5.Arthroscopic Treatment of Pathologic Plica of Knee Joint: Centered on Mediopatella Plica Syndrome.

Fifty-nine knees with a specific diagnosis of mediopatella plica syndrom; under arthroscopy were treated by arthroscopic resection. The patients were se1ected for arthroscopy only if the symptoms had continued after a course of physical therapy and medication for at least three weeks. Seven patients were lost in follow up period. Fifty-two knees were reviewed after mean period of 21.5 monl;hs (range 12 to 37). The results were graded as excelient (61.5%), good (25%), poor (9.6%) and failure (3.9%). Majority of the patients ratect their result as excellent or good. We concluded that with the meticulous resection of the pathologic plica with arthroscopy, good result can he expected with resumption of athletic activities and no or mild pain in knee joint.

8.Telemedical Marit Assistance Service by Fire Safety Headquarter.

PURPOSE: Telemedical maritime assistance service (TMAS) is performed in several ways when a seafarer falls sick under circumstances where the ship is far away from shore without possibility of medevac. The current methods operating in Korea are described and compared in order to grasp the actual circumstances in terms of importance. METHODS: From June 2012 to July 2013, a total of 523 cases were consulted roughly by means of satellite-phone, e-mail, and website homepage at TMAS center of Busan metropolitan city fire safety headquarters and the final clinical outcomes of these registered cases according to illness or injury after a certain period of time was studied retrospectively. RESULTS: Statistically significant differences in classification of ship company, headquarters of ship company informed, added photos, added drug-list, and final clinical outcome (p<0.001, p<0.001, p<0.001, p<0.001, p=0.027, respectively) according to whether or not homepage-consultation were observed between crossed-analyzed groups; and significant differences in age-group, consult through satellite-phone, added photos, and final clinical outcome according to illness or injury were observed between crossed-analyzed groups (p<0.001, p=0.046, p=0.017, p<0.001, respectively). CONCLUSION: The website designated for TMAS is expected to play a positive role. In addition, education of seafarers should also be implemented at the same time because the off-shore circumstances are quite different from those of onshore.

9.Effect of Ischemic Preconditioning on the Functional Recovery of Myocardium: Isolated heart experimental study.

BACKGROUND: Brief episode of coronary artery occlusion (i.e., ischemic preconditioning) makes the heart more resistant to injury from a subsequent ischemic insult. Although a great deal of effort has been made in studying ischemic preconditioning, the underlying mechanism of ischemic preconditioning and its effect on hypothermic insult has not been elucidated. This study was performed to see whether ischemic preconditioning protects against the depression of cardiac contractility induced by hypothermic cardioplegic arrest/reperfusion. And recently, adenosine was known to have some correlation with the mechanism of preconditioning. If so, does this effect remain after the blockade of adenosine receptor by 8-phenyl theophylline? METHOD: Twenty-four Sprague-Dawley rat weighed 250-350g were used and divided into three groups. Rat hearts were removed rapidly, and each isolated heart paced with a rate of 180/min was perused by modified Krebs-Hensleit buffer(KHB) solution on a Langendorff apparatus far an hour. After obtaining baseline data including left ventricular pressure(LVP), dp/dt, and coronary flow, cardiac arrest was induced by perfusion of 0degrees C crystalloid cardioplegic(St Thomas) solution. After that, all hearts were stored in the same St Thomas solution at salute temperature far 2 hours. In group I (control group), the hear was reperfused by KHB solution. In group II(preconditioning group), the heart was subjected to two 2-minute episode of global ischemia followed by 5 minute reperfusion with KHB solution(preconditioning) before cardiac arrest. In group III(phenyl theophylline group), the heart was subjected to preconditioning procedure and 8-phenyl theophylline at 10muM in concentration was added to KHB solution at time of reperfusion. Observing parameter was obtained in each group at 10, 20, 40 and 60 minutes after starting reperfusion and compared statistically by use of one way ANOVA test(STASTICA, release 4.5). P-value less than 0.05 was considered significant. RESULTS: Although depressed LVP, dp/dt, and Coronary flow were seen in all groups during the reperfusion period, the preconditioned group showed more effective recovery of LVP than that of the control group, especially at 10, 20 and 40 minutes(p<.05). We failed to demonstrate the difference between the phenyl theophylline group and the control group(p=NS). CONCLUSION: These results suggest that ischemic preconditioning has protective effect on recovery state of hypothermic cardioplegic arrest/reperfusion. Its protective effect was limited during early reperfusion stage and was blocked by adenosine blocker.

10.New Technique of Anterior Chamber Tube Shunt to an Eneircling Band for Refractory Glaucoma.

New techniques of anterior chamber tube shunt to an encircling band (ACTESB) were used in 49 eyes of 47 patients with neovascular glaucoma and other types of refractory glaucoma. In 34 eyes of 33 patients (Group A), by using Isingle slit incision and end suturing technique at the distal portion of tube, intraocular bressure was controlled less than 21mmHg in 21 eyes (61.8%) with a follow-up of 6 months to 18 months (mean, 13 month). The mean intraocular pressure of 36.38mmHg was reduced. Visual acuity was maintained or improved in 61 %. In 15 eyes of 14 patients (Group B), by using multiple slit incision at the distal portion of tube and single slit incision of the tube beneath scleral flap and external occlusion of tube with 8-0 vicry1, successful outcome was obtained in 86.6% of eyes after average follow up of 6 months. The mean intraocular pressure of 41.47mmHg reduced. Visual acuity was maintained or improved in 86.8%. The incidence of flat anterior chamber was decreased markedly and hyphema was not found in group B. In 4 eyes, there was evidence of blockage of the distal end of tude. Thus we think that the pattern of one piece valved implant is better than the other technique.
