2.Plasma Fibronectin Levels in Patients with Psoriasis.

Fibronectin(FN) is a physiologic glycoprotein presenton cell surface and in connective tissue, extracellular matrix and plasma. Increased plasma FN levels have been reported in the patients with psoriasis, connective tissue disease, internal malignancy, squamous cell carcinoma and basal cell carcinoma. The author investigated the plasma FN levels in 30 patients (males 13, female 17) with psoriasis and 23 healthy subjects (male 13, female 10) by gelatin sepharose 4B affinity chromatography and Laurells rocket immunoelectrophoresis method. The results were as follows : Plasrna JN levels were 450.4+/-80.3pg/ml in the patients with psoriasis and 334.0-51.3pg/ml in healthy subjects. There was no relationship between age graups and sex. Plasrna FN levels in the patients with psoriasis were higher than those of healthy subjects(p<0.001). There was no relationship between the levels of plasma FN and clinical severity of psoriasis.

6.A Study on the Laboratory Findings in Experimental Osteomyelitis Caused by Staphylococcus aureus

Even though various kinds of diagnostic and therapeutic measures have been used for the detection and cure of the acute osteomyelitis, this disease is difficult to be eradicated in many clinical cases. However it is rather easy to treat once this disease can be detected in early stage. So far radiological examination and many kinds of maneuvers including blood culture, clinical laboratory, xeroradiography, computed tomography have been performed for the early detection of this disease but invalid in the most cases. Recently some authors reported that the bone scanning was useful for the early detection of acute osteomyelitis, but this method also had difficulty in differential diagnosis and uncertaninty of its activity. This study aims to try to find methods of early detection of acute osteomyelitis. Experimental osteomyelitis was developed by the inoculation of Staphylococcus aureus to proximal tibias of 39 young rabbits already sclerosed by solution of sodium morrhuate. By comparing the clinical, radiological, bacteriological and laboratory findings in sequence from 2 days to 7 weeks after inoculation, the following results were obtained. 1. The incidence of experimental osteomyelitis after inoculation of staphylococcus aureus SNUH-S-188 in suspension of 1×10(7) colony forming units was 84%, 74%, and 79% in radiological, blood culture and pathologic examination respectively. 2. The leucocytosis was noticed only in early stage and slight degree of anemia was found during the 1st and 2nd weeks after inoculation. The sedimentation rate was increased only in early acute stage and same was eosinophil count. The leucocyte alkaline phosphatase(LAP) and platelet count. showed no significant abnormal changes. 3. Lymphocyte and eosinophil turned over from normal and eosinophil was predominant in experimental osteomyelitis. 4. Serum calcium, phosphorus, total protein and albumin showed significant decrease in all stages and was conversely paralle1 to changes in groin temperature. The level of BUN was normal indicating normal kidney function in experimental osteomyelitis. 5. Even though the enzymes of which showed no significant changes enrolled alkaline phosphatase and serumgluiamicoxalic-transaminase(S-GOT), but some increa.e was attained to 27% and 44% respectively. 6. It is important and significant to find continuous increase of lactic dehydrogenase (LDH) and creatine phosphokinase (CPK) since 48 hours after inoculation and author, think these will be the reliable and sensitive indices in early detection of the development of osteomyelitis and efficacy of therapeutic measures.

7.A Clinical Study of Ankle Fractures

Through the review of 137 cases of ankle fracture treated and followed up in Department of Orthopedic Surgery, National Police Hospital, we got the following results. 1. Slipping was the most frequent cause of the fractures(45.3%). 2. Supination-external rotation type of Lauge-Hansen classification was more common than any other types(67.9%). 3. 94 cases out of 137 cases were treated by open reduction and internal fixation, and remaining 43 cases were treated conservatively. 4. The average duration of cast immobilization was 7.3 weeks for the operatively treated cases and 9.1 weeks for the conservatively treated cases. 5. The result of treatment was good or excellent in 87 cases(92.6%) of operatively treated 94 cases and in 36 cases(83.7%) of conservatively treated 43 cases. 6. We think that the anatomical reduction of the lateral malleolar or distal fibular fracture is necessary for better end results.

8.A Case of Malignagr Lymphoma.

A 28-year-old male patient is described who developed multiple erythematous nodules and papules on the face, upper chest, back, upper and lower extrimities, leading to extensive ia.filtration of the skin and lymph nodes. Histology from the involved lymph node and skin nodule revealed widely scattered patchy infiltration of abnormal. hyperchromatic polymorphous histiocytes in the dermis. The patient died at about 8 months after onset of the disease, in spite of antineoplastic treatment.

10.A Case of Cutaneous Side Reaction of Collagen Implantation.

Injectable Collagen Implan, is composed of purified bovine dermal collagen emulsified in saline and lidocaine(0.3%) base, and has been promoted for correction of soft tissue contour defects. Although collagen is a relatively nontoxic, nonimmunologic and biolegically nonreactive biomaterial, some side reactions have been reported. We report herein a case of cutaneous side reaction to collagen implantation in a 38-year-old female. The patient presented with puritic, irritating, multiple, erythematous nodules on both the temporal and interorbital area, which had been observed since 1 day after collagen(KOKEN ATELOCOLLA-GEN IMPLANT-Japan) implantation. Histopathologic findings showed dense infiltration of acute and chronic inflammatory cells in the dermis, especially the perivascular and periadnexal area.
