1.A Case of Eccrine Chromhidrosis Caused by Silver lodide.

Eccrine chromhidrosis(colored sweating), resulting from the contamination of the sweat by dyes, pigment from microorganisms such as those of Piedraia or Corynebacterium, or other chernicals, is a very rare disease. We report a case of eccrine chromhidrosis which revealed dark-brown colored pigmented spots analyzed by energy dispersive X-ray microassay and scanning electron microscopy, which were thought to be composed of silver iodide(AgI) formed by the conjugation of silver components from outside and iodide components of eccrine sweat, on the face, postauricular area, legs, and finger tips.

2.Relationship of Body Fat Distribution and Serum Lipids in Men.

This study was conducted to investigate the relationship of body fatness indicators and serum lipids (total cholesterol, high density lipoprotein (HDL) cholesterol, low density lipoprotein(LDL) cholesterol, triglycerides, myocardial infarction(MI) index) to find anthropometric indicators of body fat distribution that are strongly associated with serum lipids. 347 adult male workers of an industry in Kyungpook province were selected and data on the general characteristics were obtained using questionnaire from June 26 to June 29, 1991. Serum lipids were tested with 5 ml fasting blood and height, weight, circumferences of waist, hip and mid-thigh, five subcutaneous fat thickeness(biceps, triceps, supscapular, suprailiac, thigh) were mea sured using Ultrasound type A TATT. Waist circumference, waist/hip circumference ratio and subcutaneous fat thickness of suprailiac area are significantly increased with increasing age, but thigh circumference is significantly decreased (P< 0.05). These results suggest that central obesity is closely related to age and waist circumference may be the best indicator of body fatness or body fat distribution in men. In younger age group(below 40 year old), the result of canonical correlations analysis suggests that circumferences(0.501) was slightly closely related to serum lipids than subcutaneous fat thickness(0.493), however, in older age group (above 40 year old), subcutaneous fat thickness(0.528) were more strongly associated with serum lipids than circumferences(0.419). Weighted canonical analyses suggest that biceps and suprailiac areas in younger age group and sub scapular area in older age group are stronger indicators of serum lipids among .subcutaneous fatness measurements. Of the circumferences, waist is the most strongest indicator of serum lipids in both age groups. Therefore, it is recommended that circumference of waist and subcutaneous fat thickness of subscapular area should be measured to estimate the degree of central obesity for preventing cardiovascular disease in men.

The effect of Roentgen rays on carcinoma of the cervix has long been of great interest to both radiologists and gynecologists. Since most cervical carcinomas are treated by irradiation, any additional knowledge either concerning the radiosensitivity of cervical tumors or their ultimate prognosis would be of value. The vaginal smear is considered to be one of convenient and rapid methods to study the effects of radiation on cervical malignancy. We observed morphologic changes in 297 cytologic preparations obtained from 60 patients who had underwent irradiation for cancer of the cervix. With the morphologic parameters such as cytoplasmic vacuolization, cytoplasmic basophilia, multinucleated giant cell formation, polymorphonuclear leucocytes (PMNL) sticking and postradiation dysplasia, we analyzed the findings in relation to the follow up time interval. The most common effect was the cytoplasmic vacuolization with basophilia of basaloid cells, which were noted in more than 90% of followed patients. The multinucleated giant cell formation and PMNL stickering were noted in 38 cases(63%) and 48 cases(80% ) respectively. The differential diagnosis of postradiation dysplasia from recurrent or persistent carcinoma, reparative atypical cells, and regressing tumor cells was difficult and further study seems to be needed to clarify the more accurate morphologic features and biologic behavior.

5.A Case of Impetigo Herpetiformis.

We report herein a case of impetigo hepetiformis treated with etretinate. The patient was 30 year old female in the 7th month of her second pregnancy, who had suffered from generalized coalesced pustular eruption with slightly itching and hurning sensation with fever on whole body except face, both had and foot. The euption began as an erythemations patch which developed pustules along the mar gin. I'here was extensian by the develupment of fresh peripheral pustules and coaleence of these patches. Histopathological findings showed hyperkeratosis, subcorneal spongiform pustule, spon,iosis in the epidermis and vascular proliferation in the devmis, She was treated successfully with etretinate for 4 weeks.

8.The Changes of Neurotransmitters in Central Sensitization after Peripheral Nerve Injury in Rats.

There has been much interest in the involvement of wind-up in the hyperalgesia and allodynia of chronic pain syndrome. For investigating the hehaviorml parallels of this wind-up phenomenon, it was limed to evaluate the changes of neurotransmitters(aspartate. lutamate, and taurine) in central sensitization after formalin-induced peripheral nerve injurv in the rats. The experiment was divided into two subexperiments; One was the formalin test.;and the other was microdialysis for evaluation of neurotransmitters. In formalin test (n=7), 50 microliter of 5% formalin was injected suhcutaneously in the foot dorsum of the right hind limb with a 30 G neeclle. To cluantify the formilin response. the number of spontaneous t1inches was counted at 1-2 minutes and 5-6 minutes and at 5 minutes intervals during the period 10- 60 minutes after formalin injeclion. In microdialysis study. the experimental animals were divided randomly into two groups. In experimentl group (n=10). the microdialysis catheter was perfused with artificial cerehrospinal fluid by 3 microliter/min for one hour. Then. the two beseline dialysis samples were collected and 50 microliter of 5% formalin was injected .iuhcutaneously in thc plantar surface of the right hind paw with a 30 G needle. Three 10 minutes dialysate samples were collected after formalin injection. In control group(n=10), the normal saline instead of formalin was used. The results were as followed: In formalin test, there was a biphasic pattern of flinchine after formalin injection. The peak in phase I was shown at 2 minutes after formalin injection. The second peak was shown at 35 minutes alter formalin injection during phase II. In microdialysis study, thcre were statistically significant increascs of aspartate and lutamate concentrations, excitatory neuro transmitters, during the first 10 minutes after formalin injection. After lhen, all values of these neurotransmitters were rcturned to baseline values. The concentrations of taurine, an inhihitory neurotransmitter, was shown a tendency of an increase after formalin injection but, it was not statistically significant. From these results, it secms that the excitatory amino acid ncurotransmitter may partly play a role to triqer the central sensitization after peripheral nerve injury.

10.The Operative Treatment for the Fractures of Distal Femur

The management of distal femur fractures are difficult because of many complications including nonunion, delayed union, infection, loss of fixation, joint stiffness and angular deformity. Until 1960s, conservative management was considered superior to operative treatment in distal femur fracture. But, with advancement of new fixation devices and techniques, open reduction and internal fixation is recent trend. For the purpose of analyzing the factors effecting the results, we studied the 36 cases of distal femur fracture treated surgically from may, 1991 to May, 1994. By AO classification nineteen cases were type C fracture and seventeen cases were type A. According to Schatzker and Lambert criteria, excellent results were in 10 cases, good in 16, fair in 6 and fail in 4. We have analysed the results by fixation device, age, osteoporosis degree, fracture classification, open or closed fracture, operative approach in type C fracture and bone graft. Fair and fail results were more common in the cases of severe osteoporotic bone, type C with lateral approach and open fracture. Excellent and good results were more common in the cases of using the anatomical plate and dynamic condylar screw. We concluded that treatment device should be decided by fracture type, degree of communication, degree of osteoporosis and soft tissue state, and then satisfactory results will be obtained by accurate anatomical reduction, rigid internal fixation and early exercise.
