2.Skin Rash in A Patient with Infectious Mononucleosis after the Intake of Ampicillin.

During the course of infectious mononucleosis, intake of ampicillin and its analogues such as amoxicillin may cause hypersensitivity skin rashes. We report herein a case of ampicillin induced skin rash in a 41-year-old female patient with infectious mononucleosis. Infectious mononucleosis was confirrned by datetion of IgM antibody against Epstein-Barr(EB) viral capsid antigen(VCA) in her serum. During the icuteillness, she taked ampicillin for 3 days, and 1 week after the intake of ampicillin, a genertliz:d erythernatous and purpuric maculopapualr eruption developed. Physicians should be careful not to use ampicillin and its analogue if batients are suspected to be infected with EB virus as ampicillin induces severe skin rashes in patients with infectious mononucleosis.

3.Anatomical Locking Plate with Additional K-wire Fixation for Distal Clavicle Fracture.

BACKGROUND: Neer type II distal clavicle fractures have the drawback of coracoclavicular instability and insufficient distal bony fragment, thereby making it difficult to achieve adequate fixation. Although various surgical treatments have been described for Neer type II fracture, the optimal treatment remains controversial. This study reports the clinical results and usefulness of anatomical locking plate with additional K-wire fixation. METHODS: A totally of 21 patients with type II distal clavicle fracture were included in the study. The surgical procedure reduced the fracture temporarily; it included insertion of one or two K-wire from the lateral margin of the distal fragment to the proximal fragment through the fracture site, followed by application and fixation of the locking plate. The bony union and migration of K-wire was evaluated in the follow-up radiography. The coracoclavicular distance and acromioclavicular joint arthrosis were assessed at the final follow-up. The Constant Score (CS) and Korean Shoulder Score (KSS) were evaluated for clinical scoring. RESULTS: Bone union was achieved in all cases. At the final follow-up, coracoclavicular distance of the injured shoulder was increased, as compared to the intact shoulder (p=0.002), with no accompanying clinical symptoms. No K-wire migration was observed. At the final follow-up, K-wire irritation was observed in two cases and acromioclavicular arthrosis in one case, with no other adverse effects. Pain visual analogue scale, CS, and KSS were improved in all cases. CONCLUSIONS: The method of anatomical locking plate with additional K-wire fixation could be useful in achieving beneficial clinical results.

4.Clinical Analysis of Retinopathy of Prematurity.

Two hundred twenty premature infants who were inhalated 02 and under 2500mg birth weight or under 37 weeks of gestational age were examined from January 1990 to June 1991. We performed clinical analysis and results were as follows. 1. Thirty one infants (14.1 %) were diagnosed as retinopathy of prematurity. 2. Male patients were 17 (54.8%) and female patients were 14 (46.2%). There was no significant difference in incidence between male and female (X2=0.01, p>0.05). 3. The first examination was performed at 2.4 weeks cf life in average, and the retinopathy of prematurity was diagnosed at 4.8 weeks in average. 4. Stage 2 was detected in 10 patients (32.3%) and stage 3 was detected in 6 patients (19.4%). 5. Plus sign was found in 2 patients (19%) with stage 2 and 6 patients (100%) with stage 3. 6. The incidence of retinopathy of prematurity was highly associated with low birth weight (<1200gm), small gestational age (<30 weeks), and oxygen concentration (>or=60%). 7. Other associated possible risk factors were hyperbilirubinemia, idiopathic respiratoty distress syndrome, anemia, apnea and transfusion.

Choledochal cyst is a relatively rare disease entity considered to be a congenital cyatic dilatation of the common bile duct. Since occurrence of malignant tumor in choledochal cyet was first reported by Irwin and Morrison in 1944, approximately 100 more cases of malignant tumors arising in congenital choledocal cyst have been reported in the world, and the risk of malignant tumor related to choledochal cyst have been reported 2.4-14%. Recently, we experienced a case of adenocarcinoma arising in choledochal cyst in 39 year-old woman who was diagnosed by ERCP and cholangioscopy with forceps biopsy, and was treated with en bloc resection of the choledochal cyst along with the pancreatic head and duodenum.

7.A Case of Partial Hypopituitarism after Recovery from Korean Hemorrhagic Fever.

Heorrhage and infarct-like necrosis of anterior lobe of the pituitary gland is one of the characteristic pathologic findings of the autopsied cases of Korean Hemorrhagic Fever (KHF) patients, but there has been rare reports of hypopituitarism in patients with KHF. Recently we have experienced a patient with hyponatremia who had recovered from KHF. He was admitted to our hospital due to nausea, vomiting, and epigastric discornfort. To determine the function of the anterior pituitary gland, hormonal levels of target galnds and pituitary gland were measured, and combined pituitary stimulation test was performed. ACTH, GH, and prolactin deficiency were confirmed by combined pituitary stimulation test in this patient. There was no evidence of hypothalamic or other pituitary diseases by brain MRI. Our experience shows that KHF can be a cause of hypopituitarism and these findings should alert physicians the possibility of hypopituitarisrn in patients who had recovered from KHF.

8.The Lsolation of Organism and Sensitivity Test in Conjunctivitis Under 1 Year old of Age.

Neonatal and Infantile conjunctivitis are common disease in ophthalmologic outpatient department but, a study of causative organisms and antibiotics sensitivity tests are insufficient till now We divided 117 babies(117eyes) having conjunctivitis into neonatal and infantile group who were visited Sung-Ae General Hospital from Jan, 1989 to May, 1990. We performed bacterial culture and antibiotic sensitivity test. The results were as follows; 1) Culture positive rates are 65% in neonates and 87% in infants. There is no sexual difference of ratio. 2) The incidence of isolated organisms is Staphylococcus aureus, Coagulase negative staphylococcus and Streptococcus. 3) Cephalothin is most sensitive antibiotics and Chloramphenicol shows relatively lower sensitivity.

9.Palliative Transcatheter Arterial Chemoembolization for Relieving Metastatic Bone Pain due to Hepatocellular Carcinoma: A Case Report

Bone metastasis is the third most common extrahepatic metastasis from hepatocellular carcinoma (HCC), following lung and lymph node metastasis. Although transcatheter arterial chemoembolization (TACE) is widely used for the treatment for unresectable HCC in the liver, its effects on bone metastasis from HCC have not been well described in past literature. We report a case of a patient with a metastatic bone lesion from an HCC treated with TACE. After the procedure, the patient's pain and narcotic requirement significantly decreased without major complications.
