1.Teratocarcinoma of Testis: A Report of a Case and Review of Literature.

The first case of testicular teratocarcinoma in Korea, found in 24 year old service man was reported. Complication of the metastases to various organs, gynecomastia and hydrocele was seen at the first examination and simple orchiectomy and delayed x-ray deep therapy were not effective to survive this case. The patient died 3 months after the affliction. Autopsy revealed matastases were to lung, spleen, lower poles of both Kidneys, mediastinal and lumbar paraaortic lymph nodes and histopathological examination showed the teratocarcinoma composed of seminoma, embryonic carcinoma and chorioepitheliomatous elements. Additionally, 13 cases of testis tumors reported in literatures of recent years in-country have peen reviewed.

3.The Immunologic Expression of Apoptosis Related Proteins and Apoptotic Cells in Human Ovarian Follicles.

OBJECTIVE: To investigate the expression of apoptosis related proteins and apoptotic cells on the human ovarian follicles. MATERIALS AND METHODS: Thirty five Formalin-fixed paraffin-embedded human ovarian tissue blocks were selected from the surgical pathology files of the department of pathology, College of Medicine, Yonsei University, for the period from 1996 to 1998. All specimen were from premenopausal women aged from 32~45. Ovarian tissues were collected from the patients performing hysterectomy for benign uterine diseases. Immunohistochemical staining was performed for the detection of DNA fragmented cell, Bcl-2, Bax, Fas and Fas-ligand. RESULTS: Bcl-2 and bax were not expressed on the surrounding cells and oocyte of the primary, primordial and preantral follicles. Fas and Fas-ligand (Fas-L) were not expressed on the surrounding cells on the primordial and primary follicles. But expressed on the surrounding granulosa cells and oocyte in the primordial and primary follicles. In the healthy follicles, Bcl-2 was expressed on the granulosa cells, however, Bax was not expressed. DNA fragmented cells were expressed on the inner granulosa cell layer of atretic follicles. CONCLUSION: Fas, Fas-ligand, and Bax may be responsible for the follicular atresia and Bcl-2 may be involved in the follicular survival in the human ovary.

4.A Case of Acute eosinophilic pneumonia.

Acute eosinophilic pneumonia is reported as a specific disease entity. But, it is different from chronic eosinophilic pneumonia in its onset, clinical course and recurrence. Badesh et al reported the following diagnostic criteria os acute eosinophilic pneumonia a less than one-month history of symptoms prior to diagnosis, no evidence of asthma, the absence of other organic disease, no obvious etiology and an evidence of recurrent disease. We experienced a case of acute eosinophilic pneumonia in 37 old male. Pathologically eosinophilic pneumonia is confirmed and other features meet Badesh's criteria.

5.A Case of Type 2 Leprosy Reaction in Borderline Lepromatous Leprosy Masquerading as Orbital Cellulitis in an Immigrant Worker

Hansen's disease(HD) is a chronic infectious disorder acquired by inoculation of Mycobacterium leprae. With the establishment of complex multidrug therapy, the incidence rate of leprosy patients has continually shown to decline by 90% compared to the incidence rate in the 1990s. However, the prevalence of the disease still remains high in southeast asian countries. Due to the rarity and diverse nature of cutaneous presentation, HD is often misdiagnosed with other dermatoses or infectious conditions. Especially, when a patient presents with unusual presentation with leprosy reaction with no classical feature such as sensory disorders and skin lesion, the diagnosis is further delayed with misguided treatments. Herein we present a 27-year-old Indonesian immigrant who displayed clinical features mimicking that of orbital cellulitis who was later diagnosed with borderline lepromatous leprosy through histologic and PCR confirmation, in light of alerting the probability of leprosy in immigrants with intractable skin presentations.

6.Clinical Study of Inguinal Hernia in Children.

A total of 335 inguinal hernias in children were analyzed by the authors at the Department of General Surgery, St. Benedict Hospital, for last 10 years, from 1986 to 1995. The male patients were predominant (2.25:1), and 78.2% were under 2 years of age. Right side was 1.63 times more frequent than the left. Among the 19 cases of incarcerated hernias, 84% could be reduced preoperatively in safe. Only 3.6% of the cases were repaired by Bassini procedure, but the others didn't require posterior wall reconstruction. Bilaterality was 25 cases (7.5%), and 8 cases (2.4%) developed later contralateral hernias after primary ipsilateral fix. Complications were in 15 cases (4.5%) such as scrotal seroma and/or hematoma (3%), wound infection (0.6%), pneumonia (0.9%). For the more comprehensive care for the herniated children, specialized practice by a pediatric surgeon would be required in the future.

Periventricular leukomalacia is a major cause of neuro-developmental delay in premature infant. Although it develops in only a small percentage of preterm infants, the condition poses a major problem. The pathogenesis of periventricular leukomalacia is not well established. Because of the topography of the lesion, a decreased cerebral perfusion occurred before delivery is thought to be a critical pathogenetic factor. Therefore, hypoxic brain damage is thought to be a major cause of periventricular leukomalacia. The importance of neurochemically mediated injury to the white matter has been stressed. Recently, high incidence of placental chorioamnionitis with periventricular leukomalacia. We experienced a case of periventricular leukomalacia related to chorioamnionitis. The pathology of placenta was chorioamniontis, and it was thought to be a cause of periventricular leukomalacia. So, we report our case with a short literature review to ensuring that chorioamionitis without perinatal asphyxia cause a periventricular leukomalacia.

9.A Case Of Primary Gastric Choriocarcinoma Associated With Adenocarcinoma.

Choriocarcinoma is a HCG-producing epithelial neoplasm derived from either trophoblastic or totipotential germ cells. It is a highly malignant tumor, which is rapidly invasive and widely metastatic, and in most cases develops within the uterus after a normal or abnormal gestation, including ectopic pregnancy. Non-gestational choriocarcinoma rarely occur as a primary neoplasm in the gonads or in the extragonadal midline locations such as mediastinum, retroperitoneum, and pineal gland. It also has been reported very rarely to arise in parenchymal organs such as the prostate, lung, breast, brain, liver, kidney, bladder, vagina, and gastrointestinal tract. A 58-year old man was admitted to our hospital with symptoms of tarry stool and epigastric discomfort. The endoscopic biopsy specimen initially revealed pooly differentiated carcinoma of stomach with regional lymph node metastasis. Patient underwent distal gastrectomy with Billroth II anastomosis. Choriocarcinoma of stomach with well differentiated tubular adenocarcinoma was diagnosed by the surgical biopsy specimen, and the serum level of beta-HCG was 2,775 mIU/mL.In three months after operation, the patient presented with pain on right upper quadrant abdomen, and the CT scan revealed multiple liver metastasis. Bleomycin, etoposide, and cisplatin (BEP) combination chemotherapy were administered, but he died three months after the initial diagnosis.

10.Significant Motion Factors Affecting the Outcome of a Medial Branch Block in Patients with Low Back Pain Developing after Trivial Trauma.

OBJECTIVE: The aim of this study was to analyze significant motion factors that affected the outcome of medial branch blocking(MBB) in patients with low back pain(LBP) after trivial trauma. METHODS: Twenty-four patients(10men and 14 women; 70 vertebrae) complained of LBP after trivial trauma and under went bilateral MBB in the lumbar spine between January 2007 and August 2008. Visual Analogue Scale(VAS) scores of discomfort were measured before and after MBB. The motion factors considered were sitting, standing, flexion, extension and rotation. RESULTS: Sitting-, standing- and rotation-related LBPs were identified in 11 patients(44.5%, 11/24), 10 patients(41.7 %, 10/24) and 8 patients(33.3%, 8/24), respectively. The ratios of flexion- and extension-related LBPs in the enrolled patients were 66.7%(16/24) and 54.2%(13/24), respectively. The reduction in VAS score after treatment in the patients with sitting-related LBP was less than that of patients without sitting-related LBP(3.2+/-2.27 and 5.2+/-2.87, p=0.169). There was no difference between patients with standing-related LBP and those without(4.3+/-2.75 and 4.3+/-2.86). Although the mean VAS score improvement of the patients with extension- and rotation-related LBP(4.4+/-2.99 and 5.4+/-2.67) was higher than that of patients without extension and rotation LBP(4.1+/-2.59 and 3.7+/-2.71), the difference was not significant(p=0.721 and p=0.552, respectively). The mean VAS score improvement of the patients with flexion- related LBP was significantly higher than that of patients without it(6.1+/-1.81 vs 2.8+/-2.59, p=0.042). CONCLUSIONS: In patients with LBP after trivial trauma, flexion- and extension-related LBP was more common than sitting-, standing- and rotation-related LBP. Flexion was a significant motion factor affecting the outcome of MBB after trivial trauma.
