Hepatocellular carcinoma is an one of the most common malignancies worldwidely. Recent advances in both the diagnosis and treatment of hepatocellular carcinoma have improved the prognosis, but it has been known difficult in early diagnosis and treatment of extrahepatic metastasis because of low diagnosis rate. We have experienced one rare case of recurred hepatocellular carcinoma with gastric invasion in the postoperative tissue which was confirmed by endoscopy and operation in a gastrointestinal hemorrhage pateint.

2.Comparison of the effects and complications of combined therapies using endoscopic variceal ligation with octreotide or terlipressin in the control of acute esophageal variceal bleeding.

BACKGROUND: In patients with liver cirrhosis, acute esophageal variceal bleeding is a life-threatening event with high mortality and subsequent recurrent bleeding despite of endoscopic therapy. So the effect of vasoactive agents used in combination with endoscopic therapy has been studied. The aim of this study was to compare the effects and complications of octreotide or terlipressin with endoscopic variceal ligation in the management of acute esophageal variceal bleeding. METHODS: From January, 1999 to February, 2001, 73 cirrhotic patients admitted to the Soonchunhyang University Chonan Hospital because of acute esophageal variceal bleeding were included in this study. The patients were randomized to receive either octreotide (37 patients) or terlipressin (36 patients) in combination with endoscopic variceal ligation. RESULTS: Initial control of bleeding was achieved in 35 (94.6%) patients receiving octreotide and in 33 (91.7%) patients receiving terlipressin. In the octreotide group, three patients with two cases of early rebleeding and one case of late rebleeding had rebleeding events, but in the terlipressin group, two patients with only early rebleeding bled again after initial control of bleeding. Two cases of myocardial ischemia were observed in terlipressin group, but there were no serious complications in octreotide group. CONCLUSION: This study suggests that both octreotide and terlipressin in combination with endoscopic therapy, are efficious in the initial control and the prevention of subsequent rebleeding. Octreotide showed a relatively lower risk of severe adverse reactions than terlipressin.

Hyperglycemic hyperosmolar state (HHS) is an acute complication mostly occurring in elderly type 2 diabetes mellitus (DM). Thyrotoxicosis causes dramatic increase of glycogen degradation and/or gluconeogenesis and enhances breakdown of triglyc-erides. Thus, in general, it augments glucose intolerance in diabetic patients. A 23-yr-old female patient with Graves' disease and type 2 DM, complying with methimazole and insulin injection, had symptoms of nausea, polyuria and generalized weakness. Her serum glucose and osmolarity were 32.7 mM/L, and 321 mosm/kg, respectively. Thyroid function tests revealed that she had more aggravated hyperthyroid status; 0.01 mU/L TSH and 2.78 pM/L free T3 (reference range, 0.17-4.05, 0.31-0.62, respectively) than when she was discharged two weeks before (0.12 mU/L TSH and 1.41 pM/L free T3). Being diagnosed as HHS and refractory Graves' hyperthyroidism, she was treated successfully with intravenous fluids, insulin and high doses of methimazole (90 mg daily). Here, we described the case of a woman with Graves' disease and type 2 DM developing to HHS.

4.A Rare Case of Ogilvie Syndrome Associated with Herpes Zoster.

Acute colonic pseudo-obstruction (Ogilvie syndrome) associated with herpes zoster is extremely rare, and few cases have been reported. An 81-year-old woman diagnosed with herpes zoster was referred for accompanying colonic ileus. The diameter of the cecum was 7 cm and a computed tomographic scan showed no definite obstructive cause. Because the patient showed minimal improvement with conservative treatment, endoscopic colonic decompression was performed successfully. Previous studies revealed that the treatment of Ogilvie syndrome associated with herpes zoster does not differ from that of other conditions, and the role of the varicella-zoster virus in this syndrome is unclear. Here, we present the first case of Ogilvie syndrome associated with herpes zoster in Korea, which was improved by endoscopic colonic decompression.

5.Association of Serum Vitamin D Status with Prediabetes and Prehypertension in Korean Adults: An Analysis of the 2013–2015 Korea National Health and Nutrition Examination Survey Data

Background:
Recent studies have indicated that vitamin D deficiency is associated with diabetes and hypertension in adults. However, this association has not been sufficiently studied in cases of prediabetes and prehypertension. Thus, this study aimed to clarify the relationship between vitamin D levels and prediabetes and prehypertension in Korean adults.
Methods:
Data from 1,808 adults aged ≥19 years who participated in the 2013–2015 Korea National Health and Nutrition Examination Survey were included in this study. The odds ratio of prediabetes and prehypertension according to the 25-hydroxyvitamin D (25[OH]D) status was calculated using multivariable logistic regression analysis.
Results:
Mean serum 25(OH)D levels were significantly different among females with normal glucose levels, prediabetes, and diabetes mellitus. Further, the mean levels were not significantly different in both males and females of all ages with normal blood pressure, prehypertension, and hypertension. In addition, logistic regression analysis showed that serum 25(OH)D levels were not significantly associated with the odds ratio of prediabetes and prehypertension after adjusting for age, sex, smoking, alcohol consumption, physical activity, calcium supplement intake, income, body mass index, systolic blood pressure, and fasting glucose.
Conclusion
Serum vitamin D status was not independently associated with the risks of prediabetes and prehypertension in Korean adults.

BACKGROUND: Several factors contribute to differences in Streptococcus pneumoniae serotype distribution. We investigated the serotype distribution and antimicrobial resistance of S. pneumoniae isolated between 2014 and 2016 in Korea. METHODS: We collected a total of 1,855 S. pneumoniae isolates from 44 hospitals between May 2014 and May 2016, and analyzed the serotypes by sequential multiplex PCR. We investigated the distribution of each serotype by patient age, source of the clinical specimen, and antimicrobial resistance pattern. RESULTS: The most common serotypes were 11A (10.1%), followed by 19A (8.8%), 3 (8.5%), 34 (8.1%), 23A (7.3%), and 35B (6.2%). The major invasive serotypes were 3 (12.6%), 19A (7.8%), 34 (7.8%), 10A (6.8%), and 11A (6.8%). Serotypes 10A, 15B, 19A, and 12F were more common in patients ≤5 years old, while serotype 3 was more common in patients ≥65 years old compared with the other age groups. The coverage rates of pneumococcal conjugate vaccine (PCV)7, PCV10, PCV13, and pneumococcal polysaccharide vaccine 23 were 11.8%, 12.12%, 33.3%, and 53.6%, respectively. Of the 1,855 isolates, 857 (46.2%) were multi-drug resistant (MDR), with serotypes 11A and 19A predominant among the MDR strains. The resistance rates against penicillin, cefotaxime, and levofloxacin were 22.8%, 12.5%, and 9.4%, respectively. CONCLUSIONS: There were significant changes in the major S. pneumoniae serotypes in the community. Non-PCV13 serotypes increased in patients ≤5 years old following the introduction of national immunization programs with the 10- and 13-polyvalent vaccines.
