BACKGROUND: Dopamine receptors are strong candidates for involvement in schizophrenia and are target of a wide variety of antipsychotics. Dopamine D5 receptor(DRD5) gene polymorphisms may be associated with various treatment response. The purpose of our study was define to what significance can be held as a predictor of treatment response in this polymorphism. METHOD: The total number of 116 Korean chronic schizophrenic patients was assessed after 48 weeks treatment. The Positive and Negative Syndrome Scale(PANSS) was rated for the clinical response to various antipsychotics. With the use of polymerase chain reaction amplification, we assessed this dopamine D5 receptor polymorphism in schizophrenic patients who had been treated with antipsychotics, and related genotype with treatment response, to test the hypothesis that DRD5 polymorphism may lead to varying resonse to antipsychotic. RESULT: DRD5 polymorphism was not associated with treatment response to a variety of antipsychotics in chronic schizophrenic patients. CONCLUSION: Genetic variation of D5 receptors do not predict treatment response to antispychotics.

It would be naive to assume that any operative procedure is the solution to the problem of sacral pressure sore in the field of the orthopaedic surgery. The procedures outlined here involve the creation of compound myocutaneous flaps of the gluteus maximus muscle, skin, and the subcutaneous tissue. The myocutaneous flap us- ing the gluteus maximus muscle is a vascular flap instead of a random flap, with better blood supply for healing and advantage of an increased amount of cushion effect. We present an alternative method which will provide satisfactory and substantial soft tissue coverage in sscral pressure sores.

Hyperkeratosis of the nipple and areola is a rare skin disordr characterized clinically by thickening and discoloration of the skin of the areola and/or nipple and pathoiog cally by papillomaosis or acanthosis-nigricans like changes. We report a case of hyperkeratosis of the nipple and areola occuring in a woman with no underlying endocrinopathy and no other additional hyperkeratotic conditions. Ttie iresent case seems to be the nevoid form by the Levy-Franckel classification. Treatment with topical medications including steroids and keratolytic agents produced no improvement. Therefore we recommeneded surgical excision because the paient was greatly disturbed by the appearance of the lesions. Surgical excision and full thickness skin graft were performed sucessfully.

PURPOSE: Transurethral prostatectomy(TURP) or open prostatectomy were considered as standard treatments for benign prostatic hyperplasia(BPH), but these treatments showed high morbidity and mortality. We searched urodynamic predictive factors for successful surgical outcome to select patients who would benefit from prostatectomy pre-operatively. MATERIALS AND METHODS: Retrospectively, we reviewed preoperative urodynamic findings of 114 patients with BPH undergoing TURP(104cases) or open prostatectomy(10cases). RESULTS: In the follow-up period of 1 month to 19 months, a strict successes were achieved in 83 patients(73%). On analysis of the success rate, 4 favorable urodynamic factors and 8 unfavorable urodynamic factors were noted. 4 favorable factors were as follows; maximal flow rate<10ml/sec, compressive flow pattern, prostatic urethral length >6cm and presence of peak elevation on prostatic urethra pressure. 8 unfavorable factors were as follow; intermittent or normal flow pattern, prostatic urethral length<4cm, maximal detrusor pressure<10cmH2O, maximal bladder capacity<250ml, maximal urethral pressure >100cmH2O, prostatic pressure area <70cm cmH2O, non-obstructive pattern on Abrams-Griffiths nomogram and detrusor pressure at maximal flow rate <50cmH2O. CONCLUSIONS: Patients with more than 1 unfavorable urodynamic factor should be investigated carefully before surgery and be treated with other non-invasive treatment for BPH. The presence of 2 favorable urodynamic factors without unfavorable factor will usually predict the best surgical outcome.

Porokeratosis

PURPOSE: To determine the accuracy of predicting potential visual acuity in cataract using the illuminated near card (INC). METHODS: Thirty-nine eyes (34 patients) having cataract were studied prospectively by comparing the postoperative distance Snellen acuity to the visual acuity (VA) obtained preoperatively using the INC viewed through a pinhole. RESULTS: The INC predicted postoperative acuity within two Snellen lines in 20 of 39. The accuracy between predicted and achieved acuities was follows: 61.5% in patients with a preoperative acuity of better than 0.3, 53.8% of better than 0.1, 50.0% of better than 0.05 and 28.6% of worse than 0.05. The disparity between INC results and postoperative VA was 3.89 lines (cortical opacity, nucleosclerosis, subcapsular opacity), 2.48 lines (cortical and subcapsular opacity), 2.08 lines (cortical opacity, nucleosclerosis), 1.8 lines (nucleosclerosis only), 3.3 lines (nucleosclerosis, subcapsular opacity), 1.6 lines (subcapsular opacity) and 3.0 lines (the others). CONCLUSIONS: The INC is easy to use and is a fast and accurate measurement instrument for predicting postoperative VA after cataract operation.
