2.A Study on Serum Lipid Profiles in Normal and Patients with Cardiovascular Diseases: Serum HDL-Colesterol.

Serum levels of HDL-cholesterol, triglyceride and total cholesterol were measured in 107 normal Koreans and in 327 patients; 66 patients with hypertension, 34 patients with coronary heart disease, 45 patients with cerebral thrombosis, 18 patients with nephrotic syndrome, and 164 patients with diabetes mellitus. Patterns of serum lipoprotein fractions were also investigated by agarose gel electrophoresis in 41 normal Koreans and in 72 patients; 14 patients with hypertension, 10 patients with coronary heart disease, 19 patients with cerebral thrombosis, 7 patients with nephrotic syndrome, and 22 patients with diaebetes mellitus. The results are summarized as follows; 1. Mean values of serum HDL-cholesterol, triglyceride and cholesterol in normal Koreans were 52.2+/-12.4mg/100ml, 110.6+/-31.6mg/100ml and 175.3+/-21.4mg/100ml respectively. No significant difference in mean value of serum HDL-cholesterol was observed between Korean males and females. 2. In Korean females serum, HDL-cholesterol level showed peak values in the fifth decade, but no significant difference with aging was observed in Korean males. 3. Serum HDL-cholesterol levels were significantly decreased in patients with hypertension, coronary heart disease, cerebral thrombosis, and in male diabetic patients with complication. 4. Serum triglyceride levels were significantly increased in all the disease groups, and serum cholesterol levels were significantly increased in all the disease groups except cerebral thrombosis. 5. Mean values of alpha-lipoprotein, pre-beta-lipoprotein and beta-lipoprotein fraction ratios in normal Koreans were 28.9+/-7.5%, 14.9+/-4.9% and 56.2+/-8.1% respectively. 6. Seum alpha-lipoprotein fraction ratio was significantly decreased in relatively advanced hypertensive patients. Pre-beta-lipoprotein fraction ratio tends to be increased in patients with hypertension, cerebral thrombosis, nephrotic syndrome and in diabetic patients with complications, and beta-lipoprotein fraction ratio tends to be increased in patients with hypertension, coronary heart disease and nephrotic syndrome, but those were not statistically significant.

4.Therapeutic Endoscopic Retrograde Cholangiography in Patients with a Billroth II Gastrectomy: 2 cases of ERBD & 1 case of endoscopic stone retrievial.

Endoscopic retrograde cholangiopancreatography(ERCP) procedures are more difficult in patients who have undergone partial gastrectomy with Billroth II anastomosis. Because its altered anatomical relationship. the endoscopist is presented with additional problems: (i) Dfficulties in entering the afferent loop, depending on the surgical techiques used. (ii) The endoscope may be too sort to reach the papillary region unless the loops are suecessfully straightened out. (iii) Difficulties in passing the ligament of Treitz, especially in patients with Braun's anastomosis, (iv) Problems in cannulating the papilla and especially the common bile duct from a reversed position. (v) Problems in carrying out a papillotomy in a correct position. We attempted endoscopic sphincterotomy in 3 opatients previously subjected to gastrectomy with needle knife, and succeeded in 2 of them. In the two patients, successful billary drainage was achieved. And one patients with Billroth II gastrectomy, presented with CBD stone and cholangit, was successfully treated with endoscopic stone retriveial. The patient with a Billroth-II operation may unergo endscopic diagnostic as well as therapeutic procedures with a high rate of success, and can be suitable candidates for ERCP and endoscopic sphincterotomy

6.Papillary Cancer Arising in Remnant Thyroglossal Duct.

Remnant thyroglossal ducts are common developmental abnormalities of the thyroid gland. The malignant transformation of these structures is extremely rare. Even though a malignancy it is highly suspected in elderly patients with midline neck swelling, the diagnosis is seldom made preoperatively. In thyroglossal duct carcinomas, a papillary carcinoma is the most common pathologic finding. The etiology of such tumors is unclear, but a de novo origin and spread from primary thyroid-gland tumors has been suggested. There are different theories regarding the origin of and the therapy for these malignancies. This has important significance for therapeutic approaches. We report a case of a papillary cancer arising in a remnant thyroglossal duct which was managed by Sistrunk's operation and right thyroid lobectomy.

9.The Effects of Single Epidural Triamcinolone Injection on the Blood ACTH and Cortisol Level.

BACKGROUND: Epidural steroids injections are often used for the treatment of low back pain but their effects on the endocrine system have not been determined. Few studies have quantified the degree or duration of the suppression of the hypothalamic-pituitary-adrenal (HPA) axis in humans given epidural triamcinolone injection (ETI) for low back pain. The evaluation of the blood adrenocorticotropic hormone (ACTH) and cortisol was undertaken to determine the extent of suppression of the HPA axis in patients given ETI. METHODS: Lumbar epidural triamcinolone injections were performed on the painful lumbar intervertebral space with patients in the lateral decubitus position. The injection consisted of 40 mg of triamcinolone acetonide diluted in 10 mL of 1% lidocaine. Patients remained in the lateral position for 10 min after the procedure. Basal blood sampling was performed at 30 min before ETI and tested blood sampling was obtained at 7 days, 10 days, and 14 days after ETI. RESULTS: The blood cortisol level was significantly decreased at 7 days and 10 days but at 14 days was not significantly decreased and the blood ACTH level was not significantly decreased at 7 days, 10 days, and 14 days. CONCLUSIONS: Above results demonstrate that blood ACTH and cortisol level normalize 7 days and 14 days, respectively, after epidural triamcinolone 40 mg injection.

10.A Case of Leptomeningeal Metastasis Presented with Bilateral Loss of Vision.

We describe an unusual case of leptomenigeal carcinomatosis presented with bilateral blindness. A 67-year old male had been suffering from progressive blindness for the past 2 months. Initially, the symptom developed in the right eye but quickly spread to the left eye becoming bilateral with complete vision loss in a matter of weeks. He did not show any other focal neurological signs and did not have a history of malignancy. An examination of the cerebrospinal fluid revealed metastatic adenocarcinoma. As this case indicated, the patients of leptomeningal metastasis may present with acute or subacute blindness, even in the absence of other focal neurological deficits.
