1.MR Findings of Sturge-Weber Syndrome Without Facial Nevus: Two Cases Report.

PURPOSE: We reported the CT and MR findings of 2 cases with Sturge-Weber syndrome which were not accompanied by facial nevi. MATERIALS AND METHODS: They were examined with both CT and MR in one case and with MR only in the other case. RESULTS: CT was better than MR in the demonstration of the characteristic cortical calcification. MR was superior to CT in the depiction of the abnormalities of the surrounding parenchyma and the intense enhancement of pial angiomatosis with Gd-DTPA enhancement. CONCLUSION: Gd-DTPA enhanced MR imaging could be useful in the demonstration of the presence and extent of pial angiomatosis in patients with suspected Sturge-Weber syndrome.

2.Knowledge and Ability Toward CPR in Metropolitan Dental Hygienists.

PURPOSE: The purpose of this study was to evaluate Cardiopulmonary Resuscitation ability and level of knowledge among certain dental hygienists in Seoul and Gyeonggi-do. METHODS: We surveyed ability and knowledge of CPR among dental hygienists working at dental offices and dental hospitals located in Soul and Gyeonggi-do from September 18, 2010 to October 16, 2010. Data acquired from dental hygienists who had completed the questionnaire that we designed were used in the analysis. Uni- and bi-variate analysis and Chi-square test were performed using SAS 9.2. RESULTS: According to our findings, among respondents, approximately 14.29% of dental hygienists had a CPR license. Their CPR licenses are affected by working period, job position, and level of education (p<.05). Factors including working period, type of CPR license, and CPR license renewal were indicators of CPR knowledge level (p<.05). Significant differences were observed among dental hygienists with regard to subjective CPR abilities, which is dependent on obtaining a CPR license, CPR knowledge level, and education channel for CPR (p<.05). CONCLUSION: CPR ability and knowledge level among dental hygienists were not sufficient to save emergency people in the dental clinic. In order to save emergency patients patients, implementation of a CPR education and renewal program for dental hygienists is recommended.

3.Hamartoma of the Breast A report of two cases.

Hamartoma of the breast is a rare benign lesion presenting as palpable mass. They are, however, sometimes misdiagnosed by pathologist due to their resemblance to other benign or physiologic conditions of the breast. We report two cases of hamartomas of the breast. The clinical diagnosis was fibroadenoma in both cases. One patient was a 49-year-old woman presented with palpable mass for the duration of 5 months. Mammography showed a well demarcated round mass with homogeneous density which is slightly greater than the surrounding glandular tissue. Grossly it was a 3.5 3.0 2.0 cm sized round firm mass covered by thin fibrous tissue. Microscopically it was a well defined mass composed of variable amount of breast lobules, dilated ducts and adipose tissue surrounded by dense hyalinized connective tissue. The adipose tissue accounted for 30% of the entire lesion. Cellular atypism was not found. The other patient was a 18-year-old woman presented with palpable mass of the duration of 3 years, which was increased in size recently. Mammography demonstratesd a well delimited lobulated mass with lucent halo. Ultrasonography showed a well encapsulated low echoic mass admixed with high echoic areas. Grossly, it was a 4.0 4.0 2.0 cm sized well demarcated oval-shaped firm mass with translucent thin fibrous capsule.. Microscopic findings were similar to the former. The amount of adipose tissue was 20%.

4.Quality of care in inflammatory bowel disease in Asia: the results of a multinational web-based survey in the 2nd Asian Organization of Crohn's and Colitis (AOCC) meeting in Seoul.

BACKGROUND/AIMS: The quality of care in inflammatory bowel disease (IBD) has not been systematically estimated. The aim of this study was to investigate the current status of quality of IBD care in Asian countries. METHODS: A questionnaire-based survey was conducted between March 2014 and May 2014. The questionnaire was adopted from "An adult inflammatory bowel disease physician performance measure set" developed by the American Gastroenterological Association. If the respondent executed the performance measure in more than 70% of patients, the measure was regarded as well performed. RESULTS: A total of 353 medical doctors from Asia completed the survey (116 from Korea, 114 from China, 88 from Japan, 17 from Taiwan, 8 from Hong-Kong, 4 from India, 3 from Singapore, and 1 each from the Philippines, Malaysia and Indonesia). The delivery of performance measures, however, varied among countries. The documentation of IBD and tuberculosis screening before anti-tumor necrosis factor therapy were consistently performed well, while pneumococcal immunization and prophylaxis of venous thromboembolisms in hospitalized patients were performed less frequently in all countries. Physician awareness was positively associated with the delivery of performance measures. Variations were also noted in reasons for non-performance or low performance of quality measures, and the two primary reasons cited were consideration of the measure to be unimportant and lack of time. CONCLUSIONS: The delivery of performance measures varies among physicians in Asian countries, and reflects variations in the quality of care among the countries. This variation should be recognized to improve the quality of care in Asian countries.

6.Differences in Bacterial Species and Their Resistance Rates based on Sputum Cultures between Tertiary Hospitals and Smaller Medical Institutions.

OBJECTIVES: Since the 1990s, drug-resistant bacteria have become common pathogens of hospital-acquired infections. In recent years, healthcare-associated infections have come to the fore, and it is reported that distribution rates of these bacteria are comparable to those of hospital-acquired infections. However, there have been few studies on differences in resistant bacteria depending on the size of hospitals. Thus, the authors studied differences in drug-resistant bacteria between a tertiary hospital and smaller medical institutions. METHODS: We retrospectively analyzed the clinical findings and sputum culture results of patients transferred from tertiary hospitals (group A, n=74) and those transferred from smaller medical institutions (group B, n=65). RESULTS: The number of patients with malignancy was higher in group A than in group B. The length of intensive care unit stay was longer in group A than in group B. Antibiotic therapy and mechanical ventilation were more frequently used in group A than in group B. There were no significant differences between the 2 groups in bacterial species (Staphylococcus aureus, Escherichia coli, Klebsiella pneumoniae, Pseudomonas aeruginosa) and their resistance rates to carbapenem, while there were significant differences between the 2 groups in the bacterial species (Acinetobacter baumannii) and its resistance rate to carbapenem. CONCLUSION: In this study, there were significant differences between the 2 groups in the bacterial species and resistance rates to carbapenem for A. baumannii infection unlike other bacterial infections. Further studies on risk factors and patient classification are needed to confirm our results.

7.The Dose and Effects of Sodium Nitroprusside ( SNP ) on the Cardiovascular System During SNP - induced Hypotension under General Anesthesia with Enflurane .

Sodium nitroprusside given by intravenous drip has been used to control hypertensive crises since 1929. Its evanescent action makes it suitable for controlled hypotension during general anesthesia with enflurane. The patients (n=10) were all undergoing Cotrel-Dubousset instrumentation for the operative treatment of scoliosis or spinal stenosis. Hemodynamic variables were obtained during administration of sodium nitroprusside. Mean arterial pressure was decreased to 50-60mmHg, while cardiac output, central venous pressure, systemic vascular resistance showed only minor changes. The average dosage of sodium nitroprusside (ug/kg/ min), plotted against age, showed a significant correlation (p<0.05, r= -0.63). The amonut of whole blood transfused during operation was 5.7+/-2.2 units (mean+/-SD) and postoperative hemoglobin and hematocrit (11.19+/-1.21g/dl, 33.49+/-3.79%, respectively, mean+/-SD) were singificantly lowered than preoperative hemoglobin and hematocrit (12.94+/-1.78g/dl, 37.22+/-4.88%, respectively, mean+/-SD) (hemoglobin: p<0.004, hematoorit: p<0.006).

8.Effects of Endotracheal Intubation on the Temporomandibular Joint.

Tracheal intubation for general anesthesia is usually performed using a rigid laryngoscope and reguires the mandible to be opened with forward and upward traction of the patient's jaw which may at times result in the inadvertant injury to temporomandibular joint(TMJ). The injury may include internal derangement, dislocation and hematoma of TMJ; subsequent intraarticular adhesion formation ; dislocation of the meniscus, and rarely auriculotemporal nerve damage from traumatic TMJ dislocation. We studied in 200 patients the size of mouth opening during intubation, the change of mouth opening by tracheal intubation evaluated after operation, and any TMJ disorder arising after tracheal intubation. The results were as follows; 1) The mean size of mouth opening before operation was 42.7+/-7.0 and 40.6+/-6.2mm in male and female patients, respectively. 2) The mean size of mouth opening during intubation was 24.7+/-2.6 and 23.4+/-2.7mm in male and female patients, respectively. When vecuronium was used to prevent fasciculation, the mean was 24.3+/-3.1 and 25.2+/-2.0mm in male and female patients, respectively. 3) One week following operation The mean size of mouth opening one week postoperation was 48.3+/-8.9 and 42.2+/-6.3mm in male and female patients, respectively. When vecuronium was used to prevent fasciculation, the mean size of mouth opening was 55.5+/-5.3 and 43.2+/-6.2mm in male and female patients, respectively. 4) Five patients complained of discomfort around TMJ after tracheal intubation. It seems that upward 45 mandibule lifting by laryngoscope caused trauma to TMJs.

10.MRI Findings of Experimentally Induced Hepatic Infarction: Correlation between Changes of MRI Findings of Liver Parenchyma and Capsule with Time Lapse and Histopathology.

PURPOSE: We experimentally induced hepatic infarction in rabbits to evaluate MR findings of liver parenchyma and capsule and its changes with time and to confirm the capsular structure correlating with its histologic findings. MATERIALS AND METHODS: After ligation of hepatic artery, vein and duct of right inferior posterior lobe of liver, T1, T2 weighted and enahnced T1 weighted image were obtained at several time intervals. Histologic samples were taken of two rabbits or more at each time intervals. RESULTS: During the first several days, the signal intensity of the ischemic necrosis showed strong high signal intensity relative to normal liver on both T1 and T2 weighted images. After 2 weeks however, the necrotic areas gradually changed to isointensity or low signal intensity. Capsular structure was observed as slightly high signal intensity compare to ischemic areas on both enhanced T1 and T2 weighted images in six cases, and five cases of 12, respectively. From the first day, homogeneous coagulation necrosis without hemorrhage or liquefaction was observed. Fibrous thickening with rich vascularity was observed along the surface of the necrotic area after two weeks. CONCLUSION: During the first several days, the signal intensity of the ischemic necrosis showed strong high signal intensity on both T1 and T2 weighted images and gradually changed to isointensity or low signal intensity. Liver capsule was shown and slightly high signal intensity along the surface of the necrotic area and could be explained by fibrous thickening of the liver capsule end the rich vascularity within in it.
