3.A Case of Platelet-rich Plasma (PRP) Treatment for Herpes Zoster Ophthalmicus-like Skin Necrosis after Filler Injection.

Dermal fillers are increasingly being used for facial soft tissue augmentation. In the appropriate patient and with appropriate training, dermal fillers can temporally eliminate facial wrinkles and depressed scars. Although complications are rare, they can occur with any dermal filler. Especially, injection necrosis is a rare, but clinically important potential complication that is caused by interruption of the vascular supply to the area by compression, injury and/or obstruction of the vessel. Injection necrosis can appear to be pyoderma or a local wound infection, and so physicians should pay attention to make an accurate diagnosis. We report here on a case of a 47-year-old female who experienced sudden onset local necrosis of the glabella area after filler injection, and this was followed by a herpes zoster ophthalmicus-like necrotic lesion. We treated the necrotic lesion on the glabella area by wound dressing combined with platelet-rich plasma (PRP) treatment.

4.Metastatic Skin Cancer That Showed Multiple Erythematous Patches on the Trunk.

Cancer that tends to metastasize to other internal organs also involves the skin with various clinical features. We report two cases of metastatic skin cancer that showed multiple erythematous patches on the trunk. Two patients presented with erythematous macules and a patch with telangiectasia on the trunk and chest wall. A microscopic examination of the skin biopsy specimens showed infiltration of large atypical cells with hyperchromatic nuclei and scant cytoplasm in the dermis. Using variable immunohistochemical staining, we identified the primary origin of the cancer, which was breast cancer. Usually, cutaneous metastatic carcinoma shows solitary and multiple nodules on the trunk, but there are also many unusual clinical findings. Dermatologists should pay attention to the sudden onset of symptomless erythematous patches and nodules on the trunk and conduct a skin biopsy and immunohistochemical staining.

5.Adult Onset Atypical Spitz Nevus.

Atypical Spitz's nevus is a rare spitzoid melanocytic proliferations with an uncertain malignant potential. Atypical Spitz's nevus have overlapping features of both Spitz nevi and spitzoid melanoma, and consequently, generate controversies with diagnosis and management. Herein, we report a case of atypical Spitz's nevus developing on the lower right leg of a 46-year-old man. Histologically, the lesion shows numerous uniform nests of spindle cells and epithelioid cells in the dermis. The lesion had a high Ki-index, prominent cellular density, but not enough to consider a diagnosis of melanoma. The nevus was removed by total excisions. No recurrences or metastases were detected during the follow-up period of 6 months.

6.Hysteroscopic evaluation of the efficacy of sonohysterography for diagnosis of submucosal myoma.

OBJECTIVE: To evaluate the efficacy of sonohysterography on exact submucosal myoma. methods: Transvaginal sonohysterography was performed on 45 patients who diagnosed as submucosal myoma and 29 patients who diagnosed as endometrial polyp by classic abdominal or vaginal sonogram, and then they received the diagnostic hysteroscopy at all. RESULTS: In 24 of 45 patients on sonohysterogram, submucosal myoma was diagnosed that was protruded to uterine cavity more than 2/3 of mass. Nine patients had intramural myoma and 12 patients had myoma that protruded only lesser than 1/3 of mass on sonohysterogram. These 21 patients revealed the same finding on hysteroscopy. On hysteroscopy, submucosal myomas were in 20 of 24 patients and polyps were in 3 patients and synechia was in one patients. In 8 of 29 patients on sonohysterogram, endometrial hyperplasia was found and confirmed by hysteroscopy. In 16 of 21 patients whose finding was polyp on sonohsyterogram, polyp was confirmed by hysteroscopy. However hysteroscopic findings were myomas in 3 of 21 patients and synechia in one patients. Therefore in 29 of 74 patients (39.2%), further hysteroscopy was not needed by sonohysterogram. In 65 of 74 patients (87.8%), sonohysterographic findings were same as hysteroscopic findings. CONCLUSION: Sonohysterogram for diagnosis of submucosal myoma and polyp is essential procedure in order to avoid unnecessary hysteroscopy. However differentiation between polyp and submucosal myoma has still some difficulty.

7.Erythroderma and Ichthyosiform Mycosis Fungoides.

Clinical manifestations of mycosis fungoides can vary from the classical type, which is associated with patch, plaque and tumor, to the rare atypical type which is associated with erythroderma, hypopigmentation or hyperpigmentation, and an ichthyosiform patch. A 45-year-old man presented with a 10-year history of asymptomatic erythroderma and an ichthyosiform patch on his trunk and lower extremities. On physical examination, there were asymptomatic erythematous nodules on the Rt. thigh and a palpable lymph node on the Lt. popliteal area. A histopathologic study showed infiltration of atypical cells into the upper dermis with mononuclear cell infiltration of the epidermis. On immunohistochemistry, atypical cells showed positivity for CD3, CD4, and CD8 staining, and negativity for CD20 staining. TCR-gamma rearrangement showed monoclonality. Excisional biopsy of Lt. popliteal lymph nodes showed pathology findings compatible with that of the trunk. A diagnosis of stage IVa mycosis fungoides, which is associated with erythroderma and ichthyosiform manifestations, was made. The patient was treated by a combination of PUVA and MTX chemotherapy and is still on regular follow up.

8.Telemedicine of Skin Diseases in Ulleung Island.

BACKGROUND: Telemedicine can be useful in dermatology on Ulleung island which is 217 km away from Pohang and without dermatologists. OBJECTIVE: The purpose of this study is to measure the effectiveness of telemedicine in treating skin diseases on Ulleung island. METHODS: 33 patients on Ulleung island were examined by a fullmotion real-time video by one dermatologist in a Dongsan hospital of Keimyung University once a week. From June 2011 to June 2012, there have been 53 treatments of telemedicine in 33 patients. RESULTS: 14 diseases were found in 33 patients. The most common disease group was infectious skin disease and eczema in 9 patients. In addition, there were other skin diseases such as acne vulgaris, allergic contact dermatitis, prurigo nodularis, lichen simplex chronicus, malignant melanoma, seborrheic dermatitis, polymorphous light eruption and so on. CONCLUSION: Dermatology favors telemedicine because we had investigated its value and possibilities by clinical experience of telemedicine between Ulleung island and Dongsan medical center in Daegu.

9.Tsutsugamushi Disease and the Importance of Performing a Thorough Medical History and Physical Examination.

Tsutsugamushi disease is an acute febrile illness that is caused by infection with Orientia tsutsugamushi. The diagnosis is based on the patient's history of exposure, the characteristic clinical features and serologic testing. But the diagnosis can be difficult when an eschar is overlooked or it is absent. Making the differential diagnosis with drug eruption is especially difficult when a skin rash develops with a prior medication history for fever or myalgia. We report here on 3 cases of Tsutsugamushi disease. Two of the cases were misdiagnosed as drug eruption due to medication for carbuncle or back pain. For the other case, Tsutsugamushi disease was accompanied with allergic contact dermatitis due to Rhus (lacquer tree). All the patients had fever, myalgia and an atypical skin rash that differed from that of drug eruption and allergic contact dermatitis. The detection of eschar was possible by having a clinical suspicion of Tsutsugamushi disease. Thus, we report here on three cases of Tsutsugamushi disease and these cases show the importance of taking a complete medical history and performing a thorough physical examination.

10.Therapeutic Trial of Platelet-rich Plasma for Superficial Ulcers Caused by Herpes Zoster.

Platelet rich plasma is derived from fresh whole blood that contains a high concentration of platelets. There are many clinical reports of the effect of PRP on diverse surgical procedures, especially in the fields of orthopedic surgery and periodontic surgery. Recently, PRP has been rapidly developing as a method of wound healing and skin rejuvenation. Therefore, the aim of this study was to evaluate the effectiveness of PRP on wound healing after herpes zoster infection. We analyzed the medical records and clinical photographs of 12 patients who underwent 1-2 PRP treatments of superficial ulcers after herpes zoster infection. The wounds that were treated by PRP healed rapidly compare to non-treated wounds. This study shows that PRP treatment is a safe and effective treatment option for skin ulcer after herpes zoster infection.
