4.Plasmacytoma on the upper cyclid.

Plasmacytoma is one of the plasma cell dyscrasia. It occurs commonly on nasopharynx and paranasal sinus, but rare on the eyelid. If it involves the orbit, variable symptoms are presented, such as painless growing mass, proptosis, diplopia, decrease of vision, eyeball deviation etc. Irradiation is generally accepted as the treatment of choice for extramedullary plasmacytoma. Although the patient treated with irradiation safisfactory, wer must carefully follow up the patient about progression of multiple myeloma. We experienced a 15-years-old female who had a progressive growing palpable and non-tender mass on the right upper eyelid. The mass was confirmed as plasmacytoma by pathologic examination. We treated the mass with radiation. So, We present a case of plasmacytoma on the upper eyelid.

5.The Role of Urinary Sodium at Transient Renal Acidification Defect during Acute Infantile Acute Gastroenteritis.

The pupose of this study is to verify the role of urinary sodium in transient renal acidification defect which frequently combine acute infantile gastroenteritis. We studied on twenty-five infants, 2month to 36 month of age, during the 4 month period, from August, 1991 to December, 1991. The patients had acidosis for a mean of 3 days and sixty urine samples were collected during this period. The mean pH of 23 rine samples with sodium concentration<10 mmol/L was significantly higher than pH of 37 samples with sodium concentration<10 mmol/L, We separately analyzed 15 urine samples collected during metabolic acidosis after completion. of rehydration. The result was that a urinary acidification defent was observed in the urine samples with low sodium concentration but not in the sodium rich samples. We concluded that impaired urinary acidification defect is frequently during metabolic acidosis in infants with acute gastroenteritis and results from a sodium deficit rather than from transient distal renal tublur acidosis.

6.Safety Education Needs and Knowledge and Attitude of Injury Prevention of Elementary School Children.

PURPOSE: This study was conducted to suggest direction and strategy of safety education proper to elementary school children. METHOD: The subject of this survey consisted of 313 3rd-5th grade from 4 elementary schools in Kyungi-Do and Kwangwon-Do. Data were collected from March to May, 2002 using a questionnaire about "safety education needs", "knowledge about injury prevention", "attitude about injury prevention. RESULT: 1. The degree of safety education needs showed averaged 77.50 on the basis of 100 points. 2. The degree of knowledge and attitude about injury prevention showed averaged 72.81 and 81.74 seperately on the basis of 100 points. 3. With the respect to the demographic characteristics, there were stastically significant differences in safety education need according to children's grade(F=8.692, p=.003), sex(t=-2.059, p=.040), family type(t=-2.229, p=.027) and in knowledge & attitude about injury prevention, there statiscally significant difference according to experience of injury prevention education(t=3.058, p=.003; t=5.308, p=.000) each. 4. The level of safety education needs is correlated at signficant level with knowledge and attitude about injury prevention of childrens(r=.166, p=.048; r=.265, p=.001) and between knowledge and attitude about injury prevention, there was significant correlation (r=.427, p=.000). CONCLUSION: From this results, nurses can plan safety education program appropriate to children's needs, level of knowledge and attitude about injury prevention.

7.Association of the CYP1B1 Gene Polymorphism with the Risk of Advanced Endometriosis in Korean Women.

OBJECTIVE: To investigate whether polymorphisms of gene encoding CYP1B1 is associated with the risk of endometriosis in Korean women. METHODS: We investigated 199 patients with histopathologically confirmed endometriosis rAFS stage III/IV and 183 control group women who were surgically proven to have no endometriosis. The genetic distribution of four different CYP1B1 polymorphisms at G119-T, G432-C, T449-C, and A453-G were analyzed by polymerase chain reaction (PCR) and restriction fragment length polymorphism of PCR products. RESULTS: We found no overall association between each individual CYP1B1 genotype and the risk of endometriosis. The odds ratio of genotype GG/GC+GG/TC+TT/AA compared to GG/CC/CC/AA (reference) was calculated as 2.06 with a 95% confidence interval of 1.003~4.216. CONCLUSIONS: This results suggest that CYP1B1 genetic polymorphism may be associated with development of endometriosis in Korean women.

8.Factors Associated with Compliance Using Diamicron in Patients with Type 2 Diabetes.

BACKGROUND: Diabetes mellitus (DM) is one of the most important chronic diseases in public health, thus optimal management of this condition is crucial. For this purpose, oral hypoglycemic agents (OHA) are commonly prescribed with their consistent use being mandatory to maintain adequate blood glucose levels. We aimed to evaluate the compliance with OHA in patients with DM, and to determine the factors associated with compliance. METHODS: From March 2006 to December 2007, we enrolled patients with type 2 DM who were newly diagnosed or had discontinued OHAs for more than four weeks. For OHA, diamicron was prescribed to the participants and response to medication was evaluated by physicians after 12 weeks. Compliance was defined as patients taking diamicron continuously or stopping the medication due to well-controlled blood sugar levels. Demographic characteristics, anthropometric measurements, fasting glucose, and lipid profiles were collected and analyzed. RESULTS: The mean age of our 326 subjects was 56.0+/-9.9 years and 74.5% was defined to be compliant. Logistic regression analysis adjusting for age and sex revealed that combined medication use (odd ratio [OR], 2.03; 95% confidence interval [CI], 1.04-3.97) and diabetes-related nutritional education (OR, 2.08; 95% CI, 1.08-4.03) (P<0.05) were factors associated with compliance with OHA. CONCLUSIONS: About three quarters of the DM patients in our study, using diamicron for the first time, showed compliance after 12 weeks of treatment. Concomitant medications and diabetes-related nutrition education were associated with compliance.

9.A Case of Split Hand Split Foot Anomaly.

We experienced a case of split hand split foot(SHSF) anomaly in a male neonate who had a deficiency of the middle finger, syndactyly of the 1st and 2nd finger and a deep median cleft in both hands. He also had a deep median cleft between 2nd and 3rd toe, syndactyly of the 1st and 2nd, 3rd and 4th toe without deficiency of the middle toe in both feet. SHSF anomaly may occur either isolated or associated with other anomalies. In this case, it occurd sporadically without family history and showed an isolated type without any other specific anomalies except both posterior iris synechiae. The karyotype of patient showed 46,XY,t(7p:14q) which has not been reported yet. We reported the case with the review of the associated literatures.

10.Improve Effects of Saengshik on Patient with Fatty Liver and Hyperlipidemia in Murine.

To examine the effect of Saengshik on parameters related to hepatoprotective, anthropometric, blood pressure, serum lipid and blood related indices, nonalcoholic fatty liver subjects were treated with two meal portion of Saengshik in the replacement of meals for a period of three months. Weight, Body Mass Index (BMI) and systolic blood pressure were significantly decreased after the treatment. Chronically elevated serum aspartate aminotransferase (AST), gamma-Glutamyl transferase (r-GTP) and Alkaline Phosphatase (ALP) levels showed reduction to the near normal range. Serum total triglyceride level were reduced following the treatment. Whereas, there were no changes of serum total cholesterol with Saengshik consumption. Also, additional study was conducted to investigate the effect of Saengshik supplementation to high cholesterol and fat diet on lipid metabolism in rats. Male Spraque-Dawley rats were administrated hyperlipidemiainducing diet containing 1% cholesterol and 10% lard to induce hyperlipidemia for 4 weeks and were fed on diet containing Saengshik (30%, w/w) for 7 weeks. The feeding of diet containing 30% Shaengshik significantly decreased total cholesterol (TC) contents and total triglyceride. These results demonstrate Saengshik may be beneficial for fatty liver patients in improving their lipid metabolism.
