2.Cholecystogastric Fistula and the Transmigration of the Gallstone.

The internal biliary fistula between the gallbladder or bile duct and intestinal tract is a rare disease. Because symptoms and signs of biliary fistula are not specific, these patients are commonly investigated with computed tomography, endoscopic retrograde cholangiopancreatography (ERCP) and barium study. Among them, a cholecystogastric fistula is a very rare type of biliary fistula. We recently experienced a case of cholecystogastric fistula and transmigration of the gallstone to the stomach. A 53-year-old woman was transferred with the impression of gallbladder cancer. On evaluating the cause of right upper abdominal pain and nausea, we confirmed the cholecystogastric fistula with gastroscopy, upper GI series and ERCP. Cholecystectomy and the repair of the fistula was done.

3.A case of primary biliary cirrhosis associated with autoimmune primary hypothyroidism.

Primary biliary cirrhosis (PBC) is an idiopathic chronic non-suppurative destructive inflammation of intrahepatic bile ducts and rare in Korea. Although its pathogenesis is not fully understood, immunologic dysregulation is suspected because of presence of antimitochondrial antibody (AMA) and association with other autoimmune disease, such as autoimmune thyroid disease. Evidence of hypothyroidism in PBC is relatively common in western countries, however it has not been reported in Korea. And it is uncertain whether the reason is ethnic factor or unawareness of the disease. Recently, we experienced a 50-year-old women with hypothyroidism who had elevated alkaline phosphatase and gamma glutamyl transpeptidase. This is the first case of hypothyroidism associated with PBC in Korea.

Leiomyoma is a neoplasm of smooth muscle relatively common in the stomach, but very rare in the duodenal location. Recurrent bouts of severe melana remain the most frequent presenting symptom for leiomyoma. A 69-year-old man presented to us with a 4-day history of melena. Endoscopy revealed norinal mucosa covered ovoid elevated lesion with central linear depression and ulcer at the second portion of duodenum. Endoscopic biopsy showed chronic inflammation only. Duodenal mass was excised through a duodenotomy. Pathological diagnosis was leiomycena.

5.Newborn Hearing Screening: 3 Years of Experience.

PURPOSE: Hearing loss is one of the most common major abnormalities present at birth, which has an incidence of 1 to 3 per 1,000 newborn infants in the well-baby nursery population, and 2 to 4 per 1,000 infants in the intensive care unit population each year. If early undetected, will impede speech and language. The purpose of this study was to confirm in prevalence of neonatal hearing loss and to establish a common screening method adjusted to our country and to emphasize the importance of early detection. MATERIALS AND METHODS: TEOAE (transient evoked otoacoustic emission) were performed in 5,512 newborn infants in the well-baby nursery. The tests were performed daily until the infant had passed. Failed infants were followed at the outpatient clinic for re-tests. ABRs were performed for the confirmation of hearing loss those who had failed 3 TEOAE tests. RESULTS: The average test durations for right and left TEOAE were 67+/-50 sec and 72+/-56 sec respectively. There was no difference in test durations of the first TEOAE between before 24 hours and after 24 hours of life. 89% of tested infants passed during admission and the rest were followed at the outpatient clinic for the further studies. Eight infants were diagnosed with hearing loss on ABR. Overall time spent for the diagnosis of hearing loss was less than 3 months. CONCLUSION: TEOAE is a simple and useful screening method for the identification of hearing loss in infants. TEOAE must be necessary to universal screening of all infants.

6.One Case of Duplicated Ureter Associated with Ectopic Ureteral Orifice.

In a case, while normal self-voiding, urinary leakage has been complained since infant without other urinary symptoms and she was treated with right ureteroheminephrectomy.

8.A Case of Primary Ovarian Pregnancy.

Primary ovarian ectopic pregnancy has remained a rare event. It is difficult preoperatively to diagnose and differentiate ovarian pregnancy from hemorrhagic ovarian cyst and tubal ectopic pregnancy. However, improved diagnosis of acute hemoperitoneum of ovarian origin may reveal a higher incidence than reported earlier. The diagnostic criteria were described in 1878 by Spiegelberg, which comprise that the pregnancy is in the ovary and does not involve the tube. Ovarian pregnancy occurs within the ovary and on the corpus luteum, usually with rupture of the ovary and a massive hemoperitoneum. We report a relatively intact primary ovarian pregnancy with a brief review of the literatures.

BACKGROUND: Adolescence is an important period with respect to health promotion, because development of health behavior at this stage is the foundation of health in adalts. Among the social factors, family factor which affects the primary social function can be the most fundamental. We investigated the effects of father's health behavior on the health behavior of their children during adolescence. METHODS: From June to August 1999, 300 adolescent students from middle schools and high schools of areas including four cities (Seoul, Incheon, Anyang, Gunpo) and their fathers were questioned about general characteristics and health behavior such as smoking, alcohol drinking, exercise, duration of sleep, body weight, height and eating habits for breakfast ('Alameda 7'). The relationship between father's health behavior and their adolescents' health behavior was analyzed by chi-squared test. RESULTS: Among 300 adolescents and their fathers, 211 pairs responded appropriately to the questionnaires. Adolescent's smoking was not associated with father's smoking. In drinking there was no significant association between father and adolescent, but in drinking amount, father's high drinking amount was significantly associated in all students, especially in females and high school students. Regular diet, exercise, sleep did not show significant association between father and adolescent. The proportion of obesity in adolescents whose fathers were obese was significantly greater compared to those whose fathers were not. CONCLUSION: High drinking amount and obesity had a statistical significance of association between fathers and adolescents. When we consider adolescents' health behavior, their fathers' health behavior must be considered.
