1.Telomerase Activity of Endometrium Related to the Effects of the Sex Steroid Hormone and Endometrial Cancer.

During the reproductive period, human endometrium undergoes a pattern of cyclic change. Human endometrium undergoes a complex pattern of proliferation, secretory activity, and menstruation over an approximately 28 days period. Proliferative activity is highest during late proliferative phase under influence of estrogen, and minimal activity in the late secretory and menstrual phase. To study a possible change of telomerase activity during menstrual cycle, telomerase activities in normal and hormone treated endometrium were tested using telomerase repeat amplification protocol(TRAP) assay. Telomerase activities were detected in 9 of 10 proliferative endometrium(90%), and maximal activity was shown in late proliferative phase. Only 3 of 10 secretory endometrium(30%) revealed weak activity. However telomerase activity was not detected in menstrual phase endometrium(N 2) and senile endometrium(N=3). Four of tamoxifen treated endometrium(N 4) and 1 of provera treated endometrium(N 3) Levels of telomerase activity of treated endometrium(N 4) and late proliferative endometrium(N 6) were as high as them of detected in endometrial cancer and hyperplasia. Above findings reveal that telomerase activity of endometrium is changed according to menstrual cycle, And the level of telomerase activity is related to proliferative activity of endometrium that is dependent on the status of female sex steroid hormone and tamoxifen treatment.

2.A Comparative Study of Mental Health between the Laid-Off and the Employed Worker in the Similar Industry.

OBJECTIVES: This study was conducted to examine the mental health status of the laid off by comparing the unemployed with the employed. METHODS: A survey questionnaire was used to investigate the mental health status of the laidoff in comparison with that of the employed. The study sample consisted of 247 laid-off people formerly employed in A auto factory and 167 employees working in B auto factory. RESULTS: The proportion exercising regularly was 20.2% for the laid-off and 34.8% for the employed and the proportion having regular meals was 10.9% and 50.6%, respectively. In addition, the frequency and amount of alcohol consumption and smoking of the laid-off were higher than those of the employed. Among the laid-off, 69.4% responded that they spent less time in talking to their spouse, and 65.4% also reported reduced sexual activities with their spouse. Because of unemployment, 41.8% of the laid-off experienced conflicts with their children, and 53.5% also perceived that they were stigmatized and treated badly by their neighbors, friends and relatives. Mean IFR in the laid-off was 22.9, which was much higher than the 15.3 recorded in the employed. The mean PWI-SF was 24.2 for the laid-off and 13.0 for the employed, which indicates the lower psychological health status of the laid-off than that of the employed. Age, annual salary, family relationship, regular exercise, regular diets, possibility to return to work and perception of the future were related to mental health status for the laid-off. Stepwise multiple regression analysis showed that the most important factor influencing mental health status for the laid-off was family relationships. CONCLUSIONS: According to the above results, the mental health of the laid-off is worse than that of the employed. Based on this study, a follow-up study focusing on the mental health status of the reemployed to the same factory, is needed to understand the direct mental health effect on the laid-off.

4.A Case of Gastritis Cystica Polyposa, Presenting as a Submucosal Tumor - like Lesion.

Gastritis cystica polyposa (GCP) is a lesion characterized by all the histological features described for hyperplastic polyps, And there is s marked proliferation of muscular elements and entrapment of numerous epithelial cysts. These lesions have been described at gastroenterostomy stomas, at peptic ulcer edges, and in association with carcinoma. It is rare that GCP presenting as a submucosal tumar-like lesion develops in the absence of above mentioned associated conditions. We had experienced a 69 year-old man with GCP. Barium X-ray and endoacopic findings showed submucosal tumor in antrum, and endoscopic ultrasonograpic findings showed markedly thickened mucosal layer with scattered hypoechoeic areas and the submucosal layer well preserved. GCP was confirmed by histological examination of resected stomach.

Alveolar soft-part sarcoma is a rare malignant neoplasm. It occurs most commonly in the deep tissue of the extremity, particularly in the anterior aspect of the thigh. It usually presents as a slow-growing, painless mass with little functional impairment. Because of the relative lack of symptoms, it may be easily overlooked. The course of the disease tends to be long but unrelenting, and local recurrences are frequent. There were no lifetime cures in the reported cases. This tumor is relatively unknown to orthopaedic surgeons, and a wider knowledge of its characteristics may lead to its more frequent recognization, Recently, two cases of alveolar soft-part sarcoma were encountered on the orthopaedic department of Dong-kang Hospital, Ulsan. In case 1, the tumor was found in the vastus lateralis muscle; and in case 2, vastus intermedius muscle. The authors will present these cases in detail to provide a glance at this very rare but more frequently recognizable tumor, the alveolar soft-part sarcorna.

6.Acanthamoeba Keratitis: Case Report.

We experienced chronic relapsing central corneal ulcer and chronic conjunctivitis with mucoid discharge. Both patients were transfered to our hospital because of no improvement with long term treatment of broad spectrum antibiotics topically and systemically Specimens from corneal scraping and conjunctival mucoid discharge were examined with calcofluor white staining method. We could find a narrow rim of light green colored cystic wall and orange red colored cytoplasm in dark field fluorescent microscopy in both cases, and those were diagnosed as Acnthamoebic cyst by pathologist. We couldn't find any report about ocular Aanthamoeba infections in Korea. To our knowledge, this is the first case report about ocular Acanthamoebic infections in Korea.

7.Effects of chlorpheniramine on emergence agitation after general anesthesia for ureteroscopic stone surgery: a retrospective cohort study

The presence of a urinary catheter, postoperative pain, and postoperative nausea and vomiting are risk factors for emergence agitation (EA). Antimuscarinic agents are primary agents used in the prevention and treatment of urinary catheter-related bladder discomfort. Chlorpheniramine has antimuscarinic, antinociceptive, and antiemetic effects. This retrospective study investigated the role of chlorpheniramine in EA prevention following ureteroscopic stone surgery. Methods: Of 110 adult patients who underwent ureteroscopic stone surgery under general anesthesia between January and December 2019, the medical records of 93 patients were analyzed retrospectively. The patients were divided into control (n = 52) and chlorpheniramine (n = 41) groups according to the receipt of intravenous chlorpheniramine before the induction of anesthesia. The incidence and severity of EA were compared between the groups as primary and secondary endpoints, respectively. The effects of chlorpheniramine on the requirement for inhalation anesthetic (desflurane) during surgery, changes in mean blood pressure and heart rate during emergence, and adverse events were also compared. Results: The incidence (21.2% in the control group, 24.4% in the chlorpheniramine group) and severity of EA did not differ between groups. The intraoperative requirement for desflurane, changes in mean blood pressure and heart rate during emergence, and adverse events were also similar between groups. Conclusions: Chlorpheniramine was not associated with a decrease in EA incidence or severity in patients who underwent ureteroscopic stone surgery
8.Optimum Conditions for the Preparation of Red Blood Cell Suspensions for ABO Antibody Titration.

BACKGROUND: There is significant inter-laboratory variation in the ABO antibody (Ab) titer levels of blood samples because a standardized method has not yet been developed. The aim of this study was to identify the best conditions for the preparation of the red blood cell (RBC) suspensions so as to aid the development of a standard ABO Ab titration method. METHODS: Serum samples from apparently healthy adults and RBCs from three different sources (residual EDTA blood from healthy adults, donor blood in citrate-phosphate-dextrose-adenine-1 [CPDA-1], and a commercially available RBC reagent) were used for Ab titrations. We measured the titers for each blood group under various conditions, including the time period of storage (days), the ratio of serum to RBC volume, and the RBC sources. The techniques for room temperature incubation and the indirect antiglobulin test were used for the tube and the gel card test. RESULTS: A storage period of 6 to 7 days significantly affected the Ab titers. Samples with 3% RBCs in a 1:1 serum to RBC volume ratio had significantly lower Ab titers than those with 2% RBCs in a 1:1 ratio or those with 3% RBCs in a 2:1 ratio. There were no significant differences in the Ab titers of RBCs from different sources. CONCLUSIONS: To reduce inter-laboratory variations in ABO Ab titrations, using RBC suspension within five days of storage and applying ratio of serum to RBC volume to 2:1 with 3% RBC in the tube test will be helpful when using home-made RBC suspension.

9.The Lsolation of Organism and Sensitivity Test in Conjunctivitis Under 1 Year old of Age.

Neonatal and Infantile conjunctivitis are common disease in ophthalmologic outpatient department but, a study of causative organisms and antibiotics sensitivity tests are insufficient till now We divided 117 babies(117eyes) having conjunctivitis into neonatal and infantile group who were visited Sung-Ae General Hospital from Jan, 1989 to May, 1990. We performed bacterial culture and antibiotic sensitivity test. The results were as follows; 1) Culture positive rates are 65% in neonates and 87% in infants. There is no sexual difference of ratio. 2) The incidence of isolated organisms is Staphylococcus aureus, Coagulase negative staphylococcus and Streptococcus. 3) Cephalothin is most sensitive antibiotics and Chloramphenicol shows relatively lower sensitivity.

10.Metastatic Skin Cancer That Showed Multiple Erythematous Patches on the Trunk.

Cancer that tends to metastasize to other internal organs also involves the skin with various clinical features. We report two cases of metastatic skin cancer that showed multiple erythematous patches on the trunk. Two patients presented with erythematous macules and a patch with telangiectasia on the trunk and chest wall. A microscopic examination of the skin biopsy specimens showed infiltration of large atypical cells with hyperchromatic nuclei and scant cytoplasm in the dermis. Using variable immunohistochemical staining, we identified the primary origin of the cancer, which was breast cancer. Usually, cutaneous metastatic carcinoma shows solitary and multiple nodules on the trunk, but there are also many unusual clinical findings. Dermatologists should pay attention to the sudden onset of symptomless erythematous patches and nodules on the trunk and conduct a skin biopsy and immunohistochemical staining.
