4.A Case of Erythromelalgia.

An 8-year-old female has suffered from intense burning with redness and increased temperature of the feet and hands since 5-year-old. We observed increase of the skin temperature in relation of pain attack by the digital thermometer and the color television thermography. No specific abnormalities were noted in the laboratory studies. Treatment with various systemic and topical medication revealed of unfavorable result, but for the conservative mangement, the pain was relieved by the exposure to the cold environment-laying bare in the outdoors and the wind of the electric fan.

5.Arterial Embolization for Management of Hemoptysis.

PURPOSE: Our purpose in this study is to evaluate the effectiveness of bronchial and nonbronchial systemic arteries for the control of hemoptysis. MATERIALS AND METHODS: Seventy patients with massive or recurrent hemoptysis underwent percutaneous transcatheter embolotherapy between 1991 and 1993. Retrospectively we reviewed 77 cases of bronchial artery embotization and 32 cases of nonbronchial systemic artery embolization in the 70 patients. RESULTS: Immdiate control of hemoptysis was achieved in 33 of 39 patients with massive hemoptysis(84.6%) and 20 of 24 patients with recurrent hemopt ysis(83.3%). In 32 cases, nonbronchial systemic arteries contributed significantly to areas of pathologic pulmonary tissue and frequently were the major arterial supply. CONCLUSION: Bronchial artery embolization is an effective and life saving procedure in non-surgical candidates. Recognition and occlusion of nonbronchial systemic arteries that feed to hypervascular pulmonary lesions is essential for successful percutaneous embolotherapy of hemoptysis.

6.The Significance of the Myelography in the Herniation of Intervertebral Disc

The herniation of intervertebral disc is possibly diagnosed through the myelography, electromyography, discography or computerized tomography. Among them, the myelography is regarded as the popularized method that helps accurate analysis on the location and pattern of the lesion. The authors have studied 363 cases of herniated intervertebral disc with respect to the diagnostic accuracy of myelographic examination from January, 1982 to December, 1985 at the Department of Orthopedic Surgery and Radiology, National Police Hospital. The results were as follows; 1. The most common age group was 21 to 30 years old and ratio of male and female was 9.7: l. 2. The most common site of the lesion was between L4 and L5 vertebra in myelography. 3. Of the 363 cases which underwent the myelography, 305(84.0%) represented positive findings. 4. Myelographic accuracy was coincided with the clinical diagnosis regarding the location of herniation of intervertebebral disc in 262 cases(72.1%).

9.Intra-Articular Alterations after Suprapatellar Nailing in Tibial Shaft Fractures: An Arthroscopic Evaluation

Purpose:
This study attempted to study the intra-articular changes due to intramedullary nailing through the suprapatellar approach by evaluating the joint cartilage damage and presence of foreign bodies through a comparison of the pre- and post-operative status evaluated by knee arthroscopy.
Materials and Methods:
This retrospective study analyzed fifteen patients who underwent intramedullary nailing through the suprapatellar approach for proximal tibial shaft fracture from January 2017 to March 2020. The condition of the joint cartilage and the presence of foreign substances in the patellofemoral joint were evaluated. The cartilage status of the patellofemoral joint was evaluated using the International Cartilage Repair Society (ICRS) grading system. Data from the ICRS grading and the visual analogue scale (VAS) scores of the femoral and patellar cartilage were compared to each independent variable surveyed.
Results:
All the intra-articular structures before nailing were normal. In all cases after nailing, articular cartilage damage of the patellofemoral joint and intra-articular debris were observed. The average VAS score was 0.6 (0-1) before surgery and 2.27 (0-4) after surgery. There were no statistically significant differences except for the correlation in the diameter of the tibia nail and femoral ICRS grade (p=0.001) and the damage to the cartilage was greater in the femoral cartilage than that to the patella (p=0.001).
Conclusion
Intra-articular damage appears to be unavoidable in suprapatellar nailing. Further research is needed on the long-term effects of intra-articular damage and on methods to reduce this damage.
