1.A Case of Postcaval Ureter.

Post-caval or retrocaval ureter is a congenital anomaly of the vascular system. This anomaly almost. always involve the rt. ureter and occurs more frequently in male We have reported a 36 years male of rt. postcaval ureter that was resulted marked hydronephrosis due to compression of rt. ureter by vena cava. Three months after the operation, excretory urogram showed previous caliectasis and pyelectasis were moderately reduced and previous dislocated ureteral course was corrected.

2.Yellow Nail Syndrome.

Yellow nail syndrome is characterized by yellow discoloration of nails, chrnnic lymphedema and respiratory dieorders such as chronic bronehitis, bronchiectasis, pleural effusion and chronic sinusitis. Other changes of nail in this syndrome are the exaggerated lateral curvature and disappearance of lunula and cuticle. These nail changes are caused by greatly slow nail growth than in normal due to the defective lymphatic drainage. We report a case of yellow nail syndrome in a 55 year-old woman who has pulmanary edema, congestive heart failure for 3 years, and yellow naiIs of all fingers and toes for 1 year.

3.Clinical Observation of the Reflex Ileus due to Urinary Stone.

Reflex ileus in acute abdominal disease were produced frequently due to cholecystitis, cholelithiasis, abscess in pelvic, appendiceal and diaphragmatic etc. But we found that a functional type of paralytic ileus due to ureteral stonetr renal stone was commonly seen on the plain roentgenogram. In these functional type of reflex ileus due to urinary stone, distension of the small intestine withgas may interfere with an accurate detection of the stone. It may lead to confusion in detecting ureteral stone and obstructive ileus. In this report, we analyzed how many cases would show a functional type of reflex ileus in urinary stone cases. 1. In 200 cases of urinary stone, reflex ileus with urinary stone were produced in 49 cases(21. 5%) 2. Many cases of ureteral stone were found on lower third of the ureter. (99 cases) 28 reflex ileus cases in 99 cases were produced (29.2%) 3. In reflex ileus due to urinary stone, we classified them by sex, age, location and stone size. The stone was often masked by nausea, vomiting, abdominal distention and reflex ileus on plain roentgenogram. So it is necessary to perform a complete urological study to detect urinary stone.

5.A Case of Pemphigus Vegetans of Neumann.

We experienced a 40-year-old female patient who had vegetating plaques, erosions and a few vesicles on the external genitalia and the oral mucasa, assoeiated with pulmonary tuberculosis. On the skin biopsy specimen of the vegetating plaque, it showed acanthoais, papillomatosis, downward proliferation of the epidermis and eosinophilic microabacess in the epidermis. Direct immunofluorescence of the perilesional skin revealed deposition of IgG, Cq, Cs and C on the intercellular substance of the epidermis. Indirect immunofluorescence disclosed IgCi auto antibody(1: 320) positive to the inter ellular substance of the normal human back skin. The skin lesion had impraved with dapsone therapy.

6.A Case of Generalized Dermal Melanocytosis.

Dermal melanocytosis includes Mongolian spot, nevi of Ota and Ito, dermal melanocytic harnartoma, and blue nevus. Nevi of Ota may be respectively associated with Mongolian spot, blue nevi and unilateral or bilateral nevi of Ito. We present a case, a 24 year-old female, who had bilaterally brownish or slatebluish pigmented patches on the face, sclera, neck, anterior chest, deltoid, supraclavicular, scapular, back and sacrococcygeal area. And also she had a bluish, bean-sized nodule on her left mid-back. It is likely that this case may be considered as an extremely extensive case of pig:m.entary anomaly, associating aberrant and persistent mongolian spots, extensive nevus of Ito, bilateral nevus of Ota and blue nevus. To our knowledges it seems that such case has not previously been reported in Korean literature.

7.A Case of Incidental Epidermolytic Hyperkeratosis Occurring Normal Looking Skin Adjacent to Folliculitic Papules: In Veterans Who Participated in Vietnam War.

On histological examination, an epidermolytic hyperkeratosis was observed adjacent to follicular papules on the back of a 53-year-old man. It has been reported that incidental epidermolytic hyperkeratosis occur either within various lesion (epidermal neoplasm, melanocytic neoplasm, scars, and inflammatory conditions) or in the normal skin adjacent to the lesion. This patient participated in the Vietnam War for 2 years, and had had contact with defoliants. He was treated for multiple peripheral neuropathies and cerebral infarcts. In keratinocytes, 2,3,7,8-Tetrachlorodibenzo-p-dioxin (TCDD; Agent Orange) contained in defoliating agents is associated with altered patterns of keratinocyte differentiation. So, as a cause of incidental epidermolytic hyperkeratosis, defoliant contact could be suspected.

8.A Case of Unusual Sparganosis.

We report herein a human sparganosis that presented confusing features in aspect of its correct identification, The patient was 35-year-old man who had a past history of eating fried frogs since 3 years prior to his first admission in 198I. Three mature Sparganum mansoni and 69 encapsulated spargana pieces were excised from his left inguinal area, left thigh and scrotum by three consecutive operations in 19 months. The encapsulated spargana were irregular in shape and size and mummified. They were identified histopathologically as degenerated spargana by no sexual organs and many calcareous bodies in the parenchyma. Differentiation of our case from proliferating sparganosis caused by Spargarrum proliferum was discussed.

9.In Vivo Delayed Hypersensitivity to the Recall Antigens in Different Dermatoses.

Authors have performed patch test to evaluate the differences between the allergic contact dermatitis to topical medicaments and to the other substances including cosmetics. One hundred eighteen patients were evaluated according to age, sex, and causative agents of the disease. Furthermore, these patients were analysed as to the causative allergic contact factors by clinical inspection and history. The most common causative allergic contact factors by clinical, history and patch test were topical medicarnents(45.8%), cosmetics(24. 6%), rubbery(10.2%), and metals(8.59%.). The common etiogloic medicaments of contact dermatitis were Madecassol, Betadin, Tiger balm, Canesten, Mercurochrom, and Tego solution. The causative agents were confirmed by the patch test with Hollister-Stiers standard allergens in 57 patients from Jan. 1983 to Dec. 1984, 1 hirty eight among 57 patients were positive to patch test, Back ground possible explanation of the patch test results were discussed and a comparison was made between this report and others.
