3.A case of nevus sebaceous associated with sebaceous epithelioma and basal cell epithelioma.

We report a case of nevus sebaceus associated with sebaceous epithelioma and basal cell epithelioma in a 48-year-old male patient. A 2.6 x 2.0cm orange-yellow erosive tumor and 1.5 x 1.0cm, l.0 x 0.8cm dark blue-yellow varigated nodules were intimately associated with a 4 x 3cm sized yellowish verrucous plaque on the parieto-occipital area of the scalp. Biopsy revealed sebaccous epithelioma, basal cell epithelioma and nevus sebaceus, respectively.

4.A case of type IV hyperlipoproteinemia with xanthoma tuberosum and xanthoma striatum palmare.

We experienced a case of a 30 year-old male with hyperlipoproteinemia associated with xanthoma tuberosum and xanthoma striatum palmare. The laboratory findings showed increased serum cholesterol and triglyceride on lipid profile, and widening of prep band on elec1rophoresis of lipoprotein. The biopsy specimens of orange yellow colored nodules on the elbow and yellow streaks on the palrnar creases showed characteristic findings of xanthoma. He had been treated with gempibrozil(lopid) for 1 month.

9.Long-Term Outcome of Surgery for Intermittent Exotropia.

PURPOSE: To evaluate long-term outcomes of surgery for intermittent exotropia. METHODS: The authors evaluated 78 patients who underwent surgery for intermittent exotropia and were available for a follow-up over a 5-year period. According to distant exodeviation and fusion control at the last visit, the patients were classified into either the stable group (distant exodeviation < or =10 PD) or the recurrent group (distant exodeviation >10 PD). The recurrent group was reclassified into the clinical success group (distant exodeviation >10 PD, <20 PD with good fusion control) or the clinical failure group (distant exodeviation > or =20 PD or distant exodeviation >10 PD, <20 PD with poor fusion control). We analyzed recurrence rate, success rate, and clinical outcome. RESULTS: Recurrence rate was 65.3% (n = 51), and the surgical success rate calculated as the rate of stable group and clinical success group was 73% (n = 57). The postoperative angle of exodeviation was more decreased than the preoperative angle in 93.6% of patients. When comparing postoperative with preoperative sensory function, only 2.6% of patients demonstrated worse distant fusion control grades, 66.7% of patients remained the same, 30.8% improved, and 5.1% of patients demonstrated poorer near stereopsis. The rest of patients had better or equal fusion control and stereoacuity. Age at surgery in the recurrent group was less than in the stable group (p = 0.004) and the recurrent group had worse preoperative distant fusion control (p = 0.021). Exodeviation angle of the recurrent group at postoperative 1 month, 3 months and 6 months was greater than that of the stable group (p < 0.005). CONCLUSIONS: Despite a high recurrence rate, surgery for intermittent exotropia showed a high clinical success rate and good long-term efficacy and safety in both deviation angle and sensory function.

10.Changes of Splenocyte Proliferative Capacity and Subpopulation of Peripheral Lymphocytes Related to the Hemorrhage Amount in Rats.

BACKGROUND: Hemorrhage itself has been shown to produce abnormalities in immunity, particularly depression of the lymphocyte function. In order to better examine the amount of hemorrhage required to suppress the lymphocyte function, we determined the effect of graded fixed-volume hemorrhage on splenocyte proliferation and the lymphocyte subpopulation. METHODS: Male Sprague-Dawley rats(weight, 350~400g) were anesthetized, subjected to hemorrhages of 7.5ml/kg, 15ml/kg, and 22.5ml/kg by percutaneous cardiac puncture with 26G needles. After 1, 2, 4, and 7 days, animals were killed to obtain the blood and spleen. The splenocyte proliferative capacity was measured by using the tritiated thymidine incorporation technique, and the peripheral lymphocyte subpopulation was determined using flow cytometry with the following monoclonal antibodies: T cell(CD3+), T helper cell(CD4+), T cytotoxic cell(CD8+), and B cell(CD45RA+). RESULTS: Hemorrhage of 7.5ml/kg did not induce depression of splenocyte proliferation. However, for hemorrhage greater than 15ml/kg, the splenocyte proliferative capacity was significantly depressed at 2 days after hemorrhage and recovered at 4 days. Hemorrhage induced no changes in the relative percentage of lymphocyte subpopulations and in the number of each cell in peripheral blood. CONCLUSION: This study suggests that cellular immunity is depressed at 48 hrs after a hemorrhage greater than 15ml/kg without any change in the peripheral lymphocyte subpopulation.
