1.A Case of Malignant Duodenocolic Fistula Diagnosed by Endoscopy.

The occurrence of a fistula between the duodenum and the colon is very rare, although these two structures commonly lie in intimate relationship to each other. It is not uncommon for tumor of the right colon and proximal transverse colon to involve the duodenum by continuity, but fistulous connection between the duodenum and colon are infrequent In most of the reported malignant cases, the fistulas follow carcinomas of the transverse colon or the hepatic flexure of the colon. We report a rare case of malignant duodenocolic fistula which was first suspected by endoscopic examination and confirmed by operation.

2.A Case of Chordoma in the Sacro: Coccygyeal Area

Chordoma is a rare distinctively malignant tumor thought to be histogenetically related to the primitive notochord, It is locally infiltrative but does not metastasize until late. The usual initial symptom is pain or some symptom caused by involvement of the nervous system. Involed in the order of frequency are the sacro-coccygeal area, the spheno-occipital area, and the other vertebral area. This case is a sacro-coccygeal chordoma that is confirmed by histologic examination. The lesion was treated by complete excision.

8.Estimation of Alteration of Peripheral Blood Immune Cells in Patients with Plopecia Areata.

Authors have quantitated the T cell and T subsets(T and T cell) in the per- ipheral blood of 16 patients with alopecia areata and 16 normal healthy controls. The results are as follows: 1. The mean values of the T cell in total patient group(54,6+7.9g) and AAP group(5,3. 1+8. 1%) showed staticstially significant decrease when it compared with the mean value of the control group(64.6+3.6%) respectively(p<0.005). In ATU group the mean value of the T cell(53. 1+8. 1%) also showed decreasing tendency in comparison with that of control. The mean values of the T cell in total patient group(30.9+8. 9%) RIld AAP group(3'l. 5+8. %) showed statistically significant decrease when it compared with the mean value of the control group(42,6+8.9%) respectively(p<0. 005). In ATU group the mean value of the T< cell(28. 7+10. 9%) also showed decreasing tende- ncy in comparison with that of control. 3. The mean values of the Tr, cell in total patient group(6. 1+2. 2%) and AAP group(5.9+2.4%) showed staticstially significant decrease when it compared with the mean value of the control group(7.9+1.3%) respectively(p(p pl, pgp p25) In ATU group the mean value of the T cell(6. 7+0. 9%) also showed decreasing tendency in comparison with that of control. 4. The mean values of the T/T ratio showed no significant difference between the patient groups(total, AAP, ATU) and control group, respectively. Above data showed that defect of T cell and T subsets may be associated with the pathogenesis of alopecia areata.

9.A Clinical and Histopathologic Study of Livedo Vasculitis.

BACKGROUND: Livedo vasculitis shows variable clinical and histopathologic features according to its courses. There are also some discrepancies in histopathologic findings between the authors. OBJECTIVE: Our purpose is to clarify the clinical and istopathologic feat tures of livedo vasculitis. METHODS: We reviewed the clinical and histopathologic features in cluding direct immunofluorescence study and response to treigtment in eight patients with livedo vasalitis. RESULTS: The ratio of males to females was 1:3 with female predusm nance. Age of onset was predominant, in young to middle-age, ranging from 19 to 64 years old. In seven patients skin lesions were aggravated during the summe, and in one patient during the winer. All patients had purpuric or telangiectatic lesions on the lower extremities. Painful ulcers developed in seven patients and some of the ulcers healed with whitish atrophic scars. Five patients had preceeding livedo reticularis. One patient had Raynauds phenomenoin and the other patients did not show alinormal findings related to the systemic diseases. Histopathologically, in two patients fibrinoid materi.il was deposited in the vessel walls mildly, and in six patients obviously with partial to complete obst,r iction of blood vessels. Extravasation of RBCs and thrombus formation were found clearly in seven patients, but in one patient these findings were observed in a mild degree. Endothelial swellings were obserrved in all patients. Perivascular infiltrations of mononuclear cells were also observed in all patients in various degrees. But only two patients with secondary bacterial infection showed neutrophil infilt ations or nuclear dusts. So, histopathologic findings are corripatible with lymphocytic vasculitis. The locations of predominantly affected vessels were variable. Direct immunofluorescence studies were done in six patients. Four of these patients showed positive findings. At least 4 months were needed for the complete healing of the skin lesions. CONCLUSION: Livedo vasculitis is chronic vasculit is aggravated in mostly summer. Most of the initial purpuric lesions progress to ulcers which is healed with whitish atrophic scars. Histopathoiogic findings suggest livedo vasculitis is a sort of lymphocytic vasculitis.
