A 44-year-old man had Sweet's syndrome (acute febrile neutrophilic dermatosis), accompanied by erythematous tender subcutaneous nodules resembling erythema nodosum(EN). The EN-like lesions histologically showed a septal panniculitis with predominantly neutrophilic in-filtrates. The association of Sweet's syndrome with EN seems to be uncommon and only a few cases have been reported until the present. We describe a patient with Sweet's syndrome associated with acute EN.

We experienced a case of unilasteral vocal cord paralysis following cuffed endotracheal intubation for abdominal surgery. The patient had have no laryngotracheal symptoms prior to the operation and anesthesia was uneventful. He complained of hoarseness post-operatively but no visible evidence of trauma secondary to the intubation was mainfested on the indirect laryngoscopic examination, except left vocal cord paralysis. There were no obvious causes for the vocal cord paralysis and possible etiologic factors. Therefore, no specific treatment was done except for the bed rest, humidification and gurgling. Vocal cord function returned nearly normal after six months.

BACKGROUND: Diabetes mellitus is a common condition, and the majority of patients have one or more cutaneous complications. Among them, xerosis is well known to be closely related to diabetics. The xerosis is inversely reflected upon skin surface hydration, the state of which can be exactly assessed with various investigating instruments, such as a hydrometer and corneometer. OBJECTIVE: Our purpose was to compare the hydration state of diabetics with normal persons by the use of a corneometer and hydrometer. METHODS: A total of 267 diabetics were asked whether they felt xerosis or pruritus. In a subgroup of 31 patients, the water content was evaluated and compared by the parameters of diabetic severity. Forty-one non-diabetics were selected for the normal control.

A 2 day old male infant, with congenital diaphragmatic herniation on the left side, received diethyl ether inhalation anesthesia, through a 14F endotracheal tube connected with a Jackson-Ree's system for repair of herniation. Throughout anesthesia, the anesthetic gases were not humidified. Signs of high airway obstruction (intercostal space and sternal notch retraction during inspiration, with wheezing and cyanosis) were revealed at the end of operation. Because of in effectiveness of endotracheal suction, immediately the endotracheal tube was extubated, and endotracheal tube obstruction with a dry mucous crust at the tip of the lumen was found. It is strongly suggested that the etiological causes of tube obstruction might be mainly the anesthesia method which has inadequate humidification during anesthesia. This case of endotracheal tube obstruction due to dry mucous crust during inhalation anesthesia was presented and the literature and management are discussed.

Metastatic breast cancer from a primary cervical cancer is extremely rare with few cases reported. A diagnosis of breast metastasis can present difficulties due to various clinical, radiological, and histological manifestations. The important factors that suggest an appropriate diagnosis are the history of the cancer, specific mammographic or ultrasonographic findings and a correlation between the histology of the metastatic and primary tumor. Only an accurate diagnosis can avoid an unnecessary mastectomy and is required to institute an appropriate systemic oncological therapy. An metastatic breast cancer has a poor prognosis. We report a case of a 39-year-old woman with primary cervical cancer who developed a breast metastasis with a review of the literature.

Extramammary Paget's disease is an uncommon intraepithelial malignancy that usually presents as a well circumscribed, reddish eczematous patch on the apocrine-bearing regions such as the genital and perineal areas. It progresses slowly with local extension. However, it may become invasive and may be associated with internal malignancies. Extramammary Paget's disease has high local recurrence rates after complete surgical excision and therefore, long-term follow-up is necessary. Extramammary Paget's disease of the axilla is extremely rare, with few reported cases. We report here a case of extramammary Paget's disease of the axilla in a 68-yr-old man, with a review of the literature.

OBJECTIVE: Teeth discoloration is a rare orthodontic complication. The aim of this study was to report the clinical progression of discoloration during orthodontic treatment. METHODS: Discolored teeth, detected during orthodontic treatment between January 2003 and December 2012 by a single dentist using similar techniques and appliances, were analyzed. RESULTS: The total number of teeth that showed discoloration was 28. Progression of discoloration was evaluated in only 24 teeth that were observed without any treatment. During the observation period, the discoloration “improved” in 8 of the 24 teeth (33.3%) and was “maintained” in 16 (66.6%). The electric pulp test performed at the time of initial detection of discoloration showed 14.3% positivity, which improved to 21.4% at the final follow-up. None of the initial and final follow-up radiographic findings showed any abnormalities. CONCLUSIONS: When teeth discoloration is detected during orthodontic treatment, observation as an initial management is recommended over immediate treatments.

Dr. Youn-Sic Chun and coworkers invented the new machine for getting information about the relative effectiveness of the orthodontic appliances and we named it Calorific machine. The author and colleagues used this machine to compare with tooth moving mechanism by 3 types of the insertion method of precision-TPA for derotating the posterior teeth. We measured the distance of tooth movement and found out the rotational center on the occlusal X-ray film and the tooth movement on the occlusogram and then processed paired t-test by SAS program. The results were as follow : 1. In the intermolar width, the mesial insertion method showed the loss of distance, and the other methods(-distal insertion method, mesial expansion method-) showed the increase of distance. 2. In the arch length, the distal insertion method was exhibited as most recommandable way for increasing the arch length. 3. The rotation center of the mesial insertion method for derotating the molar, was located between mesial pit and central pit of the lower 2nd molar. and, in the distal insertion method, it was located between distal pit and distolingual cusp, and in the mesial expansion method was located at distal pit.

In 38 of 70 patients who underwent lumbar spinal surgery, we performed a randomized, prospective double-blind study of the effect of caudal block with morphine sulfate on postoperative pain levels. Age, sex, clinical features, postoperative analgesic consumption, pain score, adverse effects and patient satisfaction on discharge were recorded. The consumption of parenteral analgesics on the 1st, 2nd and 3rd days was significantly lower in the morphine sulfate group(p<0.05), than in the control group. Caudal block with morphine sulfate also led to marked reductions in pain levels and thus increases patients satisfaction during the postoperative period(p<0.05). Transient respiratory depression occurred in three of 38 patients(7.9%), but further treatment was not needed. Seventeen patients(44.7%) developed urinary retention, but this recovered spontaneously. The present study demonstrates that caudal block with morphine sulfate leads to a highly significant reduction in pain during postoperative periods and a corresponding reduction in the need for additional postoperative analgesics, in addition, patients are more satisfied.
