1.A Clinical Study of the Subacute Osteomylitis in Children

Subacute osteomylitis is far less common than acute osteomyelitis, characterized by insidious onset of the symptoms of mild local pain or discomfort without any acute systemic illness. Radiographic findings are not infrequently confused with benign or malignant bone tumors. From Jan, 1983 to Dec. 1991, we experienced twelve patients with subacute osteomylitis of long bones. Our clinical observations were as follows. 1. There were 11 boys and one girl with an average age of 9.7 years(range, 30months-16years). 2. The involved sites were proximal in 1, distal radius in 3, midshaft of femur in 3, distal femur in 2, proximal tibia in 1, and distal tibia in 2 cases. 3. All patients had insidious onset of mild to moderate pain. ESR was increased in 9 cases(75%) with a mean value of 44 mm/hr, but leukocytosis was not found. 4. According to the Green and Edwards' classification, there were type 1 in 1, type 2 in 3, type 3 in 3, and type 6 in 5 cases. 5. Eleven patients had operative treatment. The remaining one patient was treated by antibiotic treatment. 6. Primary treatment was successful in 11 patients who were followed for and average 9.5 months. One patients was lost to follow-up.

2.Four Cases of Acquired Perforating Disease in Patients with Chronic Renal Failure.

Acquired perforating disease(APD) is characterized by hyperkeratotic papules with transepidermal elimination of degenerated material and is associated with renal disease and/or diabetes. Particular attention has been directed to transepidermal elimination because the articles on perforating diseases among patients with chronic renal failure and/or diabetes have been increasingly reported. We describe four patients with chronic renal failure and/or diabetes whose skin biopsy specimens showed transepidermal elimination.

3.The Effect of Percutaneous Nephrolithotomy on Renal Function.

The effects on renal function following percutaneous nephrolithotomy(PNL) were investigated on 37 patients. The overall success rate was 76%. The success rate on renal stone(79% ) was better than on upper ureter stone(67% ). The complication rate was much lower on later cases according to the increasing experiences and improved technique. The renal function was evaluated on 15 cases using DMSA renal scan and separate creatinine clearance rate pre-and post-operatively. Fourteen cases out of fifteen cases showed no significant renal functional change statistically. However, only one case who had massive renal bleeding for eight days postoperatively showed about 18% reduction of renal function and which case was considered due to major renal vascular damage during PCN tract creation. Conclusively, PNL seemed to be safer and has lower morbidity rate than surgical removal of stone if carried out by experienced hands.

4.Diffuse Neonatal Hemangiomatosis Successfully Treated with High Dose Corticosteroid.

Diffuse neonatal hemangiomatosis is a fatal disorder characterired by multiple cutaneous and visceral hemangiomas. The organs most commonly affected are the gastrointestinal tract, brain, liver and lung. The complications are high output cardiac failure, gastrointestinal bleeding and hydrocephalus. We present a patient with diHuse neonatal hemangiomatosis associated with massive hepatic involvement and high output cardiac failure, which was successfully treated with high dose corticosteroid.

5.A Case of Benign Symmetric Lipomatosis.

Benign symmetric lipomatosis is characterised by diffuse symmetric deposits of adipose tissue, predominantly on the neck, shoulder, back and upper extremities. We report a case of benign symmetric lipomatosis in a 61-year-old-man, which was first noted one year previously. It began growing rapidly about 3 months ago and was associated with alcoholic liver disease. Histopathological findings showed that the reticular dermis had been replaced by normal uncapsulated mature fat cells that were slightly increased in number in the fibrous connective tissue. He was treated with theophylline 300mg/day for two months.

6.Chemical Peeling and Laser Resurfacing .

The chemical peeling is the process that involves the application of one or more exfoliating agents to the skin, resulting in the destruction of portions of the epidermis and/or dermis with subsequent regeneration. These techniques produce a controlled wound with instant vascular coagulation resulting in skin rejuvenenation with reduction or disappearance of actinic keratoses and changes, pigmentary dyschromias, rhytides, and selected superficial depressed scar. We herein introduce brief skin resurfacing techniques, indications, effectiveness, side effects of chemical peeling, dermabrasion, laser resurfacing.

7.A Case of Generalized Eruptive Histiocytoma in Childhood.

Generalized eruptive histiocytoma is a papular, nonlipidic, self-healing histiocytosis affecting mainly adults. Clinically the disease is characterized by an asymptomatic, widespread, frequently symmetrical eruption of numerous dark red or bluish red papules. In adults the lesions are symmetrically distributed and may involve mucous membranes, while in children the lesions are irregularly scattered over the entire body and mucous membranes spare. We herein report a case of generalized eruptive histiocytoma developed in a 3-year-old boy. He was seen with brownish, flat papules on the face, trunk and both proximal portions of extremities. Histological and immunohistochemical investigation revealed monomorphous infiltration of S-100, CD1a negative, lysozyme positive histiocytes in the upper dermis. Neither cells with foamy cytoplasm nor giant cells was found. He did not show any disease progression without treatment, so now in regular follow-up for exaggeration.

8.A Case of Maffucci's Syndrome.

Maffuccis syndrome was first reported by Maffucci, in 1881. This syndrome is manifested by multiple enchondromas and hemangiomas, characterized by non-hereditary mesenchymal neoplasia that has a propensity for malignant transformation. Involvement of long bones, particularly the bones of the hands and feet, is most common. The most important complication of this syndrome is the high frequency of chondr osarcomatous metaplasia. We report a case of Maffuccis syndrome in a 56-year-old woman with multiple hemangioma on both the hands and forearms, left foot and enchondromas of multiple bones. The disease started at the age of 8 and was associated with deformity, but malignant transformation was not found.

9.The Effect of Electrohydraulic Shock Wave on Renal Parenchyme of White Rat.

An experimental study was performed for the observation of the effect of the shock wave on the renal parenchyme of the white rat. Sixty Sprague-Dawley rats were subjected to this study. The experimental rats were divided into four groups by the intensity and amount of shock wave and also each group was subdivided into three small groups by the time of sacrifice. The following results of gross and microscopic examination were obtained. After expulsion of shock wave, various injuries such as subcapsular hematoma, parenchymal hematoma and laceration were seen and the degree of injuries was increased according to the intensity and amount of shock wave macroscopically. On the 1st day of sacrifice, the ischemic necrosis of parenchyme was observed at directly exposed site, the hemorrhage, congestion and arteriolar damages at transitional zone. On the 7th day, the renal capsular thickening by fibroblast proliferation and focal fibrosis was appeared at direct zone. At transitional zone, moderate lymphocyte infiltration was observed. On the 28th day, renal parenchyme was replaced by fibrosis at direct zone, severe lymphocyte infiltration and thyroidaization were observed at transitional zone. The degree of injuries was not proportional to the amount and intensity of shock wave microscopically and the reasons will be discussed later. Conclusively, the shock wave caused the partial functional loss and pathologic sequela of renal parenchyme.
