1.The impact of large amount whole cow's milk intake on iron status in early childhood.

To determine the impact of intake large amount whole cow's milk (WCM) on iron status during early childhood (18~36 months), selected indices of complete blood count(CBC) and iron status were compared between the WCM large amount intake group (n=20)and small amount intake group (n=20). WCM large amount intake children's mean hemoglobin and mean hematocrit were 9.6+/-1.7 g/dL and 30.8+/-4.5% respectively. These range of peripheral blood with other indices of CBC disclosed the hypochromic microcytic anemia. WCM large amount intake children's some indices of CBC-hemoglobin, hematocrit, MCV, MCH-were significantly lower than those of small amount intake children (P<0.01). WCM large amount intake children had significantly lower mean iron, transferrin saturation and ferritin (P<0.01) and greater mean total iron binding capacity (P<0.05) than did small amount intake children. It is concluded that intake large amount of WCM for long duration impacts on the iron status of 18~36 months old children disadvantageously. It should be avoided for the protection of irreversible recognitive impairment and behavioral change.

3.Successful Reposition of Prolapsed Silicone Tube Using Hole and Lacrimal Probe Method

Purpose:
Silicone tube intubation is the most common method for treatment of nasolacrimal drainage disorder or repair of lacerated canaliculus. However, some cases are prolapsed earlier than expected, subsequently affecting the success rate of silicone tube intubation. The author introduced a successful reposition of the prolapsed silicone tube.
Methods:
This study is a retrospective case series of 11 patients who were treated with reposition of prolapsed silicone tube using hole and lacrimal probe method. In all cases, the tubes were prolapsed within 2 months after the first surgery due to careless rubbing of the medial canthus by the patient. In all 11 cases, the development of a biofilm on tube was not observed at the time when the tube was prolapsed. All patients underwent immediate reposition of prolapsed tube without local anesthesia in the outpatient operating room on the day they visited.
Results:
In all 11 cases, the prolapsed silicone tube was repositioned without any complications using this hole and lacrimal probe method. At 6-month follow-up examination, all patients presented with a well-positioned silicone tube up to the day of extubation without replacing it with a new silicone tube.
Conclusions
This hole and lacrimal probe method is effective for the repositioning of prolapsed silicone tube after bicanalicular intubation of lacrimal system. This procedure includes easy performance in a short time and without local anesthesia in the outpatient operating room.

5.Chimeric Antigen Receptor-T Cell Therapy

Chimeric antigen receptor (CAR) T-cell therapy constitutes a revolutionary advancement in personalized cancer treatment. During this treatment, a patient's own T cells are genetically engineered to express a synthetic receptor that binds a tumor antigen. CAR-T cells are then expanded for clinical use and infused back into the patient's body to attack cancer. CAR-T cells have produced remarkable clinical responses with B-cell malignancies. However, CAR-T cells therapy is not without problems. Barriers to effective CAR-T cells therapy include severe life-threatening toxicities and modest anti-tumor activity. In this review, we introduce the concept of CAR-T cells therapy, currently available CAR-T cells therapy options, and how to deal with adverse events.
6.One Family with Asymmetric Septal Hypertrophy.

One family with asymmetric septal hypertrophy was reported. The propositus of this family was 31 years old man who was admitted because of transient syncope. His father and one brother were affected but asymptomatic. His sister was suddenly died at age 21 years. The interventricular septal thickness to left ventricular posterior wall thickness ratios were measured using M-mode echocard ogram. In the affected family, echocardiogram revealed asymmetric septal hypertrophy (1.5:1, 2:1, 1.9:1 respectively). In the propositus, cardiac catheterization and simultaneous biventricular cineangiography were performed. Pressure study revealed mild peak systolic pressure gradient within left ventricular apex and left ventricular outflow tract (4mm Hg respectively). Biventricular cineangiogram showed the septal width increased inferiorly, and left ventricular endocardial surface of the septum was straight, while the right ventricular border convex toward the right ventricle.

7.A Case of Metastatic Choriocarcinoma following Preterm Delivery at 34th Week of Pregnancy.

Choriocarcinoma associated with a normal pregnancy is rare. Futhermore, choriocarcinoma coexistent with a viable pregnancy is even rarer and associated with a greater risk of hepatic and cerebral involvement. So timely diagnosis of the disease is important for successful treatment and aggressive diagnostic procedures may therefore warranted. The patient should be treated with primary intensive combination chemotherapy(EMA-CO) and the selective use of irradiation and surgical therapy. We experienced a case of metastatic choriocarcinoma of lung and kidney following preterm delivery at 34th week of pregnancy and so present it with brief review of literature.

8.A case of congenital hypertrophic pyloric stenosis in two siblings.

Congenital hypertrophic pyloric stenosis is the most common intra-abdominal disease required surgery during the first few months of life. The expression of pyloric stenosis is dependent upon the genetic influence of ancestors affected with the disease, as well as unknown environmental influences in the postnatal period. Pyloric stenosis has been reported in multiple sibs in a family, which suggests the genetic influence on the expression of this disease. Until now, the genetic influence is thought a sex-modified polygenic or multifactorial background which facilitates the expression of a common dominant gene. We experienced a case of congenital hypertrophic pyloric stenosis in a two siblings. The siblings suffered projectile vomiting for 2~4 days at 16 days old of age and 15 days old of age. After we confirmed the diagnosis by upper gastrointestinal series and abdominal sonogram, the Fredet-Ramstedt pyloromyotomy was done successfully. This case suggests the genetic influence on the expression of this disease.

9.Synchronous Occurrence of Papillary Thyroid Carcinoma and Mucosa-Associated Lymphoid Tissue Lymphoma: a Single Case Report.

Papillary thyroid carcinoma (PTC) is a common lesion, accounting for 70-80% of all thyroid cancers, whereas mucosa-associated lymphoid tissue (MALT) lymphoma of thyroid gland is rare. A simultaneous occurrence of both malignancies is extremely rare. 57 years old Korean woman diagnosed with Hashimoto's thyroiditis at left lobe of thyroid gland where atypical cells of undetermined significance at right lobe. Later, left lobe was confirmed with malignant lymphoma during series of fine-needle aspiration biopsy. Right lobe was interpreted as malignant lesions, such as papillary thyroid carcinoma based on ultrasonography images and previous biopsy results. Total thyroidectomy was performed. Pathology reported papillary thyroid carcinoma at right lobe and MALT lymphoma at left lobe. There were no post-operative complications and no recurrence yet reported. Since an association between Hashimoto's thyroiditis and development of MALT lymphoma has been reported previously, a history of Hashimoto thyroiditis should be suspected MALT lymphoma.
