1.The Significance of the Expression of p53, E-cadherin, nm23, CD44, and Tumor Angiogenesis in Colorectal Adenocarcinoma.

Many oncogenes and tumor supressor genes have been identified and studied in colorectal carcinoma. Among them, p53 is a tumor supressor gene and its mutation is frequently noted in human tumors. E-cadherin is a cell adhesion molecule and associated with tumor differentiation. CD44 is a cell surface glycoprotein that plays a role in cell migration and metastasis. nm23 is a gene known to lower metastatic potential of tumors and has been proposed to be a metastasis supressor gene. Tumor angiogenesis is required for the expansion of the primary tumor and metastasis and its degree is related to the potential of malignancy. We studied the expression of p53, E-cadherin, nm23, CD44 and tumor angiogenesis in 36 cases of colorectal adenocarcinomas. They were compared with previously known prognostic factors such as the stage, tumor size, depth of invasion, differentiation, presence of lymphatic or venous invasion, the lymph node and distant metastasis. The results were as follows. 1) The expression of p53 was not significantly associated with any prognostic factors. 2) The expression of E-cadherin was significantly associated with tumor differentiation. In the well differentiated adenocarcinomas, its expression was higher than in the poorly differentiated adenocarcinoma. 3) The expression of nm23 was also significantly associated with tumor differentiation. In carcinoma with lymph node metastasis, the expression of nm23 was reduced, but statistically it was not significant. 4) The expression of CD44 was higher in tumors with lymph node metastasis than in tumors without lymph node metastasis, but it was not statistically significant. 5) The degree of microvessel density was significantly associated with lymphatic invasion. According to the above results, the expression of E-cadherin and nm23 are related to the differentiation of the tumor and tumor angiogenesis is related to the lymphatic invasion of the colorectal adenocarcinoma.

2.Ilizarov External Fixation in High Tibial Osteotomy

Elderly patient with Genu Varus deformity is commonly treated by high tibial valgus osteotomy to unload the stress concentration medially. Most of these procedures were performed by closing wedge osteotomies and that revealed some problems. Most require a fairly large scar and later reoperation for implant removal. The Ilizarov apparatus and Ilizarov's principles of deformity correction seem to offer several advantages over other methods of high tibial osteotomy. With the Ilizarov method, distractional open wedge technique offers a biomechanically sound, predictably accurate, and reproducible method of correcting an extremity's mechanical axis deviation. From December 1991 to December 1993 at the department of orthopaedic surgery, Pusan National University Hospital, 5 patients(7 tibia) underwent the operation of correction of the genu varum using Ilizarov external fixator for osteoarthritis. The results were summarized as follows: l. All case were female, and the Mean age was 58 years old. 2. Preoperative mean varus angle were 20° and postoperative mean valgus angle were 6° at final follow-up. 3. The complication was not detected includes limb length descrepancy. 4. Mean Ilizarov external fixator fixation period was 5 months. 5. The results after average 1 year 1 month of follow-up were as follows: Excellent in 2 cases(29%), Good in 4 cases(57%), Fair in 1 case(14%), Poor in 0 case()%). 6. The early clinical and radiologic results were encouraging, although a longer follow-up period is necessary.

3.The Femoral Cortical Osteolysis in Association with Cementless Total Hip Arthroplasty

Nine hips in eight patients had development of femoral endosteal cortical erosion after a cementless total hip arthroplasty. Serial anteroposterior and lateral radiographs of all of the affected femora were reviewed to determine the interval between the operation and the first recognition of the osteolytic lesion or lesion, as well as to evaluate the radiographic appearance and progression of the lesions. We reviewed the one hundred twenty hips in one hundred eleven patients had a cementless total hip arthroplsty at the department of Orthopaedic Surgery, Pusan National University Hospital from January 1985 to December 1991. The mean average follow up period was forty five months (twenty four to one hundred nine months). Osteolysis was first noted radiographically at forty four to eighty five months(mean, sixty three months) postoperatively; it occurred most frequently around the distal portion of the prosthetic stem. Three of the femoral components were shown to be loose. The remaining six hips had been judged to be stable when the endosteal erosion had first been identified. The osteolysis was more severe in the femora in which the component was loose than in those in which it was stable. Of the six stable hips and two unstable hips except revised one hip, who were followed, four hips had an increase in the size of osteolytic lesion with time. All two unstable hips and two stable hips among six stable hips had an increase in the size of osteolytic lesion with time. Therefore, close follow up in necessary for patients in whom osteolysis develops in association with a stable cementless femoral component.

6.Cauda Equina Syndrome after Laminectomy of Lumbar Spine with Application of Autogenous Free Fat Graft: A Case Report.

Although epidural fibrosis after laminectomy is considered to be the cause of pain in a number of patients, the exact relationship of postoperative scar tissue and symptoms remains controversial. However it is generally accepted that epidural fibrosis after surgical decompression of neural tissue has to be avoided. To diminish the likelihooa that such a scar will form, fat grafts have been used to create an interpositional membrane. Compression of a nerve after the use of a fat graft is rare; postoperative cauda equina syndrome, resulting from compression by a grafted fat. We report the case of a patient who had this complication.

7.Intra-abdominal Angiosarcomatosis Induced by Irradiation.

Therapeutic irradiation can induce angiosarcoma. Radiation-induced angiosarcoma constitutes 20% of all angiosarcomas. Although its common site of origin is the skin and subcutaneous tissue, it rarely arises in small or large bowels with a presentation as multifocal abdominal angiosarcomatosis. We report a case of intra-abdominal angiosarcomatosis involving the jejunum, ileum, transverse colon, mesentery and right ovary in a 63-year-old female. It developed 10 years after therapeutic irradiation for squamous cell carcinoma of uterine cervix. She developed panperitonitis due to intestinal perforation. She died from sepsis 3 days after segmental resection of the small bowel and right oophorectomy. We reviewed the previously reported cases and describe the clinicopathologic features of this tumor.

9.Polymorphic Reticulosis.

Polymorphic reticulosis(PMR) is a unique clinicopathological entity of unknown etiology, which commonly present as an aggressive, necrotizing lesion of the upper respiratory tract. It is a separate nosologic entity from Wegener's granulomatosis and idiopathic mildine destructive disease. The origin of the cells composing polymorphic reticulosis has been controversial. We reviewed 15 cases of polymorphic reticulosis with respect to clinical and histologic bases, and immunohistochemical studies were done using UCHLI (CD45RO), as a T cell marker, CD-20 as a B cell marker and alpha- I -antichymotrypsin as a histiocytic marker. Almost all cases showed characteristic histologic and immunohistochemical features similar to those of peripheral T cell lymphoma. Thus, this study indidcated that polymorphic reticulosis is a type of malignant lymphoma of T cell lineage.
10.Clinical Analysis Between Surgically Proven Contained and Ruptured HIVD.

STUDY DESIGN: The authors retrospectively analysed the difference of clinical natures in contained vs. ruptured HIVD. OBJECTIVE: To compare contained HIVD with ruptured HIVD in respect of clinical symptoms, signs and the result after surgery. SUMMARY OF LITERATURE REVIEW: In contained disc herniation, the disc material remains beneath tile intact outer annulus. But, once disc material penetrates through the limit of posterior annulus, it is a ruptured herniation or disc extrusion/sequestration. There is few report concerning the clinical characteristics related to type of herniation. MATERIALS AND METHODS: Forty-six patients, treated by open discectomy from March 1990 to December 1994, were followed up for minimum two years. The clinical symptoms and signs including SLR, motor deficit, sensory deficit, change of DTR, and severity of radiating pain were periodically followed up on the predesigned protocol. Result: Twenty patients had contained disc and remaining 26 had ruptured disc. The mean age of contained disc was 31.4 years and that of ruptured disc was 43.3 years. In preoperative examination, 18/26 cases of patients with ruptured discs, developed pain in less than 3 months, compared with 7/20 cases of does with contained ones. Motor deficit was positive in 25/26 cases (96.2%) of ruptured discs and 12/20 cases (60%) of contained ones. 23/26 cases (80.8%) of patients with ruptured discs, had sensory deficit compared with 12/20 cases (60%) of those with contained ones. There was no significant difference in SLR and DTR change between ruptured and contained disc. CONCLUSIONS: Factors such as age, motor and sensory deficits and duration of symptoms had correlation with the type of herniation, but SLR and DTR change showed no statistical difference in this study. Clinical outcome showed no significant difference between two groups, but in ruptured group, the results were getting worse with the time goes. On the basis of this study, we concluded that the clinical symptoms and signs were different between the two groups, but surgical results showed no statistical difference.
