1.A Case of Solitary Keratoacanthoma Originated from Verruca Vulgaris.

Keratoacanthoma is a benign neoplasm that develops as a protruding mass especially on the central part of face, We experienced a case of keratoacanthoma that developed on the nasal vestibule after the eradication of verruca vulgaris on the same site by surgical intervention 3 months before. We treated it with total excision and skin graft.

2.A Case of 4P+ Syndrome.

We experinced a case of 4p+ syndrome in male infant. He had multiple anomalies such as flat occiput, hypertelorism, low set malformed ear, lower anterior hair line, depressed nose, broad nasal bridge, bilateral complete cleft lip and palate, short neck, unusual position of fingers, ventricular septal defect and umblical hernia. He menifested growth and developmental retardation. Karyotype with banding revealed an extra short arm of chromosome 4. The mother's karyotype was normal. His father and father's sister had a translocation between the short arm of chromosome 4 and the short arm of chromosome 9; their karyotypes were 46, XY, t(4;9) and 46, XX, t(4;9), respectively. In this case, trisomy 4p was the result of parental balanced translocatiom. As this is the first case in Korea, it is worthwhile to report with reviewing literature.

3.A Case of Penile Horn.

Penile Horn occurs as a single or multiple lesions. It usually starts as warty growth that later becomes hyperkeratotic and assumes the appearance of a horn. A 22-year-old male patient was presented with a slow growing, asymptomatic, dark brownish colored, corn-shaped, protruding mass on prepuce of the penis for 5 months. Histopathologic finding showed verruca vulgaris. We think that penile horn in this case may be come from verruca vulgaris.

4.Spontaneous left external iliac vein rupture.

We report a 72-year-old female patient with spontaneous rupture of the left external iliac vein. She visited our hospital for abdominal and back pain. She had the abnormal finding of hemoperitoneum. We performed an emergency operation with diagnosis of left ovarian cyst rupture though she suffered from spontaneous rupture of the left external iliac vein. This case provides insight to the experience of spontaneous rupture of the left external iliac vein.

6.Altered expression of potassium channel genes in familial hypokalemic periodic paralysis

We analyzed the mRNA expression patterns of major potassium channel genes to determine the
mechanism of hypokalemia in familial hypokalemic periodic paralysis. We used quantitative RT-PCR
to examine the mRNA levels of both inward (KCNJ2, KCNJ6, and KCNJ14) and delayed rectifi er
(KCNQ1 and KCNA2) potassium channel genes in skeletal muscle cells from both normal and patient
groups, prior to and after exposure to 4 mM and 50 mM potassium buffers. Quantitative RT-PCR
analysis revealed no changes in the mRNA levels of these genes in normal and patient cells on exposure
to 4 mM potassium buffer. However, after exposure to 50 mM potassium buffer, which was used to
induce depolarization, normal cells showed a signifi cant decrease in KCNJ2, KCNJ6, and KCNJ14
expression, but no change in KCNQ1 and KCNA2 expression. In contrast, patient cells showed no
change in KCNJ2 and KCNJ6 expression, but an increase in KCNJ14 expression. Furthermore, KCNQ1
and KCNA2 showed decreased expression. We found that the expression levels of both inward and
delayed rectifi er potassium channel genes in patient cells differ from those in normal cells. Altered
potassium channel gene expression in patient cells may suggest a possible mechanism for hypokalemia
in familial hypokalemic periodic paralysis.

8.Problems after Interlocking Intramedullary nailing for Long Bone Fracture

Interlocking intramedullary nailing has many advantages for long bone fracture. However, we can meet many problems during the operative procedure. Among the patients who were treated with interlocking intramedullary nailing from March, 1990 to December, 1991, forty-eight patients(49 cases) of femur and tibia fracture who were followed up more than twelve months(average 14months: 12months 18months) were included in this study. 1) Forty-nine cases consisted of 20 cases(41%) of femur fracture and 29 cases(59%) of tibia fracture. 2) Two cases of proximal protrusion were developed because of inadequate implant length. 3) Operative complications were encountered with 3 cases of new fracture line, 1 case of new fragmentation, 1 case of reduction loss, 2 cases of failure of proximal targeting, 3 cases of failure of distal targeting, 2 cases of distraction of fracture site more than 3 mm, 1 case of sciatic nerve injury due to excessive traction and 1 case of tibial inlet error. 4) Postoperative complications were encountered with 1 case of distal targeting screw failure, 4 cases of infection and 8 cases of delayed union. 5) According to the result, sufficient planning and confirmations(preoperatively, intraoperatively and postoperatively) can diminish the complications. And then the interlocking intramedullary nailing will be useful method in the case of long bone fracture when the operator has extensive experiences.

9.Clinical characteristics and nursing diagnoses of pediatric patients hospitalized with inflammatory bowel disease: a single-center retrospective study in South Korea

Purpose:
This study aimed to identify clinical characteristics of South Korean pediatric inflammatory bowel disease (IBD) in a children's hospital over the past 5 years, with a specific focus on comparing the features observed between Crohn's disease (CD) and ulcerative colitis (UC). Additionally, it aimed to examine the nursing diagnoses given to patients.
Methods:
This retrospective study analyzed the medical records of Korean pediatric patients under 18 years of age who were diagnosed with IBD and hospitalized at a children's hospital in Seoul, South Korea, from January 2017 to December 2021.
Results:
The number of pediatric patients diagnosed with IBD steadily increased. This finding was particularly prominent for CD patients, the majority of whom were male. Pediatric patients with CD had significantly higher rates of abdominal pain and perianal lesions, while pediatric patients with UC had a higher rate of bloody stool. Laboratory findings indicated that CD patients had higher levels of inflammatory markers and lower albumin levels than UC patients. The nursing diagnoses given during hospitalization mostly related to safety and protection, physical comfort, and gastrointestinal function.
Conclusion
This study provides insights into Korean pediatric IBD patients, enabling early detection and the development of nursing intervention strategies. From a comprehensive perspective, nursing care should not only address patients' physical needs but also their psychosocial needs.
10.A Case of Hemorrhagic Infarct of Testis.

We have experienced an unusual case of hemorrhagic infarct in the right testis at the age of 59 years. The spermatic cord showed normal appearance and no torsion was noted. Orchiectomy was performed as frozen section revealed hemorrhagic infarction of the testis. Herein, hemorrhagic infarction of testis, a rare clinical entity, was reported with review of literatures.
