As a treatment of noninvasive,safe,effective,and easy to operate,external counterpulsation has been widely used in the treatment of ischemic heart disease.The treatment mechanism of external counterpulsation may improve collateral circulation and hemodynamics.Therefore,it can provide a new measure for the treatment of cerebrovascular disease.However,the efficacy and safety of external counterpulsation in the treatment of ischemic stroke still need to be confirmed by a large sample clinical trial.

Objective To observe the effect of Chinese medicine of Gukang(GK) on bone mineral density(BMD) in osteoporosis rats.Methods Seventy-two female SD rats were randomized into 6 groups: pseudo-operation group,model group,nilestriol group(in the dose of 1mg/kg),and high-,middle-and low-dose GK groups(9.6,4.8 and 2.4 g?kg-1?d-1 respectively).Rat models of osteoporosis were induced by removal of bilateral ovaries.Three months after operation,the rats were given the corresponding medicine according the experimental design.After treatment for 3 months,in-vivo BMD as well as the in-vitro BMD in the isolated left femur and tibia was detected with dual energy X-ray bone densitometer.Results The in-vivo general BMD and lumbar BMD of the model group were decreased(P

Objective To discuss the clinical value of subglottic secretion drainage on pulmonary infection in severe craniocerebral injuried patients. Methods Eighty severe craniocerebral injuried patients in ICU with the requirement of tracheotomy were randomized into two groups. Forty patients received aspiration of subglottic secretions as the treatment group and the other 40 patients received usual care as control. Samples were taken from pharynx mop, subglottic secretion and lower respiratory tract secretion for bacterial culture at 48 hours, 7th day and 14th day past tracheotomy. Results The incidence of pulmonary infection was 22.5% in treatment group and 45.0% in control. The same rate of predominant flora in subglottic secretion and lower respiratory tract secretion was 63.0% in patients with pulmonary infection. Conclusions Transposition of the pathologic bacteria in the subglottic secretion is one of the important causes of pulmonary infection, subglottic secretion drainage can lower the incidence of pulmonary infection in severe craniocerebral injuried patients.

Background and purpose:More than 70 percent of ovarian cancer patients were diagnosed in the advanced stage. Currently the 5-year disease free survival (DFS) of stageⅢC-Ⅳovarian cancer patients wsa about 10 percent after first line chemotherapy. This study aimed to improve the 5-year DFS by three-step chemotherapy according to the mechanisms of ovarian cancer biological characteristics, cytodynamics and pharmacology. Methods:In arm A, the patients received three-step chemotherapy after primary debulking surgery, step one with paclitaxel plus carboplatin (TC regimen), every 3 weeks for 6 to 8 cycles;step two with etoposide plus cyclophosphamide, every 4 weeks for 6 cycles;step three wit carboplatin plus cyclophosphamide every eight weeks for six cycles. In control arm B, we retrospectively analysed 51 cases withⅢC-Ⅳstage ovarian cancer, who had completely response after standard chemotherapy with six to eight cycles of TC after primary surgery during 2007. We compared the 5-year DFS between the two arms. Results:The 5-year DFS of 15 cases in arm A was 80%(12/15), which was signiifcantly higher than that of arm B (5.9%, 3/51, P<0.01). Conclusion: The three-step chemotherapy after optimal debulking surgery in stageⅢC-Ⅳ can improve the 5-year DFS. This regimen is high efficacy, mild side-effect witn low cost, which deserves further exploration.

Objective To investigate the treatment effects and toxicities of extended-field intensity modulated radiation therapy (EF-IMRT) and intra-cavitary brachytherapy combined with chemotherapy for stage Ⅰ b1-Ⅳa cervical cancer with positive para-aortic lymph nodes.Methods A total of 46 stage Ⅰ b1-Ⅳa cervical cancer patients with positive para-aortic lymph nodes treated at Fudan University Shanghai Cancer Center between 2009 and 2011 were reviewed.Neoadjuvant,concomitant and adjuvant chemotherapy with paclitaxel and carboplatin were administrated for one cycle before radiation therapy,two cycles during radiation therapy or three cycles after radiation therapy.All patients received EF-IMRT and intra-cavitary brachytherapy.The positive lymph nodes received an additional boost dose.Results All patients received EF-IMRT to 50.4 Gy (1.8 Gy per fraction).Twenty-six patients was treated with boost dose of 6.0-8.0 Gy in 2.0 Gy per fraction to positive para-aortic lymph nodes.Thirty-seven patients received a positive paraaortic lymph nodes boost or(and) parametrial boost.All patient also received a high-dose-rate intra-cavitary brachytherapy at the point A dose of 20.0-30.0 Gy in 5.0 Gy per fraction.Total chemotherapy cycles were 189,and the average patient received 4.1 courses.Two cases (4％,2/46) experienced grade Ⅲ gastrointestinal toxicities,no patients suffered grade Ⅳ gastrointestinal toxicities.Fifteen cases (33％,15/46) experienced grade Ⅲ hematological toxicities,and 3 (7％,3/46) experienced grade Ⅳ hematological toxicities.Late grade Ⅲ-Ⅳ toxicity was seen in 3 cases (7％,3/46).The 3 year progressionfree survival rate was 46.2％,and the 3 years overall survival rate was 61.2％.Conclusion EF-IMRT and intra-cavitary brachytherapy combined with chemotherapy is safe and effective for stage Ⅰ b1-Ⅳa cervical cancer with positive para-aortic lymph nodes.

Objective:To approach the status quo and characteristics of traditional Chinese medicine(TCM) in intefering the related cardiovascular risk factors in obese type 2 diabetes mellitus(T2DM).Methods:The recent 10 years documents about nitric oxide(NO),endothelin(ET),C reactive protein(CRP),and etc.,eight cytokines which are involved in T2DM and its cardiovascular complications were retrieved and statistically analyzed and all related TCM were pykno-analyzed.Results:① CRP was the focal point at home and abroad,adiponectin(APN) was the hot spot in recent years.② The documents of ET at home was greatly more than that abroad,it may hint some diversity between different races.③ The related TCM were mainly characterized by cleaning heat,enriching yin and qi,activating blood circulation and removing stasis.④ ET and NO were the hot spots in the TCM fi eld.Conclusion:TCM had certain effect on the cardiovascular risk factors above in obese T2DM.

AIM: To establish identification and determination methods for aristolochic acids. METHODS: The extracts of the sample were developed on silica gel G F254 plate, using supernatant layer of the mixture of toluene-ethyl acetate-water-formic acid (20 ∶10 ∶1 ∶1) as the mobile phase. Chromatogram system:gradient elution with mobile phase consisting of (A) 1% acetic acid in water and (B) methanol was used. The initial condition was at 40% of B and gradient up to 100% B in 15 minutes before returning to the initial conditions. Detection was at 310 nm. The column temperature was set at 40℃ and flow-rate was set at 1.0 mL/min. RESULTS: TLC:the Rf value of aristolochic acid Ⅰ was 0.50 and aristolochic acid Ⅱ was 0.53. The results of assay were shown as follow: The average recovery of aristolochic acid Ⅰ was 103.3% (RSD=0.98%). The average recovery of aristolochic acid Ⅱ was 95.97% (RSD=1.2%). CONCLUSION: It reveals that the methods we described are accurate, rapid and reproducible.

Objective To investigate clinical significance of small airway function and eosinophil (Eos) percentage,levels of eosinophil cationic protein (ECP) and IL-5 in induced sputum among patients with clinically controlled asthma. Methods Sixty-two patients with clinically controlled asthma were selected for the study. Lung function was performed and percentage of Eos, levels of ECP and IL-5 in induced sputum were measured by Wrights' stain, fluorescence immuno-CAP system and ELISA,respectively. Thirty patients of asthma at acute exacerbation period and 20 healthy subjects were selected as controls. Results In 62 patients with clinically controlled asthma, 43 (69. 4% ) showed abnormal small airway function and 19(30. 6% ) normal one. Percentage of Eos [(5. 6 ±2. 9)%], levels of ECP [( 129 ±100) μg/L] and IL-5 [(21± 12) μg/L] in induced sputum were significantly lower in patients with clinically controlled asthma than those of asthma at acute exacerbation period [( 16. 2 ± 9. 7 ) %, ( 362 ±182) μg/L and IL-5(51 ±26) μg/L, respectively] (all P ＜0. 01 ), but significantly higher than those in healthy controls ( all P ＜ 0. 01 ). Percentage of Eos, levels of ECP and IL-5 in induced sputum were significantly higher in patients with clinically controlled asthma with abnormal small airway function than those with normal ane [(6.9±3.1)% vs. (2.0±1.1)%, (148±90) μg/Lvs. (54±29) μg/L and (24 ±12) μg/L vs. ( 13 ± 5 ) μg/L, respectively] ( all P ＜ 0. 01 ). Conclusions Abnormal small airway function and airway inflammation persistently exist in patients with clinically controlled asthma and it may be helpful to guild treatment during clinical control to determine small airway function and inflammatory markers in their induced sputum.

Objective To study the influence of Chiamydia trachmatis (CT) and Ureaplasma ure-alyticum (UU) infection on pregnant outcome and neonates. Methods 562 pregnant women were ran-domly selected, their cervical secretion was collected and detected for CT and UU by culture method. Groups were divided according to the results. All of them were followed-up till delivery. Results The in-cidence of infection of group CT、group UU、group CT and UU were 22.1%、28.8% and 3.7%;Compared with negative group,the group with positive result of CT、UU、 CT and UU had significant higher incidence of preterm labor, premature rupture of membranes, low weight infant and neonates pneumonia. Conclusions The infection of CT、UU can result in adverse pregnant outcome, preterm labor, premature rupture of mem-branes, low weight infant and neonate pneumonia.

Objective To investigate the effects of 5-Aza-CdR( methylation transferase inhibitor) on the expression levels of leptin gene in chondrocytes and methylation states of leptin promoter region between osteoarthritis (OA) group and control. Methods The chondrocytes in osteoarthritis group were treated with 5-Aza-CdR with different doses and time-points, and the expression level of leptin was detected by real-time polymerase chain reaction for picking up the optimum dose and time-point. Next, the chondrocytes in 5 osteoarthritis patients and 5 control patients (amputation due to severe trauma) were treated with 5-Aza-CdR. Lastly, leptin mRNA expression levels in the four groups osteoarthritis and control chondrocytes treated with/without 5-Aza-CdR were measured by real-time PCR and the methylation state of promoter region ( - 280- + 79) was detected by epityper quantitative DNA methylation analysis. Results ( 1 ) After treating the chondrocytes in OA groups with 10 μmol/L 5-Aza-CdR for 72 h, the mRNA expression levels of leptin were increased significantly. ( 2 ) The mRNA expression levels of leptin were significantly different among the four groups ( P ＜ 0. 05 ), and the chondrocytes in osteoarthritis groups treated with 5-Aza-CdR showed a marked induction of leptin mRNA expression. (3) Analysis of quantitative methylation data using an unsupervised hierarchical clustering algorithm, showed that methylation patterns of leptin promoter was different between control and osteoarthritis chondrocyte treated with/without 5-Aza-CdR. Conclusion Demethylation of leptin promoter might up-regulate leptin gene expression level and it might contribute to osteoarthritis.
