BACKGROUND: Hypertrophic scars and keloids have been regarded as representative of the proliferative change of the connective tissue of the dermis. Clinically, postburn and surgical scars show a smooth, shiny, erythematous appearance at an early stage. It is readily conceivable that, in such scars, changes may take place not only in the dermis but also in the epidermis and, possibly in the stratum corneum (SC). However, in contrast to the tremendous number of studies of scars on the dermis, those studies focusing on the epidermis and the SC have been scarce. OBJECTIVE: We have focused on the function of the SC covering the post-burn scar tissue and keloids. METHODS: Using noninvasive bioengineering measurements of functional properties of the SC, such as transepidermal water loss(TEWL), we evaluated the SC barrier function in various types of healing wounds, such as early erythematous lesion, hypertrophic scar, keloid, healed atrophic scar, scar occurring at the recipient site of the skin grafts and the adjacent normal appearing skin for control. RESULTS: 1. The TEWL values were 14.9+/-7.3 in early erythematous lesions, 13.2+/-7.5 in hypertrophic scars, 10.2+/-5.8 in keloids, 5.6+/-1.3 in healed atrophic scars, 6.9+/-4.3 in scars occurring at the recipient site of the skin grafts. Significantly increased TEWL values were found in all individual lesions(p>0.01) except for the scars occurring at the recipient site of the skin grafts(p<0.05) compared with the corresponding normal control skin. 2. PURPOSE: Our purpose in this study is to evaluate the effectiveness of bronchial and nonbronchial systemic arteries for the control of hemoptysis. MATERIALS AND METHODS: Seventy patients with massive or recurrent hemoptysis underwent percutaneous transcatheter embolotherapy between 1991 and 1993. Retrospectively we reviewed 77 cases of bronchial artery embotization and 32 cases of nonbronchial systemic artery embolization in the 70 patients. RESULTS: Immdiate control of hemoptysis was achieved in 33 of 39 patients with massive hemoptysis(84.6%) and 20 of 24 patients with recurrent hemopt ysis(83.3%). In 32 cases, nonbronchial systemic arteries contributed significantly to areas of pathologic pulmonary tissue and frequently were the major arterial supply. CONCLUSION: Bronchial artery embolization is an effective and life saving procedure in non-surgical candidates. Recognition and occlusion of nonbronchial systemic arteries that feed to hypervascular pulmonary lesions is essential for successful percutaneous embolotherapy of hemoptysis.

Schirmer test and tear film break up time(BUT) were performed in 126 patients who complained of nonspecific conjunctival symptomps. The mean value of Schirmer test was 7.85mm/5min and the mean value of the BUT was 9.68sec. Marked decrease was shown in comparison with previously studied normal values of Korean and according to these results, dry eye was suspicious. Same as for normal eyes, there was no relationship between the Schirmer test and BUT in this study. So, Schirmer test and BUT with detailed slit lamp exam should be necessary for those who complain of tearing, foreign body sensation and visual disturbances and these may helpful for diagnosis and treatment of dry eye patients.

Lupus erythematosus panniculitis is a rare variant of lupus erythematosus primarily involving a panniculus with or without discoid lupus erythematosus in the overlying skin. A 34-year-old woman showed multiple, variable-sized, indurated subcutaneous nodules on both her upper arms and axillae. A Histopathological examination revealed lobular panniculitis with infiltration of several hyperchromatic atypical large lymphocytes. The infiltrated cells were positively stained with LCA, UCHL-1, CD4, CD8, but not with CD20 and CD68. A T cell receptor -chain gene and immunoglobulin heavy chain gene rearrangement study showed no monoclonality. The result of an ANA test was positive at 1: 40 in a homogeneous pattern. Skin lesions were improved by combined therapy with daily hydroxychloroquine 400mg and prednisolone 10mg in a week and after 6weeks, resolved with atrophic depressed scars. After 4 months, she complained of symptoms of the Raynaud phenomenon and was managed by nifedipine. There has been no recurrence during follow-up for 7 months.
