3.A Case of Extramammary Paget's Disease on the Scalp.

We treated a case of extramammary Paget's disease that affected the scalp of a 45-year-old female. It is extremely rare that the disease arises in areas other than the anogenital region and the axillae. The lesion was a round erythematous oozing crusted hairless patch. Histopathologically, many pagetoid cells were found within the epidermis and dermis. The cytoplasms of these cells stained with alcian blue at pH 2.5, CEA, EMA and low-molecular-weight-cytokeratin. The patient underwent a wide local excision. We used a mapping technique to reveal the distribution of microscopically involved lesions.

6.Clinical Outcome of Small Incision Lenticule Extraction including Visual Quality Analysis.

PURPOSE: To present the clinical outcomes of small incision lenticule extraction (SMILE) including visual quality analysis in Korean patients with myopia METHODS: The medical records of 228 eyes of 116 patients who underwent SMILE in HanGil Eye Hospital LASIK Center from May 2014 to Feb 2015 and were followed-up for at least 3 months was analyzed retrospectively. The patients were followed up at 1 day, 1 week, 1 month, and 3 months after the operation. Refractive value, visual acuity, intraocular pressure, and visual quality were measured at each visit RESULTS: Preoperatively, uncorrected distant visual acuity was 0.01 ± 0.02 in log MAR, spherical equivalent was -5.03 ± 1.72 diopters, intraocular pressure was 15.85 ± 2.85 mm Hg, and the objective scattering index (OSI) value was 0.68 ± 0.49. The postoperative uncorrected distant visual acuity was 0.13 ± 0.10, 0.05 ± 0.08, 0.04 ± 0.09, and 0.02 ± 0.04 and OSI was 2.16 ± 1.89, 1.25 ± 0.64, 1.14 ± 0.69, and 0.81 ± 0.36 at 1 day, 1 week, 1 month, and 3 months after the operation, respectively. The postoperative intraocular pressure was 12.55 ± 3.74 mm Hg, 13.03 ± 4.35 mm Hg, 11.65 ± 2.49 mm Hg at 1 week, 1 month, and 3 months after the operation. The efficacy of refractive surgery 3 months after the operation was 0.97 ± 0.11, the safety was 0.99 ± 0.10, and predictability was 99.56% and 100.00% at the range of ±0.5 diopters and ± 1.0 diopter, respectively. CONCLUSIONS: The SMILE operation showed comparable clinical outcomes with conventional refractive surgery in terms of efficacy, safety, and predictability. Intraocular pressure and visual quality normalized gradually throughout the 3-month postoperative period.

8.The Relationship Between the Activity of Matrix Metalloproteinase in the Aqueous Humor and the Development of Primary Open Angle Glaucoma.

To investigate whether the abnormal expression of matrix metalloproteinases(MMPs) in the aqueous humor may case the development of glaucoma, the activity of MMP in the aqueous humor of galucoma and cataract patients were measured and compared. Six primary open angle glaucoma(POAG), 2 chronic angle closureglaucoma(CACG), 2 normal tension glaucoma(NTG), and 14 cataract patients were enrolled. Aqueous humor of each patients were collected during surgery, and total protein concentration and activity of gelatinase A in the aqueous humor were measured by protein assay kit and zymography, respectively. Total protein concentration and gelatinase A activity of POAG patients were increased by 2.1 times and 3.9 times, respectively compared to those of cataract patients. However, there were no statistically significant changes in total protein concentration and gelatinase A activity of CACG and NTG patients compared to those of cataract patients. In conclusion, the development of POAG may be associated with the abnormal expression of MMP in the aqueous humor.

9.The correlation between dental compensation and craniofacial morphology in skeletal Class III malocclusion.

This investigation was designed to analyze the degree of dental compensation according to horizontal components of craniofacial skeleton and to investigate correlation between dental compensation and craniofacial pattern in skeletal class III malocclusion. The material selected for this study consisted of standard lateral cephalogram of 59 subjects in normal occlusion group, 91 subjects in mild skeletal class III malocclusion group and 58 subjects in severe skeletal class III malocclusion group. The mild skeletal class III malocclusion group was divided into two groups, one was class III malocclusion without anterior crossbite group and the other was class III malocclusion with anterior crossbite group. The data were analyzed by Quick-ceph image program. The results were as follows. 1. Mild skeletal class III malocclusion without anterior crossbite group showed the most labial inclination of upper incisors, followed by severe skeletal class III malocclusion group and mild skeletal class III malocclusion with anterior crossbite group, the Latter showing the least. The amount of lingual inclination of lower incisors was the largest in severe skeletal class III malocclusion group, and there was no statistically significant difference between mild skeletal claw III malocclusion without anterior crossbite group and mild skeletal class III malocclusion with anterior crossbite group. 2. There were little differences in vertical skeletal structure between mild skeletal class III malocclusion without anterior crossbite group and mild skeletal class III malocclusion with anterior cwssbite group, they showed statistically significant differences in the upper incisors measurements. 3. The measurements of lower incisors in mild skeletal class III malocclusion without anterior crossbite group and upper incisors in mild skeletal class III malocclusion with anterior crossbite group represented a high correlation with skeletal structure. Especially, deltaIMPA and deltaFMIA of lower incisor measurements, and deltaU1-FR deltaUi-SN of upper incisor measurements showed high correlation with skeletal structure in each group. 4. deltaIMPA and deltaFMIA of lower incisor measurements showed high correlation with skeletal structure in all groups. deltaUI-FH, deltaU1-SN and Ui-facial plane(mm) of upper incisor measurements represented higher correlation with skeletal structure than any other upper incisor measurements.
