5.Comparison of the depression and anxiety between the elderly in the home for the aged and those in the community.

This survey was conducted from January 1991 to May 1991. Two hundred and eight residents in 4 government supported homes for the aged and hundred and eleven living in the community in Taegu, Korea, were examined to evaluate the depression and the anxiety of the aged with combined anxiety and depression scale (CADS) and somatization symptom check list (SCL). There are no significant differences between residents in the home for the aged and those living in the community on the total scores of CADS and SCL. However, in the case of the total score of CADS of the female subjects in the home for the aged were significantly higher than those of the community residents. The elderly in the home for the aged tended to have pure depression, while community residents were likely to have anxiety and depression. Fifty-two subjects of home for the aged and sixty-nine of the community scored over 50 points of CADS, which indicates considerable depression or anxiety. In psychosocial factors, the subjects who in the following situations had statistically significant higher scores than others. The results were as follow. Poor health, unhappiness, unsatisfaction to the past occupation, pessimistic thought in future view for both group and unsatisfaction of the relationship with familiar people for the elderly in the community. The SCL scores of two groups subgrouped by under 49 and 50 on CADS showed significant differences between each subgroup on all of the SCL items. It could be suggested that somatic symptoms for the aged is a sign of depression.

7.Transesophageal Echocardiographic Evaluation of Pulmonary Venous Flow before, after and One-year after Percutaneous Mitral Valvulopasty in Patients with Mitral Stenosis in Sinus Rhythm.

BACKGROUND: To evaluate the influence of changes in mitral valve area (MVA) and left atrial pressure on pulmonary vein flow (PVF) we analyzed PVF with transesophageal echocardiography (TEE) before, after and one-year after percutaneous mitral valvuloplasty (PMV) in patients with mitral stenosis (MS) in sinus rhythm. METHODS: PMV was guided with TEE. Follow-up TEE was done about one year after PMV. MVA and transmitral mean gradient (TMG) were measured. Systolic velocity (S), diastolic velocity (D), atrial reversal velocity (AR), their time-velocity integral (S-TVI, D-TVI, AR-TVI) and their ratio (S/D ratio, S-TVI/D-TVI ratio were evaluated. RESULTS: The number of patients was twenty-two (F:20). The age was 39+/-9 years (range:26-64). Follow-up duration was 16+/-6 months (range:7-28). MVA increased from 0.9+/-0.2 cm2 to 1.9+/-0.3 cm2 after PMV and decreased to 1.7+/-0.3 cm2 on follow-up TEE significantly. TMG decreased from 15.4+/-4.3 mmHg to 5.5+/-1.9 mmHg after PMV and was 6.2+/-2.4 mmHg on follow-up. S increased significantly on follow-up at both pulmonary vein (PV). D increased on follow-up at left PV. S/D ratio increased on follow-up at both PV. AR increased on follow-up at both PV. S-TVI increased after PMV at left PV and increased on follow-up at both PV. D-TVI had no change. S-TVI/D-TVI ratio increased on follow-up at left PV. AR-TVI increased on follow-up at right PV. CONCLUSIONS: The main changes after PMV in patients with MS in sinus rhythm were increasing tendency in S, S-TVI, S/D ratio, S-TVI/D-TVI ratio and AR. And these changes were statistically significant on follow-up TEE rather than immediately after PMV.

9.Tinea Capitis of Adults in Taegu City for 11 Years ( 1978 ~ 1988 ).

Epiderniologic description of the tinea capitis of adults was made for the cases detected in Taegu city for 11 years from January 1978 to December 1988. 1. Totalnumber of adult patients(20 years or older) were 41(2.0%) out of 2,029 cases of all tiena capitis. Among 41 patients, 34 cases were detected between 1983 and 1988. 2. Clinically 24 patients were superficial tinea capitis and 17 patients were kerion celsi. 3. Age of the patients ranged from 21 to 85 years with an average of 58.5 years and mode interval of 61-70 years. Patients over 50 years of age accounted for 70.7% of all csses. 4. Sex distribution of the cases was 39 females(95.1%) and 2 males(4.9%). 5. Causative agents isolated from the patients were Microsporum canis in 36 cases, Trichophyton verrucosum in 2 cases, T. mentagrophytes, T. ferrugineum and T. rubrum in 1 case, respectively. 6. Solitary lesion was most common and alopecia was not prominent in most of the cases. The most frequent site of involvement was frontal and posterior scalp. At initial exarnination, 5 cases were misdiagnosed as seborreic dermatitis and 1 case as multiple folliculitis. 7. Sources of infection were presurned to be cat in ll cases, cattle in 2 cases and the rests were unidentified. Interfarnilial infection was observed in 5 cases.

10.A Case of Vitiligo Associated with Systemic Lupus Erythematosus.

We present a case of systemic lupus erythematosus(SLE) asseci ted with vitiligo in a 57 year-old male patient. Physical examination revealed two distinct skin lesions those of itiligo showed as centrally located depigmented patches surrounded by erythernatous patch on the right cheek, scalp and left dorsurn of hand and those of SLE showed as erythematous papules and patchs with partial adherent scales on the face, scalp, neck, both extensor surface of arm, and left dorsun of hand. Their coexistence lends credence to the contention that altered autoimrnunity may play a role in the pathogenesis of each of these diseases. In Western countries and Japen, patients with lupus erythema osis have been observed to develop vitiligo or depigmented spots on the skin, while in Korea, its has not been previously observed.
