1.Periosteal Reaction of Osteomyelitis: MRI Findings Compared with Plain Radiographs.

PURPOSE: To evaluate MR characteristics of periosteal reactions and subperiosteal abscesses in osteomyelitis as compared with radiographs. MATERIALS AND METHODS: We retrospectively reviewed 28 patients(18 males, 10 females) with osteomyelitis. Nineteen patients underwent MR imaging with 0.5 T. We analyzed for morphologic patterns, intervals of appearance and disappearance of periosteal reactions after symptom onset. Twenty-three patients were confirmed by surgery and 5 patients by the radiologic findings, clinical and laboratory data. RESULTS: Periosteal reaction appeared as low signal intensity arc on the T1- and T2-weighted axial images. In 3-7 days after symptom onset, periosteal reactions and subperiosteal abscesses were noted on MR imaging, and only two of eleven patients were noted in radiographs. Periosteal reactions of 8-14 days after symptom onset were demonstrated in all of 13 patient on MR, and 9 on plain radiographs. All patients with 2 weeks after symptom onset showed perioste.al reaction in plain film and MR. The periosteal reactions persisted until approximately 2 to 3 months after treatment on follow up radiographs. CONCLUSION: Periosteal reactions in osteomyelitis are detected on the MR imaging earlier than plain film. MR is valuable in detecting subperiosteal abscess which is not appecent in simple radiographs. Periosteal reactions can be seen on MR in 3 days after symptom onset and persist for 2-3 months after treatment.

3.A Case of Eczema Herpeticum with Hailey-Hailey Disease.

Eczema herpeticum is the dissemination of herpes simplex virus in the setting of a preexisting skin disease. Hailey- Hailey disease [familial benign chronic pemphigus] is a blistering dermatosis that is inherited as an autosomal dominant trait and it usually presents itself around the third or fourth decades. Coexistence of eczema herpeticum and Hailey-Hailey disease is an infrequent occurrence. Four such cases have been reported in the English and German medical literature. We report here on an unusual case of eczema herpeticum that coexisted with Hailey-Hailey disease in a 47 years old man and we review the relevant literatures.

6.A Study on Clinical Features and Laboratory Findings According to the Severity of Atopic Dermatitis.

BACKGROUND: Although atopic dermatitis (AD) is a very common disease, there is no specific diagnostic or prognostic marker for it. Minor clinical features and a few laboratory findings can be used as ancillary diagnostic criteria and reflect the present state of AD, but cannot predict the disease severity. OBJECTIVE: We performed this study to find out if there is any correlation among minor clinical features, laboratory findings such as blood eosinophil count and serum IgE levels, and the severity of AD. METHODS: One hundred patients with AD were involved in the study. They were divided into two groups by the age of 12 years; group 1(< or = 12-year-old, n=47) represented childhood AD and group 2 (> or = 12-year-old, n=53) represented adolescence and adult AD. Subsequently, the severity of AD was assessed by the SCORAD index, and then all the patients were classified into 3 subgroups; mild, moderate and severe groups. They were evaluated for 28 minor clinical features of AD. In addition, blood samples were measured for blood eosinophil count and serum IgE levels. RESULTS: The pattern of minor clinical features correlated significantly with the disease severity was quite different between the two groups, except ventral wrist eczema and eyelid eczema. Blood eosinophil count and serum IgE levels increased significantly as the SCORAD index increased. While perifollicular accentuation was a significant minor clinical feature that correlated with blood eosinophil count in the both groups, there were no minor features showing statistically significant correlation with serum IgE levels. In group 1, keratosis pilaris and infragluteal eczema appeared more frequently as all 3 variables such as the SCORAD index, blood eosinophil count and serum IgE levels, increased simultaneously. However, no minor clinical features which correlated significantly with all variables were observed in group 2. CONCLUSION: We concluded that minor clinical features such as keratosis pilaris and infragluteal eczema reflect the disease severity and can be used as an anticipating sign of severe disease, especially in childhood AD.

7.Basic Demand Survey of Community Based Stroke Disabled in an Urban Area.

OBJECTIVE: To investigate the status and basic demand of community based stroke-disabled for rehabilitation program development in an urban area. METHOD: The subjects were 46 residencial stroke disabled over 6 months after disease onset. Two teams (composed of one physiatrist and one nurse, respectively) visited patient's home, and evaluated physical and functional status and surveyed for the status and basic demand of community based rehabilitation. RESULTS: Forty two patients (91.3%) received primary medical care after onset of the stroke, and twenty two patients(47.8%) received rehabilitation therapy. The most common reason for not having received rehabilitation therapy was poor economic state. University hospital was the most common place of their rehabilitative management. Mean score of Modified Barthel Index (MBI) was 60.7. Mean score of Craig Handicap Assessment and Reporting Technique (CHART) was 223 and economic self-sufficiency scale presented the lowest score as 6.5+/-4.6. The desire of patients for rehabilitation and welfare service was rehabilitation management and the desire of subjects for the government and society was warrant for livelihood. CONCLUSION: We concluded that rehabilitation program utilizing common resource such as areal university rehabilitation team and facilities of community health center should be developed.

Adrenal myelolipomas are rare, benign tumors cornposed of mature fat and bone marrow elements. Most are small, asymptomatic tumors found incidentally at postmortem examination, Fine needle aspiration biopsy can be used to confirm the diagnosis and avoid an unnecessary operation. We report a case of adrenal myelolipoma confirmed by fine needle aspiration biopsy. A 77-year-old woman with complaining of upper abdominal pain for 2 days was found to have an adrenal mass. She took dexamethasone frequently for 3 years due to multiple arthralgia. Ultrasono-graphy showed a 6cm-sized, suprarenal mass and a stone in the gall bladder with thickened wall. Computed tomography and magnetic resonance imaging scan also presented a suprarenal mass. Endocrinologic results were within normal limits. Adrenal myelolipoma was confirmed by computed tomography-guided fine needle aspiration biopsy without surgery.

Churg-Strauss Syndrome is a disorder of hypereosinophilia and systemic vasculitis in subjects with asthma and allergic rhinitis. Clinically, a multiple organ system can be involved with various manifestations of disease of lung, heart, skin, musculoskeletal system, nervous system, gastrointestinal and hepatobiliary tract. We experienced a case of Churg- Strauss syndrome presenting as the appendicitis and the lower gastrointestinal bleeding in a 37-year-old male patient with acute lower abdominal pain. He also showed peripheral eosinophilia, bronchial asthma, and mononeuritis multiplex. He initially received a high dose corticosteroid and was maintained with low doses of corticosteroid, cyclophosphomide and exchange plasmapheresis.

10.FDG Uptake in PET by Bladder Hernia Simulating Inguinal Metastasis.

A 70-year-old man with past history of hemicolectomy due to colon cancer underwent a follow-up abdominal/pelvic CT scan. CT revealed a right adrenal metastasis and then he underwent FDG-PET/CT study to search for other possible tumor recurrence. In PET images, other than right adrenal gland, there was an unexpected intense FDG uptake at right inguinal region and at first, it was considered to be an inguinal metastasis. However, correlation of PET images to concurrent CT data revealed it to be a bladder herniation. This case provides an example that analysis of PET images without corresponding CT images can lead to an insufficient interpretation or false positive diagnosis. Hence, radiologists should be aware of the importance of a combined analysis of PET and CT data in the interpretation of integrated PET/CT and rare but intriguing conditions, such as bladder herniation, during the evaluation of PET scans in colon cancer patients.
