1.Clinical Utility of Bronchial Washing PCR for IS6110 and Amplicor for the Rapid Diagnosis of Active Pulmonary Tuberculosis in Smear Negative Patients.

BACKGROUND: There is a well recognized interlaboratory variation in the results using the polymerase chain reaction(PCR) to detect the IS6110 sequence. The clinical utility of a commercially developed PCR test(Amplicor) in bronchial washings for detecting pulmonary tuberculosis in smear negative patients was evaluated. The sensitivity and specificity of Amplicor was compared with that of an in-house PCR test used for detecting the IS6110 sequence of Mycobacterium tuberculosis(M.tbc) in the bronchial washing fluid. METHODS: 66 patients whose sputum smear for M.tbc were negative or who could not produce any sputum were recruited from January 1999 to July 1999. They all had a bronchoscopy performed to determine if there were signs of hemoptysis, patients who could not cough up sputum, lung lesion that exclude pulmonary tuberculosis. Pulmonary tuberculosis was diagnosed on the basis of a positive culture or a response to anti-tuberculosis therapy. RESULTS: 19 patients with tuberculosis were identified and samples from 16 patients were later confirmed by culture. Bronchial washing for Amplicor PCR revealed a sensitivity, specificity, positive and negative predictive values of 94.7%, 97.9%, 94.7%, 97.9%, respectively. Using IS6110 based PCR, the sensitivity, specificity, positive and negative predictive values were of 73.7%, 87.2%, 70%, 89.1% respectively. CONCLUSION: Bronchial washing for Amplicor PCR proved to be more useful than IS6110 based PCR in rapidly diagnosing smear negative pulmonary pulmoary tuberculosis in patients where tuberculosis was likely to be differential and rapid diagnosis was essential for optimal treatment.

2.Designing a Mobile Intervention Platform to Help Alleviate Insomnia Symptoms in College Students

Objectives:
:An increasing number of people are affected by sleep problems every year. The current study presents findings from experiments that utilize a mobile application-based cognitive-behavioral treatment for insomnia (CBT-I).
Methods:
:Our application aims to alleviate insomnia symptoms by providing real-time and direct interventions in people’s daily lives. We recruited 50 participants to test the effectiveness of the app via a six-week-long prospective experiment.
Results:
:We find that insomnia symptoms are reduced significantly for both the treatment group, who used the app, and the control group, who watched educational videos [F(2,39)=60.82, p<0.001]. Sleep efficiency improved more quickly in the treatment group than in the control group. Participants who followed higher compliance to the intervention reduced their wake after sleep onset (WASO) time more substantially.
Conclusion
:Our results suggest that app-based CBT-I interventions may have additional benefits for enhancing objective sleep quality, including sleep efficiency or WASO, compared to sleep hygiene education. The preliminary findings of the current research shed light on future mobile intervention apps’ design choices for insomnia.
3.Effects of Child Maltreatment on Physical Activity and Sleep in Healthy Adults:A Wearable Device Use Experiment *

Objectives:
:Maltreatment is one of representative risk factor for psychiatric illnesses. While several studies indicate the relationship between physical activity and maltreatment, there was lack of study using objective measurements. This study was conducted to find the biomarkers of physical activity related to maltreatment experiences in healthy adults using wearable device.
Methods:
:A total of 60 healthy adults were recruited; 15 subjects reported maltreatment experiences. Physical activity and sleep data were collected using Fitbit for 2 weeks. Child maltreatment experiences were assessed by Adverse Childhood Experiences (ACE). Symptoms were measured using the Insomnia Severity Index (ISI), Morningness-Eveningness Questionnaire (MEQ), Patient Health Questionnaire-9 (PHQ-9), and Generalized Anxiety Disorder-7 (GAD-7).
Results:
:The child maltreatment group showed lower movement distance (p=0.025) and number of walks (p=0.010) compared to the control group. After controlling for anxiety and depressive symptoms in women, maltreatment group represented significant reduction in light activities (p=0.021) as well as movement distance (p=0.024) and number of walks (p=0.011). There was no significant difference in sleep variables between the two groups.
Conclusions
:Even in healthy participants, maltreatment experiences can negatively affect daily light physical activity.

Obesity-Hypoventilation syndrome (OHS) is characterized by morbid obesity, hypoxia, and hypercapnea during wakefulness without parechymal lung disease or severe obstructive sleep apnea. A woman was admitted because of mental deterioration and diagnosed as OHS on the basis of obesity and hypoventilation, while awake, after ruling out other causes. By bilevel positive airway pressure (BiPAP) therapy, hypercapnea and hypoxia were resolved. We report that BiPAP can be an effective treatment for severe hypercapnea and hypoxia in OHS, which obviate the need for invasive endotracheal intubation.

6.Analysis of Dynamics of Gait in Parkinson's Disease with 3-Demensional Gait Analysis System.

BACKGROUND: Gait disturbance is one of the most disabling symptoms of Parkinson's disease (PD). To better understand of the dynamics of gait in PD, we quantitatively evaluated the spatiotemporal and kinematic parameters of gait in PD patients and normal controls with 3-demensional gait analysis system. METHODS: The diagnosis of PD was based on UK Brain Bank criteria. Subjects with PD were recruited from Parkinson disease clinic of Korea University Medical Center. The severity was indicated by Hoehn and Yahr stage and Unified Parkinson disease rating scale (UPDRS). The gait analysis and assessment was performed in the off condition. RESULTS: Thirty PD patients and thirty normal controls were included. The statistical significant results of gait analysis between PD and control groups were as followings; reduced walking velocity, reduced stride length, increased double supporting time, reduced pelvic oblique movement with preserved pelvic tilting, reduced maximal flexion and extension of hip joint, reduced flexion angle of knee joint especially in terminal stance phase and swing phase, and reduced plantar flexion of ankle. But there were no significant difference between PD and control groups in cadence. And these parameters except cadence had significant correlation with clinical disability in PD patients. The cadence did not correlated significantly with clinical disability. CONCLUSIONS: The PD patients showed statistically gait cycle change with preserved cadence, reduced range of motion in hip, knee, and ankle joint with preserved ankle dorsiflexion in the profiles. These features were aggravated in advanced stage.

7.Comparison of the Uniaxial Tensile Strength, Elasticity and Thermal Stability between Glutaraldehyde and Glutaraldehyde with Solvent Fixation in Xenograft Cardiovascular Tissue.

BACKGROUND: With the advances of cardiac surgery, the demand for an artificial prosthesis has increased, and this has led to the development and utilization of diverse alternative materials. We conducted this research to improve an artificial prosthesis by examining the changes of the physical qualities, the pressure related tensile strength, the change in elasticity and the thermostability of a xenograft valve (porcine) and pericardium (bovine, porcine) based on the type of fixation liquid we used. MATERIAL AND METHOD: The xenograft valves and pericardium were assigned into three groups: the untreated group, the fixed with glutaraldehyde (GA) group and the glutaraldehyde with GA+solvent such as ethanol etc. group. The surgeons carried out each group's physical activities. Each group's uniaxial tension and elasticity was measured and compared. Thermostability testing was conducted and compared between the bovine and porcine pericardium fixed with GA group and the GA+solvent group. RESULT: On the physical activity test in the surgeon's hand, no significant difference between the groups was sensed on palpation. For suture and tension, the GA+solvent group was slightly firmer than the low GA concentration group. In general, the circumferential uniaxial tension and elasticity of the porcine aortic and pulmonary valves were better in the fixed groups than that in the untreated group. There was no significant difference between the GA and GA+solvent groups (p>0.05). Bovine and porcine pericardium also showed no significant difference between the GA group and the GA+solvent group (p>0.05). When comparing between the groups for each experiment, the elasticity tended to be stronger in most of the higher GA concentration group (porcine pulmonary valve, porcine pericardium). On the thermostability testing of the bovine and porcine pericardium, the GA group and the GA+solvent group both had a sudden shrinking point at 80degrees C that showed no difference (bovine pericardium: p=0.057, porcine pericardium: p=0.227). CONCLUSION: When fixing xenograft prosthetic devices with GA, adding a solvent did not cause a loss in pressure-tension, tension-elasticity and thermostability. In addition, more functional solvents or cleansers should be developed for developing better xenografts.

8.A Case of Splenic Lymphoma with Villous Lymphocytes.

Splenic lymphoma with villlous lymphocytes (SLVL) is a B cell chronic lymphoproliferative disorder which affects predominantly elderly males. Most patients present with splenomegaly without lymphadenopathy and a lymphocytosis. This report describes a case of SLVL in 66 year old male who complained abdominal discomfort and abdominal mass. The leukocyte count was 47.8x109/L with 89% atypical lymphocytes showing variable amout of moderately basophilic cytoplasm and unevenly distributed or polar cytoplasmic villi. The bone marrow was effaced with these neoplastic lymphocytes. The immunophenotype of the neoplastic B cells was CD19 (+), CD20 (+), CD22 (+), CD11c (+), CD2 (-), CD3 (-), CD5 (-), CD10 (-), CD25 (-) and CD15 (-). The cytogenetic findings exhibited 46,XY,del (3) (q24),del (10) (p12). The spleen was weighted 520g and had nodular infiltrates of neoplastic B cells in the region of the white pulp with minimal red pulp involvement.

Laundry detergent pod (LDP) exposure has been reported to be fatal in children younger than 2 years, leading to respiratory or central nervous system depression. While gastrointestinal irritation is the most common symptom, there are reported cases of severe acidosis with respiratory depression or pneumonia, resulting in mortality. To our best knowledge, there is no report on a case of LDP exposure presenting with acute respiratory distress syndrome requiring extracorporeal membrane oxygenation support. Here, we present a case of a child with severe acute respiratory distress syndrome following LDP exposure, who was successfully treated with veno-pulmonary extracorporeal membrane oxygenation and steroids.

Bronchopulmonary dysplasia (BPD) and pulmonary hypertension (PH) are potentially fatal complications in prematurely born infants. Extracorporeal membrane oxygenation (ECMO) may be a life-saving option for managing infants with BPD and PH. We present 2 patients who were successfully weaned off mechanical ventilators (MVs) through the application of ECMO. The patients were transferred to our institution after receiving MV care for 8 and 10 months, respectively, for BPD and PH. We were able to remove the patients from MVs after a period of ECMO-mediated lung rest. Although more research is required to determine specific criteria for ECMO use in patients with BPD and PH, our clinical experiences may contribute to the early application of ECMO in MV-dependent patients.
