Osteogenesis imperfecta is a heterogenous group of inherited disorder, which has abnormalities in the synthesis of collagen. Among variable clinical manifestations, orthopaedic clinical features are laxity of ligament and deformities arising from frequent fractures or angulation due to skeletal fragility. We had managed 4 cases of osteogensis imperfecta, which were belonged to type IV-B in two cases, type I-A in one, and type I-B in remained one by Sillence classification and they had suffered from recent fractures or deformities due to previous fractures in the long bones of the lower extremities. These patients had been treated with various types of intramedullary nails, such as Bailey-Dubow extensile rod. Rush rod and Kuntcher nail, and the result of intramedullary nailing lead to improvement of walking ability.

5.Quantitative Comparison of Dopa - Positve Cells in the Skin of Normal Person and Pigmentary Disorders.

Melanocytes were identified by the dopa reaction, and counted in vertical sections of 183 specimens of normal skin and 218 specimens of patients with pigmentary disorders. The incidence of dopa-positive cells in the basal layer was determined. One cell in 9 in the basal layer was dopa-positive in normal specimens. There was no significant difference in the incidence of dopa-positive cells between male and female and between adult and children. Specimens from face, neck and prepuce revealed comparatively high incidence of dopa-positive cells. Among pigmentary disorders, vitiligo and halo area of halo nevus were characterized by an abscence or marked reduction in the number of dopa-positive cells, but lentigo and cafe-au-lait spot were characterized by an increase in their number In tuberous sclerosis, tinea versicolor and idiopathic guttate hypomelanosis, there wasn't any change of the melanocyte population between lesional skin and normal skin. The significance of these findings in vertical skin sections was discussed.

6.Utility of 99mTc-MAG3 Perfusion Indices in the Evaluation of Renal Transplant Function During Early Post-transplantation Period.

PURPOSE: We have examined the utility of 99mTc-MAG3 perfusion indices for assessing renal graft function in early post-transplantation period. MATERIALS AND METHODS: Our study included 80 renal transplant recipients (48 men and 32 women, mean age: 40.3 years). Diagnosis was based on biopsy, laboratory data and clinical course. Renal scintigraphy (RS) was obtained using 100 MBq of 99mTc-MAG3 from 11 days to 23 days of kidney transplantation. We measured 5 indices in whole-kidney (WK) and cortical (C) renograms; Hilson's perfusion index (PI), transplant perfusion index (TP) and transplant function index (TF) as perfusion parameter, and the time to peak activity (Tmax) and the ratio of renal counts at 20 min to that at 3 min (K20/3) as functional parameter. RESULTS: The diagnoses at the day of RS were normal graft (NG) in 44, acute rejection (AR) in 14, acute tubular necrosis (ATN) in 10, and Cyclosporine A nephrotoxicity (CsA) in 12. TP and TF were significantly decreased in AR, ATN and CsA, compared to those in NG. K20/3 of AR and ATN were significantly greater than that of NG. WK-Tmax of AR was significantly longer than that of NG. K20/3 of AR and C-K20/3 of ATN were significantly prolonged relative to those of CsA. There were no statistically significant perfusion indices among complication groups. CONCLUSION: TP and TF reflecting microperfusion and initial tubular extraction are reliable in assessing graft function. However, it is required to correlate perfusion indices with functional indices and clinical course in differentiating from one another among complication groups.

7.A Clinical Analysis on the Patients of Orient-Medical Therapies in Emergency Department.

We have observed that many patients who were under the treatment of orient medicine at the emergency department of our hospital but no data are available describing the medical habits of these patients. We therefore surveyed these patients seeking care in our ED to characterize their use of orient-medical therapies.4 analysis on the 427 patients who visited our ED and were under the treatment of orient medicine from Jan. 1996 to Jun. 1996 was carried out The results were as follows 1) There was no difference in the sex ratio (m:f=1:0.93). 2) With respect to the age groups of in patients, feat numbers of elderly people(50-year and 60-year old age group, over 60%) visited to our ED. 3) In the leading causes for visit to orient-medical therapy, cardiovascular disease ranked at the top(40%), followed by injury, gastrointestinal disease, and respiratory disease in decreasing order of frequency.4) The total numbers of CVA patients admitted to Oriental medicine were 105 patients. 5) The duration of admitted patients to Oriental medicine was prolonged for elderly patients.6) In the distribution of severity(AVPU) of adimitted patients, the number of patients with alert was 12 and verbal response was 91 painful response was 2 and unresponse was 0.

10.The effect of respiratory syncytial virus infection on neutrophil adherence to airway epithelial cells.

BACKGROUND: Respiratory syncytial virus (RSV) is the most common cause of lower respiratory infection in infants and young children, but the pathogenesis of RSV-induced inflammation is not well defined. MATERIAL AND METHOD: In order to examine the potential interactions between virus-infected airway epithelial cells and neutrophils, we studied the ability of neutrophils to adhere to yirus-infected airway epithelial cell monolayers by myeloperoxidase assay. Also we measured the ability of airway epithelial cells to secrete interleukin-8(IL-8) and inter-cellular adhesion molecule-1(ICAM-1) in virus-infected airway epithelial cell cultures by enzyme-linked immunosorbent assay(ELISA). The degree of IL-8 and ICAM-1 gene expression in the RSV-infected BEAS-2B cell cultures were analyzed by reverse transcription-polymerase chain reaction(RT-PCR). RESULTS: The RSV-infected BEAS-2B cell resulted in significantly enhanced level of neutrophil adherence compared to the uninfected control(p (0.001). IL-8 and ICAM-1 production significantly increased by RSV infection(p<0.05). There was a significant positive correlation between neutrophil adherence and IL-8 level(r=0.73, p=0.002), and ICAM-1 level (r=0.843, p=0.001) in RSV-infected cells. The degree of both IL-8 and ICAM-1 mRNA expression increased in the RSV-infected cells compared with the uninfected ones. CONCLUSION: These results suggest that RSV infection significantly enhances the production of IL-8 and ICAM-1 in airway epithelial cells which then results in increased neutrophil adherence.
