3.Occurrence of Anthracnose on English Ivy Caused by Colletotrichum trichellum in Korea.

Anthracnose symptoms severely occurred up to 50% on leaves of English ivy growing in greenhouses in Cheongwon area of Korea during disease survey in April, 2000. The symptoms developed as concentric spots with dot-like acervuli on leaves of the plant. A total of 24 isolates of Colletotrichum sp. were obtained from the spotted lesions and identified as Colletotrichum trichellum based on the morphological and cultural characteristics. Leaf spots similar to the original anthracnose symptoms were induced on the host leaves by artificial inoculation with the isolates of the fungus. This is the first record of C. trichellum causing anthracnose of English ivy in Korea.

5.Etiological Classification and Epidemiology of Dementia.

Etiological classification of a syndrome is most warranted. However, it is very difficult in the case of dementia because many research activities are still being made about the cause of degenerative dementias. In the current paper, clinical aspects and pathological findings were also con-sidered in the etiological classification. Prevalence and annual incidence of dementia are 5-15% and 1.5-2.0% in the elderly over 65, respectively. Old age, female sex, genetic background, lower education, ethnicity or geographical difference, and head trauma seem to be risk factors for developing dementia. Urinary and fecal incontinece, extrapyramidal symptoms, primitive reflexes, psychotic symptoms, and cognitive decline seem to adversely affect on the mortality, while social class, age at onset, depressive symptoms, aggressive behavior, wandering, increased appetite, increased sexual behaviors, and early diagnosis do not.

6.A Comparative Study of Mental Health between the Laid-Off and the Employed Worker in the Similar Industry.

OBJECTIVES: This study was conducted to examine the mental health status of the laid off by comparing the unemployed with the employed. METHODS: A survey questionnaire was used to investigate the mental health status of the laidoff in comparison with that of the employed. The study sample consisted of 247 laid-off people formerly employed in A auto factory and 167 employees working in B auto factory. RESULTS: The proportion exercising regularly was 20.2% for the laid-off and 34.8% for the employed and the proportion having regular meals was 10.9% and 50.6%, respectively. In addition, the frequency and amount of alcohol consumption and smoking of the laid-off were higher than those of the employed. Among the laid-off, 69.4% responded that they spent less time in talking to their spouse, and 65.4% also reported reduced sexual activities with their spouse. Because of unemployment, 41.8% of the laid-off experienced conflicts with their children, and 53.5% also perceived that they were stigmatized and treated badly by their neighbors, friends and relatives. Mean IFR in the laid-off was 22.9, which was much higher than the 15.3 recorded in the employed. The mean PWI-SF was 24.2 for the laid-off and 13.0 for the employed, which indicates the lower psychological health status of the laid-off than that of the employed. Age, annual salary, family relationship, regular exercise, regular diets, possibility to return to work and perception of the future were related to mental health status for the laid-off. Stepwise multiple regression analysis showed that the most important factor influencing mental health status for the laid-off was family relationships. CONCLUSIONS: According to the above results, the mental health of the laid-off is worse than that of the employed. Based on this study, a follow-up study focusing on the mental health status of the reemployed to the same factory, is needed to understand the direct mental health effect on the laid-off.

7.Multicentric Extrahepatic Tumor Involving the Gallbladder and Common Bile Duct A Case Report of Longterm Survival.

A 60-year-old woman has experienced of right upper quadrant abdominal discomfort. This partient took cholecytectomy due to gall stone 15 years ago. At that time microscopic finding the neck of gallbladder showed well differentiated papillary adenocarcinoma confined to superficial part of musele layer. 3 years before admission she took T-tube drainage due to obstructive jaundice. The microscopic finding af distal common bile duct showed well differentiated adenocarcinoma. Recently she complained of pain and discomfort on T-tube site and so we perforrned endoscopic retrograde biliary drainage after removal of T-tube. The computed tomogram showed no mass in liver or lymph node enlargement. 4 months after procedure of ERBD; we changed to the new catheter for the prevention of cloggiag of prosthesis. We report here a case of longterm survival over 15 years after the diagnosis of cancer of extrahepatic bile duct.

8.A Case or Nutcracker Syndrome Surgically Corrected by Extraperitoneal Flank Approach.

Nutcracker syndrome is a rare disease caused by the compression of the renal vein between the aorta and superior mesenteric artery. It is known to cause of venous hypertension, peripelvic and periureteral varicosities. Clinical manifestations include varicocele, hematuria, and flank or abdominal pain. Surgical correction is needed in nutcracker syndrome cases with severe or recurrent hematuria and abdominal or flank pain. Reanastomosis of the renal vein at 5cm below the original insertion site at the inferior vena cava is the one of the most common corrective methods and the transabdominal approach is usually employed. This disease usually appears in healthy young people and in order to decrease postoperative complications such as intestinal obstruction and bowel adhesion by avoiding unnecessary intraperitoneal manipulation, the authors used the extraperitoneal approach through a flank incision. We report because, in selected cases, we consider that this extraperitoneal approach is one of the alternative methods for the usual transperitoneal approach.

10.Clinical Observation on Benign Prostatic Hypertrophy.

A clinical study was made on 112 cases of benign prostatic hypertrophy, admitted to the Department of Urology, East City Hospital, Seoul, Korea during the period from January 1979 to December 1982.The results were summarized as follows.1. Benign prostatic hypertrophy were 112 cases of all 761 cases hospitalized, admission rate was 14.7%. 2. Majority of cases was found in 6th and 7th decades with mean age of 72,4 years old. 3. In composition of sexual life during 2th to 5th decades, long-term obstinence or irregular sexual life were 57 cases, long-term bachelohood were 38 cases, normal sexual life were 17 cases. 4. In most cases, the duration of the obstinence or the irregular sexual life was 11 years to 25 years. 5. The presenting symptoms were frequency in 94 cases, acute urinary retention in 59 cases and dysuria in 37 cases etc. 6. The blood chemistry showed elevated BUN in 83 cases and elevated creatinine in 61 cases. 7. The endoscopic examination was performed in 98 cases which revealed trabeculation in 87 cases, inflammation in 61 cases, kissing of lateral lobes in 46 cases and diverticular formation in 15 cases. 8. Hypertension in 12 cases and pulmonary tuberculosis in 12 cases were frequently associated with benign prostatic hypertrophy. 9. Methods of operative treatment were suprapubic prostatectomy in 24 cases, transurethral resection 21 cases and suprapubic cystostomy in 33 cases. 10.Postoperative complications were temporary incontinence in 33 cases, delayed bleeding in 8 cases, acute epididymitis in 3 cases and wound infection in 2 cases.
