Acute Febrile Neetrophilic Dermatosis(AFND, Sweet's syndrome) ia characterixed by painful erythematous plaquea aaaociatcd with arthralgia, fever and leukocytoeis, We preagented a case of acute febrice neutrophilie denmatoeis in acutemyelogenous leukemia which occurred in a 55-year-old man. After 10 days of treatment with prednisolone, the skin lesion disapceared, but he die of pneumonia in spite of anticancer therapy.

2.Clinical Observations on Acute Pyogenic Osteomyelitis and Arthritis in Infancy

Clinical observations of 11 cases of acute pyogenic osteomyelitls and 14 cases of acute pyogenic arthritis in infancy during past 11 years were carried out and following results obtained. 1. The highest age incidence was less than 3 months after birth (70.6%). 2. Average duration of symptoms was 7.8 days. 3. The most important underlying cause was multiple femorai vessel puncture (23.5%). 4. The chief symptom or sign on admission was pseudoparalysis or limitation of motion (82.4%), and local swelling (82.4%). But high fever on admission was present in only 29.4% of cases. 5. The femur was the most common site in acute pyogenic osteomyelitis (36.4%), and the hip joint in acute pyogenic arthritis (42.9%). 6. Leucocyte count was Increased more than 20,000/mm in 60.0% of the cases. Erythrocyte sedimentation rate was increased more than 21 mm/hr in 72.0% of cases. The positive result on bacteriologic culture was 76.0%. Coagulase-positive Staphylococcus aureus was cultured in 4 cases of acute pyogenic osteomyelitis and 5 cases of acute pyogenic arthritls. a-hemolytic Streptococcus was cultured in 2 cases of acute pyogenic osteomyelitls and 1 case of acute pyogenic arthritis. Hemophilus influenzae was cultured in 1 case of acute pyogenic osteomyelitls and 2 cases of acute pyogenic arthritis. 7. 45.5% of acute pyogenic osteomyelitis was combined with acute pyogenic arthritis. 8. The longer the duration of symptoms was, the longer the abnormal hematologic picture lasted. Erythrocyte sedimentation rate was the most accurate index of activity in acute infection of bone and joint. 9. Shortening or deformity due to epiphyseal plate damage occured in 1 case of acute pyogenlc osteo-myelitis, and 3 cases of acute pyogenic arthritis. Those complications could be minimized wlth early diagnosis and treatment.

4.Cervical Spinal Epidural Abscess due to Salmonella Group D.

We report a case of cervical spinal epidural abcess due to Salmonella group D. A 58-year-old man complained of progressive neck pain and right arm weakness with fever. Clinical and radiographic findings including cervical magnetic resonance(MR) image showed paraspinal soft tissue inflammation of neck and abcesses in prevertebral and epidural space at C3-5 level. The patient underwent a C4 corpectomy and adjacent diskectomy and continous irrigation suction drainage. A MR image taken at second postoperative day revealed diffuse cord swelling from C2 to T1 suggesting edema and/or myelitis and then laminectomy from C3 to C6 was performed immediately to decompress the spinal cord. The patient recovered excellently and the abscess culture revealed Salmonella group D.

5.Rapid Identification of Mycoplasma Pneumoniae, Mycobacterium Tuberculosis and Staphylococcus Aureus in Pleural Fluid by PCR.

PURPOSE: Pleural effusions may develop during the course of bacterial pneumonia. The aim of this study was to evaluate the significance of the polymerase chain reaction (PCR) method for detection of Mycoplasma pneumoniae, Mycobaterium tuberculosis and Staphylococcus aureus from pleural fluid. METHODS: Total 12 samples were obtained from pleural fluid; 2 samples from children with Mycoplasma pneumonia, 5 samples from adults with tuberculous pleurisy, and 5 samples from sterile pleural fluid seeded artificially with staphylococcus aureus. The primers used for our PCR were prepared to amplify M. pneumonia-specific MP5 gene, M. tuberculosis-specific IS6110 gene, and S. aurus-specific femA and mecA gene. The amplified PCR products were detected by ethidium bromide-stained agarose gel electrophoresis. RESULTS: A total of 12 pleural fluid samples were tested by nested PCR using the specific primer set. We could amplify MP5 gene in 2 samples, IS6110 gene in 5 samples, mecA gene in 3 samples, and femA gene in 5 samples. These PCR data were correlated with serolological data, microbiological data and methicillin-sensitivity test result. There were no false-positive results due to cross-contaminating DNA between these 3 organisms. CONCLUSIONS: We conclude that enzymatic amplification of specific gene from pleural fluid might be useful to diagnose the infectious pleural effusion by Mycoplasma pneumoniae, Mycobacterum tuberculosis or Staphylococcus aureus.

6.Anterolateral Intrumentation and Spinal Stabilzation of Thoracolumbar Burst Fracture.

Between March 1994 and March 1995, 10 patients with thoracolumbar burst fractures underwent a one-stage operation consisting of anterior decompression, reduction, bony fusion with iliac bone and stabilization with Kaneda device. The mean follow-up was 6.4 Months. Most patients with incomplete neurologic lesions showed postoperative improvement and were upgraded one or two steps in the Frankel scale. No patient showed neurological deterioration after surgery. Loss of reduction was 5.5 degree during follow-up period. Anterior decompression and strut fusion was used to effectively recover the neurological deficit and reduce the pain in a thoraco-lumbar burst fracture.

7.An approach to the treatment of the hypoglycemia dropped after meal.

BACKGROUND: Many patients with type I Diabetes Melitus treated by insulin therapy were observed their blood sugar dropped even after meal. The ecology and treatment for this phenomenon was unknown. A clinical research making an approach to treat this phenomenon is inevitable. METHODS: 9 patients whose blood sugar dropped right after meal (from 15 minutes to 1 hour) were selected among 58 IDDM(Insulin Dependant Diabetes Mellitus) patients admitted in Daejon Sungsim Hospital from March 1999 to Feb. 2000. The subjects were consist of 6 female and 3male, average age was 57.2. Blood samples were taken from their capillaries and measured by Super Glucocard II glucometer. Insulin was injected 30 minutes before breakfast and 50% glucose 30 ml was taken orally 15 minutes after the insulin injection. Breakfast was taken 15 minutes after the glucose taken. Bolld sugar was measured 15, 20, 25, 30, 35, 40, 45, 50, 55, 60, 120 minutes and 2 hours after a breakfast. RESULTS: Though average blood sugar dropped after breakfast was 44.5 mg/dl, average blood sugar after taken 50% glucose 30 ml was 114.1 mg/dl. 2 patients had good effect among 3 male patients and every 6 female patients had good effect. CONCLUSION: Taking 50% glucose 30 ml was in oral 15 minutes after insulin injection improved the IDDM patient's blood sugar drop right after meal.

8.Clinical Analysis of Stereotactic Thalamotomy for Medically Intractable Essential Tremor.

Stereotactic ventrolateral thalamotomy has been successful in treating a wide spectrum of involuntary movement disorders. But very little has been reported concerning their application in essential tremor. Twenty three patients with medically refractory disabling essential tremor underwent ventralis(VL) thalamotomies. The procedure was unilateral in twenty three cases. Contralateral tremor remained abscent or markedly reduced in 22 patients at the time of the most recent follow-up examinations at a mean of 2.9 months after surgery. Disability was determined by a modified form of an estabilished rating scale for termor reported by Fahn and was reduced from a mean score of 11 to 3(p<0.0001) after the operation. Persistent surgical morbidity was limited to three patients with mild dysarthria and two with a mild cognitive impairment. There were no surgically related deaths. It is concluded that stereotactic VL thalamotomy is a treatment option for medically intractable disabling essential tremor.

To improve the situation of the department of psychiatry in university hospitals, policy development, expansion of psychiatric fields, strengthening of education and research, and support from government may be necessary.

10.Spinal Intradural Ventral Arteriovenous Fistula Mimicking an Intramedullary Ependymoma: A Case Report.

We present a rare case of an intradural ventral arteriovenous fistula (AVF) mimicking an intramedullary ependymoma. A 46-year-old woman presented with sudden onset of right leg weakness, which she had been experiencing for two weeks. Whole-spine magnetic resonance imaging (MRI) scan revealed a 0.5-cm-sized intramedullary lesion of high signal with a dark signal rim on a T2-weighted image at the T9 level. The T1-weighted MRI after contrast enhancement revealed a nodular and rim-like enhancement. However, a signal void, likely to be seen in the case of an engorged vein, was not obvious, thus giving the impression of an intramedullary ependymoma. Surgery was planned, but was aborted due to the observation of a large engorged vein as soon as the dura was opened.
