1.Comparison of the Stability Between Three-piece and Single-piece Aspheric Intraocular Lenses.

PURPOSE: The goal of the present study was to compare the stability of the 3-piece (Tecnis ZA9003) and single-piece (AcrySof IQ) aspheric intraocular lenses (IOL) by testing decentration, tilt, anterior chamber depth, and refraction. METHODS: The subjects of this study consisted of 101 eyes who had undergone cataract surgeries with a 3-piece aspheric IOL (54 eyes) and with a single-piece aspheric IOL (47 eyes). The decentration, tilt, anterior chamber depth, and refraction were measured on postoperative day 1, 1 month, and 2 months, using an anterior eye segment analysis system (EAS-1000, Nidek, Japan). RESULTS: There was a statistically significant difference in the decentration on the postoperative day 1 (p = 0.04). However, there was no statistically significant difference on postoperative 1 month (p = 0.15) and 2 months (p = 0.13). There was no statistically significant difference in the tilt on postoperative day 1, 1 month, and 2 months. There was no statistically significant difference in the anterior chamber depth on postoperative day 1, 1 month, and 2 months. There was a statistically significant difference in the refraction on postoperative day 1 (p = 0.03). However, there was no statistically significant difference on postoperative 1 month (p = 0.07) and 2 months (p = 0.07). CONCLUSIONS: There was no statistically significant difference in the decentration, anterior chamber depth, and refraction between the 3-piece and single-piece aspheric IOL. Therefore, there is no difference between the 3-piece and single-piece aspheric IOL in the capsular bag stability until 2 months postoperatively.

2.Two Cases of Primary Carcinoma of Female Urethra.

Primary carcinoma of the female urethra is an uncommon neoplasm that accounts for less than 0.02% of all malignant disease occurring in women. The rarity of the disease as well as varieties of treatment modalities has prevented unanimity of opinion as to the selection of proper treatment in individual cases. Herein we report two cases of female urethral primary carcinoma with brief review of the literatures.

3.Transcriptional Regulation of H2B Histone Gene Expression by Nocodazole in HL-60 Cells.

PURPOSE: Nocodazole, a microtubule disrupting reagent, is known to arrest cells in the M phase, To gain insight on the regulatory mechanism of H2B histone gene expression by nocodazole in HL-60 cell, the binding pattern of nuclear proteins to cis element in the human H2B histone gene promoter has been investigated with DNase I footprinting and DNA mobility shift assay. MATERIALS AND METHODS: Northern blot hybridization was performed by the method of Virca et al. A Hinc II-Sac I fragment of pSPH28 was used as probe for Northern blot analysis of H2B histone mRNA. DNase I footprinting and DNA mobility shift assay were performed by the method of Lim et al. End labeled DNA oligomer (upper strand, 5'-CTTCACCTTATTTGCATAA GCGATTC-3') for octamer binding activity was mixed with nuclear extracts in a 20 ul reaction volume containing 60 mM KC1, 12 mM HEPES, pH 7.9, 5 mM MgCl2, 0.2 mM EDTA, 0.2 mM DTT, 12% glycerol, and 2 ug of poly [dI-dC]. RESULTS: The level of H2B histone mRNA rapidly was reduced at 24 hours in nocodazole-treated HL-60 cells and the mRNA was repressed in proportion to the concentration of nocodazole. Nocodazole-dependent repression of H2B histone gene was restored by replacement with nocodazole-free media. In DNase I footprinting analysis, one nuclear factor bound at 42 bp site (octamer motif) in the absence of nocodazole. In the presence of nocodazole, the binding of nuclear factor on octamer motif partially vanished. In DNA mobility shift assay, one DNA-protein complex (Octl) was formed when octamer motif was incubated with nuclear extract of HL-60 cell. After nocodazole treatment, Octl binding activity was reduced by time dependent manner. CONCLUSION: These results suggest that nocodazole-dependent repression of H2B histone gene is correlated with reduction of Octl binding activity in HL-60 cell.

4.Trace of Occupational History on Lung Cancer Patients.

The relationship between occupation and risk of lung cancer was analyzed in hospital based case-control study conducted in Taegu on 168 patients with histologicatly confirmed lung cancer and age group (30-39, 40-49, etc) and sex frequency-matched 168 controls admitted into hospitals for other internal medicine problems. A self administered questionnaire was used to obtain occupation, resident environment, smoking histories and specific agent exposure histoy. The conditional logistic regression was used to estimate odds ratio for age group and sex frequency matched, after controlling for smoking status (non smoker, exsmoker, current smoker) Significant association was observed for farmer[odds ratio(OR), 1.84 , 95% confidence interval(CI), 1.16-2.94], briquette stove users(OR, 3.0 , 95% CI, 1.06-8.25), agricultural chemicals users[0R, 2.14 , 95% CI, 1.35-3.37], and welding/fume exposures[0R, 10.56 ;95% CI, 1.07~103.90]. Although there were elevated risks associated with production, home industry, transportation/storage/communication, lodgement/food worker and construction, none were statistically significant.The effect of smoking was showed significantly increased risk for exsmoker [0R, 4.50 ,95% CI, 1.99-10.16] and current smoker [OR, 3.98 , 95% Cl, 1.88-8.45] As results, although smoking is important risk of lung cancer, occupation and resident environment are showing association of lung cancer. So further evaluation is necessary for occupation risk and take preventive measures.

5.Endoscopic Sentinel Lymph Node Biopsy in Breast Cancer Surgery: Feasibility and Accuracy of the Combined Radioisotope and Blue Dye.

PURPOSE: Since its introduction in the mid-1990s, sentinel lymph node biopsy has been rapidly and widely adopted for the axillary staging of clinically node-negative breast cancer patients. However, there is some controversy in the clinical application because of its various identification rates and its false negative rates. The objective of this study was to assess the usefulness of endoscopic sentinel lymph node biopsy (ESNB) and to compare the value of two methods for identification of ESNB using blue dye only or a combination of blue dye and radioisotope. METHODS: This study was carried out in 137 breast cancer patients (bilateral breast cancer, 3 cases) who underwent ESBN, at the Department of Surgery in Soonchunhyang University from May of 2007 to August of 2008. The technique involved the injection of 5 mL of 0.5% indigocarmine or Tc-99m tin colloid into subareolar plexus. The Visiport docked with a telescope was inserted through a low transverse axillary incison (1.0 cm in size) lateral to the pectoralis major. During the dissection, we identified sentinel nodes by following blue-stained lymphatic duct directly into blue-stained lymph nodes. We compared the value of two methods for identification of ESNB using blue dye only or a combination of blue dye and radioactive tracer. RESULTS: The mean number of sentinel nodes was 1.27 (range, 1-4). The identification rate and false negative rate of the sentinel node were 94.3% (132/140) and 6.9% (3/43), respectively. We compared ESNB with using blue dye only (n=77) vs. a combination of blue dye and radioactive tracer (n=63). Sentinel lymph node identification rate were 90.9% (70/77) vs. 98.4% (62/63) (p=0.043). CONCLUSION: The endoscopic technique of sentinel node biopsy can keep better operative visual fields and is less invasive. The combination of blue dye and radioactive tracer was superior to blue dye only for identification rates.

7.Clinical Characteristics of Young Patients with Lung Cancer.

BACKGROUND: It has been reported that younger patients with lung cancer have characteristic features that differ from those in older patients. The prognosis for young patients with this disease is controversial. This study aimed to determine the clinicopathological characteristics, the survival rate, and the risk factors associated with the overall survival rate in younger patients with lung cancer. METHODS: The records of 120 young(age≤40) patients with histologically confirmed lug cancer in the Korea Cancer Center Hospital(KCCH) between Jan. 1992 to 1998, 120 older(age>40) patients were randomly selected as the controls. RESULTS: More female patients(45.0% vs. 20.0%, p<0.001) and more adenocarcinoma cases(64.2% vs. 38.3%, p<0.001) were found in the younger group, when compared to the older patients. In NSCLC, advanced disease(stage III B and IV) was more common in the younger patients(90.2%) than in the older patients(62.7%) (p<0.001). The Median survival was 8.6 months in the younger patients and 12.2 months in the older(p=0.003). In a multivariate analysis, only the advanced-stage was an independent negative prognostic factor. CONCLUSION: Lung cancer in the younger age group presents with a more advanced stage resulting in a poor survival rate, which suggests that lung cancer in this population is more aggressive than in older patients.

8.Prognostic Value of Restaging F-18 Fluorodeoxyglucose Positron Emission Tomography/Computed Tomography to Predict 3-Year Post-Recurrence Survival in Patients with Recurrent Gastric Cancer after Curative Resection

Objective:
The aim of this study was to investigate the prognostic value of the maximum standardized uptake value (SUVmax) measured while restaging with F-18 fluorodeoxyglucose (18F-FDG) positron emission tomography/computed tomography (PET/ CT) to predict the 3-year post-recurrence survival (PRS) in patients with recurrent gastric cancer after curative surgical resection.
Materials and Methods:
In total, 47 patients with recurrent gastric cancer after curative resection who underwent restaging with 18F-FDG PET/CT were included. For the semiquantitative analysis, SUVmax was measured over the visually discernable 18F-FDG-avid recurrent lesions. Cox proportional-hazards regression models were used to predict the 3-year PRS. Differences in 3-year PRS were assessed with the Kaplan–Meier analysis.
Results:
Thirty-nine of the 47 patients (83%) expired within 3 years after recurrence in the median follow-up period of 30.3 months. In the multivariate analysis, SUVmax (p = 0.012), weight loss (p = 0.025), and neutrophil count (p = 0.006) were significant prognostic factors for 3-year PRS. The Kaplan–Meier curves demonstrated significantly poor 3-year PRS in patients with SUVmax > 5.1 than in those with SUVmax ≤ 5.1 (3-year PRS rate, 3.5% vs. 38.9%, p < 0.001).
Conclusion
High SUVmax on restaging with 18F-FDG PET/CT is a poor prognostic factor for 3-year PRS. It may strengthen the role of 18F-FDG PET/CT in further stratifying the prognosis of recurrent gastric cancer.
9.Development of a monoclonal antibody against deoxynivalenol for magnetic nanoparticle-based extraction and an enzyme-linked immunosorbent assay.

Monoclonal antibody (mAb, NVRQS-DON) against deoxynivalenol (DON) was prepared. DON-Ag coated enzyme linked immunosorbent assay (ELISA) and DON-Ab coated ELISA were prepared by coating the DON-BSA and DON mAb. Quantitative DON calculation ranged from 50 to 4,000 ng/mL for DON-Ab coated ELISA and from 25 to 500 ng/mL for DON-Ag coated ELISA. 50% of inhibitory concentration values of DON, HT-2, 15-acetyl-DON, and nivalenol were 23.44, 22,545, 5,518 and 5,976 ng/mL based on the DON-Ab coated ELISA. Cross-reactivity levels of the mAb to HT-2, 15-acetyl-DON, and nivalenol were 0.1, 0.42, and 0.40%. The intra- and interassay precision coefficient variation (CV) were both <10%. In the mAb-coated ELISA, mean DON recovery rates in animal feed (0 to 1,000 microg/kg) ranged from 68.34 to 95.49% (CV; 4.10 to 13.38%). DON in a buffer solution (250, 500 and 1,000 ng/mL) was isolated using 300 microg of NVRQS-DON and 3 mg of magnetic nanoparticles (MNPs). The mean recovery rates of DON using this mAb-MNP system were 75.2, 96.9, and 88.1% in a buffer solution spiked with DON (250, 500, and 1,000 ng/mL). Conclusively we developed competitive ELISAs for detecting DON in animal feed and created a new tool for DON extraction using mAb-coupled MNPs.

10.A Case of Recurrent Fecal Impaction.

Fecal impaction is defined as a large compacted mass of feces in the rectum or colon that cannot be passed by the patient. Fecal impaction may be caused by predisposing conditions such as mechenical impediments, neurologic diseases, psychiatric disorders, endocrine diseases, and some medications. And ninety percent of fecal impaction occur in rectum and sigmoid flexure. Recently, we experienced a case of fecal impaction in young female who had no predisposing conditions. A 30-year-old woman was adimtted with abdominal distension and constipation. She was diagnosed fecal impaction in splenic flexure and transverse colostomy was taken. After colostomy was repaired, recurrent fecal impaction was occurred and second transverse colostomy was done. After all, right hemicolectomy was done due to recurrent fecal impaction. She have one or two bowel movements a day after surgery.
