1.Pregnant Women's Knowledge and Compliance about Prevention of Respiratory Infection.

BACKGROUND: Respiratory infection in pregnancy can cause various side effects and affect the fetus. Therefore, efforts to prevent infection during pregnancy are essential. This study investigated knowledge and compliance regarding the prevention of respiratory infection among pregnant women. METHODS: A survey was conducted on May 10, 2012 on 300 pregnant women who attended a maternity school education program in a tertiary care hospital. The responses of 259 women were collected and analyzed. RESULTS: Only 2 women (0.8%) had been educated about respiratory infection prevention methods, while 106 (40.9%) experienced respiratory infection during pregnancy. The mean score of respiratory infection prevention knowledge was 11.63 out of 15 points (percentage of correct answers: 77.5%). The mean score for compliance to respiratory infection prevention was 32.34 out of 52 points (percentage of practice: 62.19%). Knowledge and compliance were found to be positively correlated. CONCLUSION: Although many pregnant women experience respiratory infection during pregnancy, few have opportunities to be educated about prevention. Thus, the positive correlation between knowledge and compliance highlights the need for respiratory infection prevention education programs.

2.Whitening Effect of Cosmetics Containing Magnesium L-Ascorbyl-2-Phosphate(VC-PMG, Vitamin C Derivatives) Assessed by Colorimeter.

BACKGROUNDS: An inhibitory effect of magnesium L-ascorbyl-2-phosphate (VC-PMG, a stable derivative of ascorbic acid) on melanogenesis has been described. Furthermore, glabridin in licorice is known to have inhibitory effects on melanogenesis and widely used for raw materials for depigmenting agents. OBJECTIVE: The purposes of this study are to provide objective data by measuring the visual clinical effects of VC-PMG with the colorimeter and to promote the development of de-pigmenting agents. METHODS: 20 volunteers joined the study. With an artificial UVB irradiation, eight tanned areas were made on the inner side of the forearm. During two months, each tanned area was treated with five whitening cosmetics with 3% VC-PMG and increasing concentration of licorice from 0% to 3%. Darkness degree of each area was measured weekly by the colorimeter and the visual assessment. RESULTS: For all cosmetics, whitening effect was measured by colorimeter and visual as-sessment. The cosmetic containing VC-PMG 3% and licorice 1% had more whitening effect than any other cosmetics of different concentrations. Moreover, VC-PMG 3% alone also had whitening effect in some volunteers. CONCLUSION: VC-PMG was clinically found to have whitening effect.

3.Tuberculin skin test in newly employed Health Care Workers.

OBJECTIVES: Health care workers(HCWs) are more likely to become infected with tuberculosis from patients in the hospital. When HCWs have an active tuberculosis infection, it is possible that they also become a source of infections to other HCWs and patients. METHODS: Tuberculin tests were performed with mantoux method using 5 units of tuberculin by infection control practitioners on Feb. 21, and Mar. 7, 1997. A total of 138 newly employed HCWs including 44interns and 94 nurses were tested. Results of skin test were interpreted by themselves according to the instructions. RESULTS: Among 138 personels, skin tests were negative in 27 personels (19.6%); 4 interns (9.1%) and 23 nurses (24.5%). There were 3 persons (2.2%) who had a history of tuberculosis which had been treated. CONCLUSION: The study showed that approximately 20% of newly employed HCWs had negative tuberculin skin test. Given the possibility of being infected with tuberculosis in the hospital, regular follow up of tuberculin skin test is warranted to protect HCWs,

5.A Case of Adenomyotic Cyst Within Myometrium Accompanied with Endometriosis.

Adenomyotic cyst is very rare disease, their sizes are mostly lesser 5mm. The intrauterine adenomyotic cyst may arise from progressive expansion of cyst due to progressive menstrual bleeding. Authors experienced a case of large adenomyotic cyst within myometrium occuring in a l9-year-old woman, and who was accompanied with endometriosis. The cyst was about 3 x 3em sized, and had chocolate colored thick viscous contents, We experienced one case of adenomyotic cyst which was thought to be degenerated uterine myoma, so we report the case with a brief review of the concerned literatures.

6.Three Cases of Vesicular Pemphigoid.

We report herein three cases of vesicular pemphigoid that is a varient of bullous pemphigoid, occuring in 38-year old female, 51-year old male, and 35-year old female patients. They had recurrent and moderate to severe pruritic tense vesicles with symmetrical distribution chiefly on the head and neck areas. The histological findings showed subepidermal bullae which contained a few eosinophils and the direct irnmunofluorescent tests revealed linear deposition of IgG, IgM and C, on the basement mernbrane zone. Futhermore, all the patients showed a good response to dapsone.

7.The Efects of Autologous Blood Pleurodesis in the Pneumothorax with Persistent Air Leak.

BACKGROUND: In patients with severe chronic lung diseases even a small pneumothorax can result in life-threatening respiratory distress. It is important to treat the attack by chest tube drainage until the lung expands. Pneumothorax with a persistent air leak that does not resolve under prolonged tube thoracostomy suction is usually treated by open operation to excise or oversew a bulla or cluster of blebs to stop the air leak. Pleurodesis by the instillation of chemical agents is used for the patient who has persistent air leak and is not good candidate for surgical treatment. When the primary trial of pleurodesis with common agent fails, it is uncertain which agent should be used f or stopping the air leak by pleurodesis. It is well known tbat inappropriate drainage of hemothorax results in severe pleural adhesion and thickening. Based on this idea, some reports described a successful treatment with autologous blood instillation for pneumothorax patients with or without residual pleural space. We tried pleurodesis with autologous blood for pneumothorax with persistent air leak and then we evaluated the efficacy and safety. METHODS: Fifteen patients who had persistent air leak in the pneumothorax complicated from the severe chronic lung disease were enrolled. They were not good candidates for surgical treatment and doxycycline pleurodesis failed to stop up their air leaks. We used a mixture of autologous blood and 50% dextrose for pleurodesis. Effect and complications were assessed by clinical outcome, chest radiography and pulmonary function tests. RESULTS: The mean duration of air leak was 18.4 :1:6.16 days before ABP (autologous blood and dextrose pleurodesis) and 5.2 ± 1.68 days after ABP, The mean severity of pain was 2.3 ± 0.70 for DP(doxycycline pleurodesis) and 1.7 ± 0.59 for ABDP (p<0.05). There was no other complication except mild fever. Pleural adhesion grade was a mean of 0.6 ± 0.63. The mean dyspnea scale was 1.7 ± 0.46 before pneumothrax and 2.0 ± 0.59 after ABDP (p>0.05). The mean FEV1 was 1.47 ± 1.01 before pneumothorax and 1.44 ± 1.00 after ABDP (p>0.05). Exɡpt in 1 patient, 14 patients had no recurrent pneumothorax. CONCLUSION: Autologous blood pleurodesis (ABP) was successful for treatment of persistent air leak in the pneumothorax. It was easy and inexpensive and involved less pain than doxycycline pleurodesis. It did not cause complications and severe pleural adhesion. We report that ABP can be considered as a useful treatment for persistent air leak in the pneumothorax complicated from the severe chronic lung disease.

9.A Case of Granulocytic Sacoma in Esophagus.

Granulocytic sarcoma.(chloroma) is a localized tumor mass composed of immature cells of the granulocytic series infiltrating an extramedullary site. The tumor may involve anywhere in the body, especially orbit, bone, epidural space and lymph node, but there is no case involved the esophagus. In a 34-year-old male presented with sudden anset of dysphagia, esophagoscopic examination revealed concentric narrowing of the esophageal lumen wirith alight granular mucosa in the midesophagus, which hiatologically proved to b a granulocytic sarcoma. Peripheral blood and bone marrow aspiration smears showed blastic crisis of chronic myelocytic leukemia. After the systemic chemotherapy, dysphagia and mid-esophageal luminal narrowing disappeared completely.

10.Efficacy of the Preventive Abduction Brace in Dementia Patients After Bipolar hemiarthroplasty of the Hip.

Purpose: To evaluate the efficacy of the preventive abduction brace in dementia patients after bipolar hemiarthroplasty of the hip. Materials and Method: A review of 30 patients who had dementia prior to bipolar hemiarthroplasty of the hip. 20 females and 10 males were evaluated and their mean age was 80.4 years old (range, 68 to -92 years old). The average MMSE-K in dementia patients was 14.2 (range, 8 to 20). The reasons for the bipolar hemiarthroplasties were femoral neck fractures in all the cases. We analyzed the dislocation rates of those who had preventive abduction braces applied prospectively. Results: The average follow-up period after hemiarthroplasty was 2.2 years. All 30 patients had preventive abduction braces and no dislocations occurred after the bipolar hemiarthroplasties. Conclusion: Hip dislocations were not detected in those patients who preventive abduction braces applied. The present findings suggest that having a preventive abduction brace will definitely reduce the dislocation rate in dementia patients who have difficulties with rehabilitation after their operations.
