4.Collecting Duct Carcinoma of the Kidney Mimicking Invasive Transitional Cell Carcinoma: A Case Report.

Approximately 100 cases of collecting duct carcinoma have been reported in the medical literature. We herein report on a case of collecting duct carcinoma of the kidney in a 75-year-old patient. The abdominal sonography depicted a relatively poorly defined 7X6 cm sized, isoechoic mass lesion, as compared to the normal parenchyma, at the left kidney lower pole and the affected kidney showed preservation of the reniform shape. CT revealed a heterogeneous poorly defined low-attenuation mass that was mainly located in the medulla with involvement of the cortex and the lower half of the renal pelvis. Retrograde ureteropyelography showed a filling defect at the lower renal pelvis and severe narrowing of the left proximal ureter. We initially thought this lesion was invasive transitional cell carcinoma. Subsequent surgery confirmed a collecting duct carcinoma.

5.A Case of Epidermolysis Bullosa Dystrophica.

Epidermolysis bullosa dystrophica(EBD) is a rare, hereditary and chronic mechanobullous disease characterized by blistering and erosions of the skin in response to even minor trauma. A 5-year old girl visited with a complaint of continuous vesicobullous skin eruptions since birth. On examination, bullae, vesicles, atrophic scars, crusts and ero:ions were scattered on whole body. Loss of finger and toe nails, partial syndactyly of all toe webs and milia on dorsa of feet were noticed. We present a case of EBD that seems to be a recessive type considering the absence of family history with histopathologic and electron microscopic findings.

7.Intraoperative use of Real-time Ultrasonography in Neurosurgery.

Recent technological advances in ultrasound instrumentation have resulted in equipment that will image in "real-time" slices of the human brain similar in quality to computerized tomography(CT). Multiple authors have attested to its usefulness in locatin, characterizing, and facilitating in the biopsy or removal, or both, of intracerebral masses. In this study, the author has examined the efficacy of intraoperative neurosurgical ultrasound in 18 patients who were admitted to the Department of Neurosurgery, Kyung Hee University Hospital. The results were as follows : 1) It can provide critical assistance in precise localization of subcortical masses, since in many instances the overlying surface of brain provides no clue for localization. 2) This technique is very useful for the identificantion of deep lesions, provding infromation about their solid and cystic components and about the shortest route of access to tumor. 3) It provides precise information to help guide biopsy needles into deep lesions, and to confirm their presence within the lesion itself. 4) The system is highly accurate in visualizing ventricular catheters during and after their placement. 5) The system is inherently safe for the patient and the operating room personnel since there is no radiation exposure.

8.Clinical observation on renal injury: 66 cases.

The clinical observations were made on 66 patients of renal injuries during the period from 1985 to 1991) The results were as follows: I. The renal injuries were classified as contusion, laceration, rupture, pedicle injury according to Cass and Luxenberg`s classification. The patient numbers of each group were 42 (63.6%), 11 (16.3%).11 (16.3%) and 2 (3%). 2. The sonography was useful by complementary diagnostic tool of excretory urography in major renal injuries. 3. The computerized tomography was useful in diagnosing combined injuries and detecting injuries which were not detected in excretory pyelography. 4. The treatment of many. major renal injuries could be done conservatively with detailed and accurate diagnosis using computerized tomography.

9.Mixed lymphoepithelioma-like carcinoma and adenocarcinoma of the gallbladder.

Lymphoepithelioma-like carcinoma (LELC), an undifferentiated carcinoma with intense lymphoplasmacytic infiltrates, is commonly reported in the nasopharynx and occasionally in other organs. Pure type of LELC has previously been reported in the gallbladder. Mixed type could be reportable in comparison with other organs. Here we present a case of an 83-year-old man with mixed LELC and adenocarcinoma in the gallbladder. To the best of our knowledge, this is the first case of mixed LELC and adenocarcinoma in the gallbladder.
