Sinus venosus is a rare cardiac defect, which may lead to an interatrial shunt. Diagnosis on
echocardiography may be difficult requiring an evaluation by a board-certified cardiologist.
We report a case of a 41 year-old male who presented with recurrent episodes of hemiparesis (first left
sided, second right sided). Surgical correction of sinus venosus led to resolution of his symptoms.

2.Delayed Diagnosis of Pulmonary Artery Injury, Due to Blunt Trauma, which Mimicked Traumatic Lung Pseudocyst.

A 67-year-old female presented to the emergency department with complaints of dyspnea and chest wall pain after a fall from a cultivator. Initial chest CT showed multiple left rib fractures, a loculated hematoma without active bleeding, and hemothorax. On the third day of admission, the chest X-ray showed an abrupt aggravation of haziness and the chest CT showed that the size of the hematoma had increased with active bleeding from the pulmonary artery. In cases of loculated hematomas adjacent to the hilum on CT scan, the diagnosis of pulmonary artery injury should be considered.

3.Iatrogenic Bronchial Injury: A Rare Complication of Tube Thoracostomy with a Small Bore Catheter.

A 69-year-old male was admitted for dyspnea and chest pain. The patient had undergone coronary artery bypass graft surgery and tube thoracostomy three years ago. The chest radiograph showed pleural effusion, which was drained using a percutaneous catheter with CT guidance. However, residual pneumothorax was observed four days later. Despite insertion of the 12 Fr trocar-type tube, pneumothorax did not improve and air leaks were observed. Chest CT showed that the tube was placed in the left main bronchus. After removal of the tube, the patient recovered uneventfully from pulmonary hemorrhage and bronchial perforation without complications.

4.Subdural Empyema in Infants.

OBJECTIVE: The goal of this study was to review the etiologies, phathophysiology, clinical presentations and to compare the results of the surgical methods of subdural empyema especially in infants. PATIENTS AND METHODS: We reviewed the clinical data of five infantile subdural empyemas experienced in our hospital from 1993 to 1998 which were all surgically treated by craniotomy or burr hole trephination. We experienced five cases of infantile subdural empyema: two males and three females. The ages ranged from 40 days to 11 months. RESULTS: The etiologies of all five cases were unproven but all five cases had meningitis before the subdural empyemas were diagnosed. The causative organism in one was streptococcus pneumoniae, and the other were unknown. Two were treated with craniotomy(one with a good outcome and the other died) and three were treated with burr hole trephination(all three had a good outcome). CONCLUSION: Subdural empyema is a rapid progressing disease and it is important to detect and treat in the early stages of disease. The choice of surgical method must be based on the stage of the disease and its location in the cranial cavity.

Twin reversed arterial perfusion (TRAP) sequence is a rare specific anomaly of twin gestation with fused placenta and umbilical anastomosis. This syndrome occurs once in about 34,600 births and reported first by Beneditti in 1533. We report on prenatal diagnosis of a case of TRAP sequence with color Doppler sonography, this case is presented with a brief review of the literature.

8.C. T. arthrography on Bankart lesion

Accompanying to the development of the knowledgment of mechanism and pathology of recurrent dislocation of the shoulder, accurate diagnostic tools to detect that pathology have been developed. During the period from May 1990 to May 1992, we treated 28 cases of recurrent anterior dislocation of the shoulder. Twenty-eight cases were examined with C.T. arthrography before the operation. Among the 28 cases, we identified the Bankart lesion in 20 cases by C.T. arthrography, and in 21 cases intraoperatively. We concluded that the C.T. arthrography is an accurate method for confirming the clinical diagnosis of recurrent dislocation of the shoulder and to be helpful in detecting the Bankart lesion and its severity or pathologic findings.

9.Analysis of Internal Rotator-External Rotator Muscle of the Shoulder with Cybex II

In recent trends of popularized sports and increased industrial hazards, we need the therapeutic modality in clinical setting, as a training and testing device for sports practicians, and as a method for scientific examination of post-operative status of muscles and joint function for orthopedist. Isokinetic exercise with Cybex II overcomes many biomechanical factors and can be used for fatigued patients by control of resistance. We examined internal rotator and external rotator muscle force of the shoulder joint, in order to acquire the isokinetic normative data of the shoulder for clinical use. The result were as follows 1. Strength and Power decreased as ages getting old, and there was significantdiminishbetween5th decade and 6th decade. 2. Power endurance did not decrease significantly as ages getting old. 3. The mean values of strength and power in internal rotator were higher than external rotator.
