2.Observe the shape of plug mesh and patch by B ultrasound after plug-mesh hernia repair.

ObjectiveInvestigate the shape of plug mesh and patch after plug-mesh hernia repair and discuss the necessity of improving the operative procedures. MethodsUse B ultrasound to investigate the shape of plug-mesh and patch after mesh plug hernia repair and review the patients medical record,evaluate how operative procedures affect the shape of patch and the results. ResultsIn 4 of the 16 cases,the distant end of patches overlapped the pubic margin,in the remaining 12cases the distant end of patches didn't reach the pubic margin. When changing the cut-in direction of B ultrasound probe,a wide and dense shadow was discovered in 8 cases whose patches didn't reach the pubic margin. Between the pubic margin and dense shadow there was a space showing low dense shadow. These photocopies showed that the patches had folded up or crimped. In 11 of 12 cases,the patches didn't fix on pubic margin or cut shorter during operation. Conclusion It is suggested to fix the distant end of patch on the aponeurosis tissue of pubic in the procedure of mesh plug hernia repair,and combine the plug-mesh repair with Lichtenstein repair.
3.Monitoring Analysis on Medication Orders for Clinical Inpatients by PASS

OBJECTIVE:To evaluate the performances of prescription automatic screening system(PASS).METHODS:40101medication orders of616inpatients in14departments of our hospital were monitored retrospectively by using“Army Health No.1”Hospital Information System Pharmaceutical Workstation that equipped with PASS.RESULTS:There were835irrational drug use medication orders and the incidences of irrational drug use stood at2.08%.Of which,91.02%involved adverse drug interactions,4.55%involved irrational in vitro injection combinations and4.43%were irrational use of drugs for children.CONCLUSION:PASS system is conducive to rational drug use in clinics,yet it is still for from being perfect thus further improvement is needed.
4.Deep vein hemodynamics of the lower limbs in obesity

Objective To evaluate lower extremity deep venous hemodynamic changes and lower extremity deep venous disease in abdominal obesity.Methods To compare venous flow parameters of the lower limbs as assessed by Duplex ultrasound scanning in obese and nonobese individuals according to body mass index(BMI).Venous hemodynamics were studied in a prospective cohort study in nonobese( BMI < 25 kg/m2) and obese individuals( BMI > 30 kg/m2 ).Diameter,peak,mean,and minimum velocities were assessed.Results The study examined 45 limbs in 24 nonobese individuals and 44 limbs in 22 obese individuals.The diameter of the femoral vein was significantly greater in obese vs nonobese limbs ( 10.4 ± 1.3) mm vs (7.1 ± 1.0) mm,P < 0.01 ).Calculation of venous amplitude and shear stress showed significantly higher values in nonobese vs obese limbs [ ( 11 ± 5 ) vs ( 8 ± 3 ) cm/s ],( P < 0.01 ) and [ (0.50 ± 0.17 ) vs ( 0.30 ± 0.09 ) ] dyn/cm2,( P < 0.01 ).Spearman rank correlation revealed a significant inverse correlation between waist-to-hip ratios and waist circumference and velocities amplitude (peak ve locity-minim velocity),and shear stress.Conclusions Lower limb venous flow parameters differ significantly between obese and nonobese individuals.These findings suggest the mechanical role of abdominal adipose tissue potentially leading to elevated risk for both chronic venous insufficiency and venous thromboembolism.
5.Accumulation of intravenously injected carboxylated single-walled carbon nanotubes in rat axillary lymph nodes

BACKGROUND:Lymph-targeted tracing and therapy based on carbon nanotubes have been one of the hottest researches on targeting tumor diagnosis and treatment. To evaluate the accumulation of carbon nanotubes in axil ary lymph node can provide experimental evidences for developing nano-tracers and drug carriers which are more lymph-specific and more biocompatible. OBJECTIVE:To study the accumulation of the intravenously injected carboxylated single-wal ed carbon nanotubes in axil ary lymph nodes of Sprague-Dawley rats, and to evaluate their effect on blood cel s. METHODS:Sixty-four Sprague-Dawley rats were randomly divided into two groups. Rats in testing group were injected with carboxylated single-wal ed carbon nanotubes suspension (2 mg/kg), while those in control group were injected with 5%glucose solution (1 mL/kg), both through the tail vein, three times per week. Four periods of 7, 60, 90 and 120 days were set (the 120-day period referred to 90 days of administration fol owed by 30 days of drug withdrawal). At the end of each period, eight rats from each group were randomly picked out, to col ect blood samples via the abdominal aorta for blood routine test. Final y the axil ary lymph nodes were observed, and the lymph node samples of rats in the testing group were col ected and analyzed at 120 days by transmission electron microscope. RESULTS AND CONCLUSION:Compared with the control group, black staining of axil ary lymph nodes of rats in testing group was not obvious at the end of the 7-day period. However, with the increase of the dosing periods, the lymph nodes of the rats in the testing group became enlarged, firm and black stained, coupled with a significant rising in the percentage of blood neutrophils. After 30 days of drug withdrawal, the size of the rat axil ary lymph nodes was reduced and black staining partly faded, with the decreasing of blood neutrophil percentage. Under the transmission electron microscope, abundant carboxylated single-wal ed carbon nanotubes were uptaken by lymphocytes to form a large number of phagocytic vacuoles after drug withdrawal for 30 days. It indicates that the short-term tracing of rat axil ary lymph nodes by carboxylated single-wal ed carbon nanotubes injected through the tail vein is relatively weak, while the long-term intravenous injection can cause their accumulation in rat axil ary lymph nodes, coupled with the increase of neutrophils;after drug withdrawal, the carboxylated single-wal ed carbon nanotubes can be slowly cleared by the lymph nodes.
6.How to well prepare before the start of medical projects of The National Key Research and Development Program of China

Objective The management of medical projects of the National Key Research and Development Program of China is difficult.Thus this article aims to analyze the common problems and summarize the preparatory work before the project initiation.Methods Comprehensively adopted the methodologies of literature analysis,survey investigation to analyze the common problems before the project initiation,and then particularly summarize the preparatory works for biomedical research,especially for clinical research,from the perspective of investigators.Results Proposed several aspects that should take into consideration before the initiation of the projects..clarify the organizational management framework,play the role of kick-off meeting,organize tailored training on financial management,prepare research protocol and related documents,seek Institutional Review Board approval and conduct clinical research registration,normalize document managment,formulate project management plan,and prepare research facilities timely.Conclusions Investigators should develop a detailed project management plan before initiation of the project,preparation work should focus on personnel,financial resources,facilities,research progress,quality,data,etc.
7.Differential expression profile of microRNAs between human hepatocellular carcinoma cell line and human normal hepatocyte cell line

Objective To establish the expression profiles of microRNA (miRNA) in SMMC-7721 and CCC-HEL-1 cell lines in order to provide new clues to study the mechanism of miRNA function in hepatocellular carcinoma (HCC) and its treatment. Methods SMMC-7721 and CCC-HEL-1 cell lines were cultured in vitro. Total RNAs were extracted by TRIzol, followed by RNA quantification and quality control. The RNAs were used to detect the expression of miRNA in these two cell lines by miRNA array. The miRNA of interest was then verified by real-time PCR. Results In total,238 differentially expressed miRNAs were found, including 154 overexpressed and 84 underexpressed miRNAs. 64 miRNAs were upregulated more than 4 times and26 miRNAs were upregulated more than 10 times. 22 miRNAs were downregulated more than 4 times 10 miRNAs weredownregulated more than 5times, and 1 miRNA was downregulated more than 10 times. Real-time PCR validation suggested that has-miR-205 and has-let-7f in liver cancer increased by 2. 7 and 2. 3 times, respectively.Conclusion There are differences in the expression of miRNA between the SMMC-7721 and CCC-HEL-1 cell lines and the profiles of miRNA expression were established for both cell lines. It was found that has-miR-205 and has-let-7f were upregulated in liver cancer.
8.Detection of common chromosome abnormalities in myelodysplastic syndrome with a panel fluorescence in situ hybridization.

OBJECTIVETo evaluate the value of a panel fluorescence in situ hybridization (FISH) in the detection of common chromosome abnormalities in myelodysplastic syndrome (MDS).
METHODSTwenty cases of MDS patients, whose karyotypes were unknown by the FISH examiner beforehand, were analyzed with a panel FISH using YAC248F5 (5q31), YAC938G5 (7q32), CEP8 and YAC 912C3 (20q12) probes to detect the frequently occurring chromosome abnormalities (-5/5q, -/7q-, +8, 20q-) in MDS. Then the results were compared to those of conventional cytogenetics (CC).
RESULTSAmong 20 cases, 13 cases were found to carry common chromosome abnormalities by panel FISH (trisomy 8, five cases; -5/5q-, one case; 20q-, five cases; 5q- accompanying 20q-, one case; complex abnormalities, one case). However, on CC examination, only five cases were found to have common chromosomal abnormalities (20q-, four cases; 5q- accompanying 20q-, one case). In addition, trisomy 21, marker chromosome and complex abnormalities comprising -5, -7 and marker chromosomes were seen in one case each, the rest were normal.
CONCLUSIONPanel FISH is a useful tool of molecular cytogenetics in the detection of common chromosome abnormalities in MDS.

9.Chromosome study on chronic lymphocytic leukemia using CpG-oligodeoxynucleotide as immunostimulant agent.

OBJECTIVETo investigate whether CpG-oligodeoxynucleotide (CpG-ODN) can improve the detection rate of the karyotypic abnormalities in chronic lymphocytic leukemia (CLL).
METHODSThe bone marrow (BM) or peripheral blood (PB) cells from 57 cases of CLL were collected and cultured with CpG-ODN DSP30+interleukin-2 (IL-2), phytohemagglutinin (PHA), pokeweed (PWM) or IL-2, respectively. Five days later cells were harvested for chromosome preparation. Karyotypic analysis was done using R banding technique. Panel fluorescence in situ hybridization (FISH) was carried out on 19 cases of CLL with normal karyotypes using the following probes: Cen12, D13S25, Rb1, ATM, p53, MYB and IgH. Genomic DNA from 21 cases of them was extracted from BM or PB leukocytes. The immunoglobulin variable heavy chain (IgVH) was amplified by polymerase chain reaction (PCR) and sequenced. CD38 and ZAP70 expressions in the leukemic cells were determined by flow cytometry (FCM).
RESULTSThe detection rate of karyotypic abnormalities in the CpG-ODN+IL-2 group (43.85%) was obviously higher than that in the PHA (15.09%), PWM (17.31%) and IL-2 (3.13%) groups (P<0.01). Fifty-two types of karyotypic abnormalities were found. Among them, trisomy12 (+12) or +12 with other abnormalities were the most common, while translocations were the most frequent structural abnormalities including 3 unbalanced and 11 balanced translocations, among them 7 had rearrangements involving 14q32. Thirteen cases showed one or more abnormalities on FISH including trisomy 12 and p53 deletion each in one case, IgH rearrangement and partial deletion each in one case, 13q14.3 deletion in 11 cases of which 5 cases also had Rb1 deletion, 1 case had Rb1 partial deletion. No case with ATM or MYB deletions was found. PCR detected IgVH mutations in 10/21 cases. FCM showed 10/45 cases were CD38 positive, but 35 /45 were CD38 negative, 11/27 cases expressed ZAP70, but 16/27 did not. Among the 26 cases examined for CD38 and ZAP70 expressions simultaneously, 5 cases were CD38+ZAP70+, 13 were CD38-ZAP70-, 6 were CD38-ZAP70+, and 2 were CD38+ZAP70-, respectively. Statistic analysis showed a correlation between complex karyotype and IgVH without mutation, but no association between karyotype and CD38 or ZAP70 expression was observed.
CONCLUSIONCpG-ODN immunostimulation can obviously raise the detection rate of abnormal karyotypes, especially translocations in CLL. FISH is an important complement to conventional karyotypic analysis. The combination of both methods can provide more comprehensive genetic information for CLL.

10.An analysis of the development of national clinical research center: An embeddedness perspective

Objective:To analyze and summarize the process of integration between national clinical research center and the hospital to which it affiliated.Methods:Embeddedness was introduced to establish a framework for analysis. Information was collected from 81 respondents from 16 institutions to understand the expectations for national clinical research center.Results:The establishment of national clinical research center is expected to help researchers with research space, talent cultivation, funding, dada sharing, research outcome transformation and further cooperation. Research platform and communication cost are expected to be optimized.Conclusions:To further develop the national clinical research center, we need to identify the key indicators for performance appraisal, physician scientist play the role as administrative leader, coordination between university and hospitals, provide comprehensive compensation plan for the talents, conducting clinical research project, construction big data analysis platforms, promote research outcome transfer, reduce the communication cost between doctors and basic science researchers.
