6).Electroencephalography (EEG) of rats in both groups were recorded to show the electrical activity of the rat brains.Nissel staining was performed to investigate the pathological changes,and immunohistochemistry and Western blotting were applied to analyze the expression and distribution of Kv1.6.Results (1) The EEG of rats in control group showed α wave with uniform amplitude,while the EEG in rats subjected to pilocarpine injection showed slow waves and spike waves with irregular amplitude and rhythm.Sustained spike waves were observed during SE.(2) Nissl staining showed that 6 hours after SE,the neural morphology of hippocampi in experiment group was as the same to that in control group.There was no change in neuronal morphology and numbers.However,1 day after SE,the structure of hippocampi was discrete and the number of neurons was decreased.Two and 3 days after SE,the number of neurons in hippocampi decreased further.Swelling and deformation of neurons were seen.Nissl bodies emerged,decreased or even disappeared.(3) There was no statistically significant difference in the expression of Kv1.6 in CA3 and CA1 areas between experiment group and control group 6 hours after SE (all P>0.05).One day,2 days and 3 days post SE,the expression of Kv1.6 in CA3 area (0.117 ±0.040,0.135 ±0.029,0.112 ±0.050) decreased obviously compared to control group (0.192±0.054,0.201 ± 0.062,0.184 ±0.042;t---3.861,3.745,5.579,all P <0.05);and the expression of Kv1.6 in CA1 area (0.114 ± 0.02,0.082 ± 0.031,0.106 ± 0.043) decreased significantly compared to control group (0.165±0.036,0.187 ±0.025,0.172 ±0.052;t=3.221,6.360,3.645,all P<0.05).The expression of Kv1.6 did not show statistically significant difference in DG area between experiment group and control group at all time points examined (P > 0.05).The Western blotting showed the same trend.Conclusion The decreased expression of Kv1.6 may play a role in the epileptogenesis.
5.Effect of Serums Containing Xuefuzhuyu Capsules and Xuesetong Capsules on the Proliferation of Vascular Smooth Muscle Cells Induced by Lysophosphatidie Acid

Objective To investigate the effect of the serums containing Xuefuzhuyu capsules and Xuesetong capsules on the proliferation of vascular smooth muscle cells (VSMC) induced by 1ysophosphatidie acid (LPA). Methods Rat VSMCs were obtainded by tissue-piece inoculation and divided into seven groups:blank serum group, Xuefuzhuyu (Xuesetong) capsules-containing serum group, LPA+ Xuefuzhuyu (Xuesetong, captopril) containing serum group and LPA+blank serum group. Effects of different groups on the proliferation of VSMCs was evaluated by MTT colorimetry. Analysis of cell cycle and DNA content were performed by flow cytometry. Results The optical density (OD) and the S phase fraction (SPF) of LPA+blank serum group and Xuefuzhuyu capsules-containing serum group were more increased than the blank serum group (P 0.05, P 0.05). Conclusion Xuefuzhuyu capsules and LPA promoted the proliferation of VSMCs in vitro. Xuefuzhuyu and Xuesetong capsules can inhibit the proliferation induced by LPA, and effect of Xuesetong capsules was greater. Part of the mechanism may be associated with the inhibition of DNA synthesis and preventing the transition from G0/G1 to S phase of VSMCs.
6.Standardization of the Griffith Mental Development Scales for Children Aged 0~7 Years in the Cities of Shanxi Province

Objective: To revise the Griffith Mental Development Scales and establish the norm being suited to the Chinese local conditions. Methods: The standardized sample consisted of 2022 children aged 0 to 7 years old was collected in a stratified and random way according to city rank, age, sex, paternal occupations. Each subject was tested individually and filled in a form. Results: The total sample had better representation. The mean of GQ was about 100, and the distribution of GQ is normal. Item analysis indicated that the item difficulty was about 0.5 and discrimination was from 0.62 to 0.78. A few orders of items were revised. The scorer reliability was 0.953. The test-retest reliability was 0.948. The correlation coefficients were 0.481 to 0.854 among the sub-scales or between sub-scales and General DQ. The result of exploratory factor analysis had three factors. The ratio of variance was 79.8%.Conclusion:Griffith Mental Development Scales have ideal reliability and validity.
7.Analysis of clinical value of low dose of Oxaliplatin combined with transcatheter arterial chemoembolization in the treatment of primary hepatic carcinoma in elderly patients

Objective To investigate the adverse reactions and survival rate of low dose of Oxaliplatin combined with transcatheter arterial chemoembolization in treatment of elderly patients with primary hepatic carcinoma.Methods A total of 84 patients with primary liver cancer were enrolled and randomly divided into low dose group and high dose group.Patients received 40 mg/m2 and 80 mg/m2 of Oxaliplatin respectively,and adverse reactions and survival rate were compared between the two groups.Results Incidence of nausea and vomiting was obviously lower and the number of patients with pain scale above 6 was significantly smaller in low dose group than in high dose group,with statistically significant differences (11.9% vs.21.4% and 28.6% vs.78.6%,x2 =9.910,20.056,both P < 0.05).White blood cell count was significantly higher and alanine aminotransferase (ALT) was significantly lower in low dose group than in high dose group after treatment (t =13.447,6.774,both P<0.05).1 year and 3 year survival rate was 69.05% and 30.95% in low dose group and 61.9% and 21.43% in high dose group,respectively,with no statistically significant difference (x2 =0.474 and 0.985,P=0.491 and 0.321).Conclusions Low dose of Oxaliplatin combined with transcatheter arterial chemoembolization in the treatment of elderly patients with primary hepatocellular carcinoma is worth to be promoted in clinical application because it does not affect the patient survival rate and the toxic effects of the chemotherapy are very little.
8.Influence of trauma on MAC of sevoflurane and plasma ?-endorphin concentration

Objective In order to explore the effects of trauma on MAC and its mechanism, the changes in MAC of sevoflurane and plasma ? endorphin were investigated in state of trauma. Methods Twenty New Zealand rabbits were randomly divided into control group and trauma group. MAP,CVP, P ET CO 2 and ECG were monitored continuously. The MAC of sevoflurane was measured in the two groups. In trauma group, the left femur of rabbits was fractured combined with parenchyma damage.Plasma ? endorphin concentration was assaied at various times in two groups. Results MAC of sevoflurane was 2 2%?0 2% in control group and 1 7%?0 2% in trauma group. Trauma induced increase of plasma ? endorphin concentration significantly 5min and 30min after trauma,? endorphin concentration increased by 44% and 52% in trauma group, respectively. Conclusions MAC of sevoflurane in rabbits is 2 2%?0 2%.Trauma can reduce MAC of inhalational anesthetic significantly. The endogenous ? endorphin releasing enormously induced with trauma may be one of the mechanisms to decrease MAC of inhalational anesthetics.
