3.A Case of Spindle Cell Lipoma.

A 50-year-old man developed 3cm sized, well-marginated, subcutaneous mass on the right upper back. Histopathologic finding showed the lipomatous tissue was replaced by a mixture of uniform spindle cells and mature fat cells that were closely associated with a mucoid matrix and a varying number of collagen fibers. The patient had been treated with excision and showed no evidence of recurrence.

4.Surgical Treatment of Comminuted Supracondylar and Intercondylar Fractures of the Femur in Adults Using the Extensile Approach

From January 1985, to June 1986, 7 cases in which supracondylar and intercondylar fractures of the femur were treated at our orthopaedic department using the Extensile Approach for an anatomical reduction and firm internal fixstion, show successful results in follow up stuides. l. All 7 cases were type III fractures of Neers classification. 2. Among the 7 cases treated surgically using the Extensile Approach, 5 cases showed excellent and good results and the remaining 2 csses showed one fair and one failed results. 3. Probably due to the additional open proximal tibia and patella fracture and delayed surgery from trauma one case had a fair results. The case which failed wss due to ipsilsteral ligamentous injuries and a nerve injury in addition to the femur fracture and slso the impossibility of early knee joint exercise due to associated injuries. 4. None of the results of the 7 csses had any relationship with the patients age. 5. One case of postoperative skin necrosis developed around the tibial tuberosity and was treated successfully by a cross leg flap. 6. In comminuted supracondylar and intercondylar fractures of the femur, the Extensile Approach is thought to be an excellent surgical technique by means of a wide surgical field for an anatomical reduction and firm internal fixation with the repair of associated ligamentous injuries.

5.Congenital Pseuldarthrosis of the Tibia

Congenital pseudarthrosis of the tibia was accepted as one of the most difficult disease in respect of the frequency and treatment for the orthopaedists. Since April, 1984, we experienced 3 cases of congenital pseudarthrosis of the tibia, one 2 year-old boy involved left tibia only and the other 4 year-old female involved bilaterally, obtained solid bony union in all which were treated by massive surgical management that contained vascularized bone graft and electric stimulation. Several complications were faced such as refracture, valgus deformities of the ankle, skin irritation sign, angular deformities. But refracture was healed without delayed or nonunion by simple plaster cast immobilization. The vascularized bone graft snd electric stimulation is thought to be more improved methods for the treatment of congenital pseudarthrosis of the tibia. Althought solid bony union was obtained in congenital pseudarthrosis of the tibia, periodic follow-up and adequste management must be mandatory up to the end of the bony growth.

8.An Experimental Study on Sweat Secretion During 2 Hours of Heat Exposure.

Impaired sweating reportedly has been associated with a high incidence of heat pyrexia. An important cause is attributed to sweat gland fatigue following intense prolonged sweating. For further investigation, sweat gland fatigue was induced by heat exposure for 2 hours in a hot room (dry bulb 46c, wet bulb 37c) with 9 healthy subjects. Sweating rate of the right forearm increased progressively for the first 45 minutes with a maximum value of 40.0mg/4. 91 cm2/15 min followed by a slow decrease to 24.9 mg/4.91cm2/15 min at the end of 2 hours. Active sweat pores decreased in number following 2 hours of heat exposure but the differences were not significant statistically. In spite of the higher sweat rate on the back, the number of active sweat pores was smaller as compared to the right forearm. Na+ concentrations of the second hour sweat collections were higher than those of the first hour with difference of 14. 9 mEq/l on the right forearm and 12.3 mEq/1 on the back.

9.A Changes of Electrolytes and Biochemical Components during Hemodialysis.

To investigate the changes of electrolytes and bio-chemical components during hemodialysis in 15 patients(7 men and 8 women) with end-stage renal disease undergoing long-term hemodialysis were studied. The mean age of 15 patients undergoing long-term hemodialysis was 45 year(range ; 26-62). The mean duration of hemodialysis was 58 months(range: 19-143) at time of the study. Hemodialysis was performed with acetate dialy- sate having 37mEq/1 acetate and 8.57g/100ml glu-cose(group A), with bicarbonate dialysate having 8 mEq/1 acetate and 30mEq/1 bicarbonate without glu-cose(group B) for 4-hour. Each blood sample was drawn for blood gas analysis, blood glucose, blood urea nitrogen and electrolytes from the arterial line at 0, 1, 2, 3, and 4-four during hemodialysis. In group A, the body temperature measured at 1 hour increased significantly compared with that of predialysis. The blood glucose level increased pro-gressively during dialysis. In group B, the body temperature measured at 1 hour also increased sig- nificantly compared with that of predialysis. However, the blood glucose level increased progressively during dialysis as removing urea in blood during dialysis. These results suggested that blood urea nitrogen may be a cause of hypothermia. Mean corpuscular volume(MCV) increased significantly at 2-hour in group A and returned to baseline values at 4-hour. In group B, the mean level of glucose value in dialysate of outlet line of dialyzer increased to 38.3mg/dl from zero in inlet line, and PaC&z decreased significantly because of diffusion of COz across the dialyzer. In Group A, PaOz decreased significantly at 1-hour and returned to baseline values at 4-hour, whereas HCC4 was increased significantly at 4-hour. The intracellular potassium content may decreased in long-term hemodialysis patients cornpared with normal control. In Group A, hernoglobin diphosphoglycerate(2, 3-DPG) concentraion increased from 10.9 pmoVg before dialysis to 19.9pmol/g after dialysis. Despite substantial anemia, hemoglobin 2, 3-DPG prior to dialysis was significantly lower than the value obtained in the normal control(17.5+4.3 p moVg). In group A, the blood lactate level decreased significantly at 1-hour and returned to baseline values at 4-hour. Our data suggested that body temperature might be increased due to removal of urea nitrogen, and blood glucose may be decreased because of the dif- fusion across dialyzer. In addition, this study showed that intracellular potassium content was lowered, the MCV of RBC was increased during hemodialysis. Transient decrement of PaCOz during early phase of dialysis and decreased hemoglobin 2, 3-DPG of despite anemia before dialysis improved with the increment of blood pH and HCC4 at the end of dialysis.

10.A Statistical Study of Dermatoses in Wonju Area During the Recent 5 Years ( 1980 ~ 1984 ).

In this report, 20,536 new outpatients who have visited the Department of Dermatology of Wonju Christian Hospital from Jan. 1st, 1980 to Dec. 31st, 1984 were analized. The results suggest that the age, sex, monthly and yearly distribution patterns of dermatoses in Wonju area are similar to those of other areas in Korea except that infective dermtoses(40.3%), especially scabies(9.8%) and herpes zoster(1.6%) are more common.
