4.Assessment of Pregnancy Outcome in IUGR Fetuses, Using the Doppler Flow Velocimetry of the Umbilical and Middle Cerebral Artery Authors.

OBJECTIVES: The purpose of this study in intrauterine growth retardation(IUGR) is to assess the relationship between the Doppler flow velocimetry and pregnancy outcome. STUDY DESIGN: 356 normal and 159 growth retarded fetuses between 21 and 42 gestati- onal weeks were investigated retrospectively. All IUGR fetuses were divided into four gro- ups according to the results obtained by Doppler flow velocities as follows. 1) Group 1(n= 89) : patients had normal umbilical artery resistance index ( Rlu ) and normal middle cerebral artery resistance index ( Rlmca ) 2 ) Group 2 ( n= 9 ) : patients had normal Rlu and abnormal Rlmca 3 ) Group 3 ( n=25 ) : patients had abnormal Rlu and normal Rlmca 4) Group 4 ( n=36 ) : patients had abnormal Rlu and abnormal Rlmca. RESULTS: We obtained normal values of RIu and RImca in oriental. Abnormal cardiotoc- ography(CTG) findings and operative delivery for fetal distress were significantly increased in group 2(p < 0.05) and markedly increased in group 3 and 4 ( p < 0.01 ) compared with group 1. Adverse neonatal outcome was significantly increased in group 3 and 4 compared with group 1. There is significant increase of adverse neonatal outcome in group 4 compared with group 3 ( p < 0.05 ). CONCLUSIONS: We obtained the normal values of Rlu, Rlmca and cerebroplacental ratio ( CPR ) in oriental. Adverse neonatal outcome was significantly increased in group 2, 3 and 4 compared with group 1. There is significant increase of adverse neonatal outcome in gr- oup 4 compared with group 3. We also found a strong correlation between the CPR and adverse neonatal outcome in IUGR. In IUGR, centralization of the fetal circulation turned out to exist, resulting in a preferential shunting of blood flow to the fetal brain. Brain-sp- aring effect with Doppler flow velocimetry could be demonstrated by this result.

5.Seridiagnosis of Syphilis.

The most frequently used serodiagnostic test for syphilis in Korea is VDRL test, which is one of several flocculation tests utilizing cardiolipin-lecithin as antigen. This test remains popular because it is easily performed, inexpensive, is standard for screening purposes, and is useful for following the serologic response to treatment. A common problem is weakly reactive (or 'rough' result) VDRL test or reactive VDRL in the absence of definite evidence of present or past syphilis; the diagnosis of syphilis in these cases should be confirmed by using the specific treponemal antigen tests. The Fluorescent Treponemal Antibody-Absorption (FTA-ARS) test is the result of continued attempts to improve the original FTA procedures. This test uses virulent T. pallidum (Nichols strain) as antigen and uses fluorescein-tagged antihuman globulin as reaction indicator. The FTA-ABS test is very sensitive in all stage of syphilis, especially in early syphilis. The FTA-ABS test was carried out on 448 persons with reactive and weakly reactive VDRL tests and divided the groups of routine physical check and out patients. (countinued..)

6.Identification of eplication-ompetent Dimeric Human Hepatitis B Viral Using PCR Screening Technique.

BACKGROUND/AIMS: Transfection of the hepatitis B virus (HBV) genome requires the cloning of tandem HBV sequences inserted into a plasmid vector, which is usually screened for by the restriction enzyme digestion of plasmid minipreparation from at least a dozen of bacterial colonies. The aim of this study was to develop a simple alternative screening method for bacterial colonies harbouring tandem HBV sequences by a PCR. METHODS: A set of polymerase chain reaction (PCR) primer was designed to detect the bacterial colonies harbouring "head to tail" dimeric HBV DNA. PCR which amplifies the head to tail junction site of two tandem HBV molecules was performed. RESULTS: PCR products with appropriate size (1.2kb) were obtained. The accurate detection by PCR screening technique was confirmed by enzyme digestion. CONCLUSIONS: These results suggest that PCR screening technique is a simple and rapid method for the identification of bacterial colonies containing tandem HBV sequences.

9.Primary Signet Ring Cell Carcinoma with Widespreand Metastasis.

We present an autopsy case of signet ring cell carcinoma of urinary bladder which underwent widespread metastsis through nearly entire organ. The patient, 45-year old male, suffered from hematuria, dysuria and flank pain for 6 months. Under the diagnosis of the signet ring cell carcinoma by the cystoscopic biopsy, radical cystectomy and ileocutaneous ureteral division were performed. But he died after 11-months after the diagnosis. The autopsy confirmed the widespread metastasis at the lung, kidney, adrenal gland, parathyroid gland, heart, stomach, ileum, spine, rib and meninges. The preveously resected urinary bladder discolosed the well defined fungating mass at the dome and anterio-lateral portion without apparent evidence of the remnant of the urachus. Histologically, the signet ring cells were predominent and focally mucinous pool floating the signet ring cells was noted. We could find several foci of the well differentiated adenocarcinoma and poorly differentiated adenocarcinomas. The above findings may reflect that the origin of carcinoma may be from the multipotential transitional cell epithelium.

10.Personality characteristics, depression and anxiety for patients with essential hypertension.

BACKGROUND: Majority of hypertension is essential in type, so its clear cue usually is not found, and the patients are tend to be persisted of psychopathy such as emotional tension, depression, anger, anxiety and psychologic conflict, also not to be adapted to stressful event. So we decide to survey the personality characteristics of essential hypertensive patients and factors of psychopathology. METHODS: Patient group-50 persons(male 23, female 27) are selected in the course of antihypertensive medication, being in or outpatient treatment at the dept. of F.M. or I.M. in Kae-jung, Dae-sung or Jung-ang Hospital from August 1, 1995 to July 31, 1996. Control group 52 persons(male 25, female 27) are selected. Exclusision criteria are any clinical disease hystory, over 140/90mmHg of BP, under 20-year old age or no cooperation. We assessment of the MMPI, BDI and STAI results about both group. RESULTS: Comparing hypertensive group with normal control group, significantly higher F, Hs, D, Hy, Pd, Pt and Sc scales of MMPI and BDI(p<0.01), and anxiety scale of STAI(p<0.05). There were no significant differences in all scales comparing between both sexs in the hypertensive and control groups, except comparing hypertensive male with hypertensive female in Pt scale(p<0.05) and control female with control male in Hy scale(p<0.05). CONCLUSIONS: Comparing essential hypertensive group with normal control group, significant higher score was found in scales of Hypochondriasis, Depression, Hysteria, Psychopathic deviate, Psychasthenia, Schizophrenia and anxiety. And in the treatment of hypertensive patient with antihypertensive drugs, appropriate additive psychotherapy appears useful in the case of revealing psychopathy.
