The effect of Roentgen rays on carcinoma of the cervix has long been of great interest to both radiologists and gynecologists. Since most cervical carcinomas are treated by irradiation, any additional knowledge either concerning the radiosensitivity of cervical tumors or their ultimate prognosis would be of value. The vaginal smear is considered to be one of convenient and rapid methods to study the effects of radiation on cervical malignancy. We observed morphologic changes in 297 cytologic preparations obtained from 60 patients who had underwent irradiation for cancer of the cervix. With the morphologic parameters such as cytoplasmic vacuolization, cytoplasmic basophilia, multinucleated giant cell formation, polymorphonuclear leucocytes (PMNL) sticking and postradiation dysplasia, we analyzed the findings in relation to the follow up time interval. The most common effect was the cytoplasmic vacuolization with basophilia of basaloid cells, which were noted in more than 90% of followed patients. The multinucleated giant cell formation and PMNL stickering were noted in 38 cases(63%) and 48 cases(80% ) respectively. The differential diagnosis of postradiation dysplasia from recurrent or persistent carcinoma, reparative atypical cells, and regressing tumor cells was difficult and further study seems to be needed to clarify the more accurate morphologic features and biologic behavior.

The time course of recovery in vestibular neuritis varies between individuals. The aim of this study was to identify the predictors for the early or late recovery of vestibular neuritis. The inclusion criteria were patients 1) who had an acute onset of vertigo lasting at least 24 hours, 2) with a horizontal-torsional unidirectional spontaneous nystagmus, and 3) with a canal paresis of 20% or more on the bithermal caloric tests. The primary endpoint for this study was an early or late recovery of vestibular neuritis as a dependent variable. A functional level scale was used to define the late recovery (5 or more points) at seven days after the symptom onset. The secondary endpoint was the duration of hospitalization. One hundred twenty eight patients met the inclusion criteria for this study, and among them, 71 patients had an early recovery. Multiple logistic regression analysis showed that diabetes mellitus was the only independent significant variable for the prediction of a late recovery of vestibular neuritis. In addition, the diabetes mellitus was a predicting variable for long duration of hospitalization. Diabetes mellitus was a predictor for a late recovery of vestibular neuritis.

The management of distal femur fractures are difficult because of many complications including nonunion, delayed union, infection, loss of fixation, joint stiffness and angular deformity. Until 1960s, conservative management was considered superior to operative treatment in distal femur fracture. But, with advancement of new fixation devices and techniques, open reduction and internal fixation is recent trend. For the purpose of analyzing the factors effecting the results, we studied the 36 cases of distal femur fracture treated surgically from may, 1991 to May, 1994. By AO classification nineteen cases were type C fracture and seventeen cases were type A. According to Schatzker and Lambert criteria, excellent results were in 10 cases, good in 16, fair in 6 and fail in 4. We have analysed the results by fixation device, age, osteoporosis degree, fracture classification, open or closed fracture, operative approach in type C fracture and bone graft. Fair and fail results were more common in the cases of severe osteoporotic bone, type C with lateral approach and open fracture. Excellent and good results were more common in the cases of using the anatomical plate and dynamic condylar screw. We concluded that treatment device should be decided by fracture type, degree of communication, degree of osteoporosis and soft tissue state, and then satisfactory results will be obtained by accurate anatomical reduction, rigid internal fixation and early exercise.

Benign symmetric lipomat,osis is a rare disease of undetermined cause characterized by symmetric deposits of adipose tissue o:n the neck, upper trunk and abdomen. We report three cases of kienign symmetric lipomatosis, one of which is associated with alcoholic liver disease and the patient complained of a diffieulties in neck motion, All of them are about sixty year old men with history of frequent alcohol consumption. In histopathologic findings, the reticular dermis was completcly replaced by mature fat cells and there was a slight increase in vascular and fibrous elements.

Measuring motor nerve conduction (MNC) in the hypoglossal nerve is an uncommon electrophysiologic study. However, this measurement can be easily performed with a wooden tongue depressor, to which a bipolar bar electrode is fixed by skin tape. We measured the MNC of the hypoglossal nerve in a patient with idiopathic unilateral weakness of the tongue. In the initial MNCs of the hypoglossal nerves, the CMAP amplitude was decreased and the latency was delayed in the lesion site when compared to the healthy site. In the follow-up study after 3 months, similar findings were found. Comparing the initial study with the follow-up one, however, the CMAP amplitude was increased and the latency was shortened in the lesion site. In conclusion, the MNC in the hypoglossal nerve is easy to measure and is useful for the evaluation of the hypoglossal nerve damage.

PURPOSE: In order to evaluate the prognostic factors of vitrectomy for the teatment of retinal detachment due to macular hole. METHODS: The medical records of 19 patients who had undergone pars plana vitrectomy were analyzed. RESULTS: After the first operation, the retinal reattachment was achieved in 12 patients out of 19 patients(63.2%). Among 7 patients who had persistent retinal detachment after the first operation, 6 patients underwent reoperation and 4 out of whom were successfully reattached. The final success rate was 88.9%(16 out of 18 patients). Between 12 patients with successful first operation and the remaining 7 patients, there were statistically significant differences in the incidence of posterior staphyloma, myopic chorioretinal atrophy, proliferative vitreoretinopathy, retinal detachment in two quadrant or more, and complete removal of epiretinal membrane(p<0.05). CONCLUSION: These results suggest that surgical success of post-operation of retinal detachment due to macular hole seems to be related to complete epiretinal membrane removal, proliferative vitreoretinopathy, posterior staphyloma, myopic chorioretinal atrophy and extension of retinal detachment.

OBJECTIVES: This study investigated the prescribing patterns of atypical antipsychotics for the various psychiatric disorders in the psychiatric ward of a University hospital. METHODS: We reviewed the medical records of patients who were discharged from an open psychiatric ward from May, 2003 through April, 2014. The association between psychiatric disorders and prescription pattern of atypical antipsychotics was analyzed. RESULTS: The study included 3091 patients' prescription of psychiatric medication. 60% of prescription included antipsychotics; quetiapine was the most frequently prescribed antipsychotics, but the average dosage was the lowest among all the atypical antipsychotics. According to the diagnoses, prescription rates and dosage of antipsychotics were different. Prescription rates of antipsychotics were the lowest in patients with anxiety disorders, and the mean dosage were the lowest in those with delirium, dementia, and amnestic and other cognitive disorders. CONCLUSION: This observational study shows prescription patterns of atypical antipsychotics for the treatment of psychiatric disorders in a University hospital; atypical antipsychotics were widely used for the treatment of the various disorders, and there were differences in prescription patterns for each disorders. The results of this study may be used to identify the proper atypical antipsychotics effective on certain psychiatric disorders and to propose expanding the indications of each atypical antipsychotics in the future.

The tibial condylar fractures involving the articular surface can produce some disability of the knee joint because it is frequently accompanied by soft tissue injury to the ligaments and menisci. Accurate anatomical reduction and rigid internal fixation with early motion is known to decrease the complications. During a period of 10 years, from January 1978 to December 1987, we treated 105 tibial condylar fractures at Severance Hospital. Of the above, 77 cases have been analyzed according to the classification, cause of injury, method of treatment, final result and complication. The 77 cases were classified as follows ; total condylar depression 17(22%), undisplaced 16 (21 %), split compression 16(21 %), comminuted 14 (18%), local compression 5 (7%), split 1 (1%) and others 8(10%). Among these, 59 cases revealed the result of “ACCEPTABLE” according to Porter's criteria. Of the conservative group, 86% obtained the rating of “ACCEPTABLE” and 67% of the operative group did as well. Possible complications include traumatic arthritis (9), limited motion (9), wound infection (6), instability (3), angular deformity (2), intraarticular loose body (2), nonunion (1) and myositis ossificans.
