1.Food ( wheat flour ) dependent exerclse - induced anaphylaxis in asthmatics : related with the amont of wheat flour and exercise.

A 25-year-old male with asthma suffered from generalized urticaria, angioedema and syncope associated with exercise after ingestion of food made with wheat flour. Skin prick test with bread showed strong positive reaction(++++), but with wheat flour, a weak positive(+). Challenge test with wheat flour, and food made with wheat flour, without or with exercise, were done several times. Exercise such as 25 min of running on treadmill following 30 min after 50 grams of bread or 50 grams of hot pepper-stuffed rice cake only showed positive reaction such as generalized urticaria, angioedema and hypotension, but not with 15 grams of bread or 15 grams of hot pepper-stuffed rice cake. Ingestion of bread in such high amounts as 15 grams alone did not trigger reaction, not even with is min of exercise. This is the first clear case of food dependent exercise induced anaphylaxis (FDEIA) in which the allergic reaction depended on both the amount of food ingested and the amount of exercise was clarified.

2.Prolonged Horner's Syndrome Following Interscalene Brachial Plexus Block: A case report.

The brachial plexus block by interscalene approach is useful for any procedure on upper extremity, including the shoulder. Complications such as high epidural block, total spinal anesthesia, permanent neurologic damage, phrenic nerve block, cardiac arrest, pneumothorax and Horner's syndrome etc. were reported. Ipsilateral Horner' syndrome can be used for confirmation of the accompanying sympathetic block. We experienced a case of 35-year old male patient who developed ipsilateral Horner's syndrome following the injection of 40ml of 1% lidocaine with 1 : 200,000 epinephrine to brachial plexus by interscalene approach. This compication lasted for 10days after interscalene brachial plexus block.

3.Bilateral torsion of the testes in a newborn infant.

We report one case of bilateral testicular torsion. A 400 gm male infant, born by cesarean section due to induction failure at 42 weeks gestation, was hospitalized at 9 days of age secondary to bilateral testicular mass. Physical examination revealed an healthy baby with enlarged, firm, nontender mass on both sides of the scrotum. Transillumination of the mass showed no transmission of light. Laboratory data included a normal CBC and a urinalysis. Ultrasonogrophy showed well encapsulated 0.9 x 0.9 x 1.0 cm sized and 1.5 x 1.3 x 1.3 cm sized homogenous hypoechoic solid mass in left and right testicles respectively, remainder testicular parenchyme showed normal structure. Exploration of the testes was performed at 25-days old under general anesthesia. Exploration revealed a infarction of the right testes secondary to spermatic cord torsion, evidenced by clockwise turns outside the tunica vaginalis of 270 degrees. Left testes was infarcted but a quarter of them seemed normal. Orchiectomy on right and fixation on left testes were performed respectively. Left spermatic cord seemed to be detorted spontaneously. Postoperatively the infant did well and discharged 5 days after operation. Follow up examination during the next 3 months have shown appropriated sized testicle with normal colorization and arterial pulse. Testicular scan performed at age 39 days showed good perfusion of left testes.

6.Tuberculin skin test and measles vaccination.

Although the incidence of tuberculosis in Korea in general has declined in recent years, it remains an importat health problem, especially in children. Inspite of the recent controversy over the necessity of tuberculin skin test prior to measles vaccination, this brief report shows 13.8% of positive skin test at 9 and 15 months of age at the time of measles vaccination. We feel, in Korea, practicing tuberculin skin test prior to measles vaccination is good way of detecting asymptomatic infection in young age who needs preventive therapy.

7.Clinical Evaluation of ASCUS.

OBJECTIVE: To evaluate the significance of atypical squamous cells of undetermined significance(ASCUS) by following-up cervical cytologic smears or histologic finding. METHODS: Two hundred fifty three cases were diagnosed as ASCUS on cervical cytologic smears from January, 3, 1997 to september, 16, 1998 at cytopathology laboratory, College of Medicine, Keimyung Univemity. These cases were evaluated by cervical smears or subsequent biopsies. RESULTS: The results were as follows, On repeat cervical smears, inflammation was 19.7%, normal was 11,1% and HSIL was 1.6%. On histologic diagnosis, CIS was 13,4%, cervicitis was 12,6%, invasive cervical cancer was 4.0%, and severe dysplasia was 3.2%. CONCLUSION: A diagnosis of ASCUS on smears need careful follow-up.

8.Knowledge on Sexuality and Needs of Sex Education for College Students.

PURPOSE: The purpose of this study was to measure the knowledge. Attitude and the needs of sex education for college students in order to provide basic sex education information. METHOD: For this study, the questionnaires were given to 320 students in the H, D colleage of Daegu and collected during the period from August 25 to October 25, 2003. These results were analyzed statistically by means of frequency, percentage, t-test, chi-squire test. RESULT: Female students have a high score of sexual knowledge about genital physiology, sex physiology, V. D. and contraceptive. Both gender in sexual knowledge, male students correctly responded 86.4% form ejaculating sperm from the penis. Females on the other hand correctly responded 96% in the highest score to menstruation blood through out the vagina. Both gender male and female is wished to be co-education in sex education in college at a freshmen level. Sex education should be under by special lecture when education is in progress. Males student want to learn more about V.D. Females in the other hand want to learn a lot about contraceptive. CONCLUSION: In conclusion college sex education should be basic, knowledgeable and for the future of there on going marriage. It should be responsible in sex education, it should run under regular curriculum in college.

9.Relationship between the Discomfort of Tinnitus and the Quality of Life in Tinnitus Patients.

PURPOSE: To investigate audiometery and characteristics related to tinnitus in tinnitus patients. and to understand the relationship between the discomfort of tinnitus and the quality of life. Methods: The subjects were 110 patients with tinnitus. The data were collected using tinnitus handicap questionnaire. quality of life, and pure tone audiometry, which measured hearing level in dB and frequency in Hz matching of tinnitus. RESULTS: Majority of tinnitus patients (75.5%~81.8%) had normal auditory sensation. but 82.7% of patients indicated that they have heard tinnitus frequently. or always, and the major sounds of tinnitus were wing, wung, wang, The matching sounds of tinnitus had various range in hearing level(dB) and frequency(Hz). Tinnitus patients reported moderate levels of the discomfort of tinnitus and the quality of life, and there were negative correlation between the discomfort of tinnitus and the quality of life(r=-.314, p .00l). CONCLUSION: These findings suggest that special attention is needed for tinnitus patients because the physical. psychological. and social impacts of tinnitus could be decrease their quality of life, Specialized nursing intervention considering specific conditions related to tinnitus is required to solve various health problems of tinnitus patients.

10.A Case of Partial Hypopituitarism after Recovery from Korean Hemorrhagic Fever.

Heorrhage and infarct-like necrosis of anterior lobe of the pituitary gland is one of the characteristic pathologic findings of the autopsied cases of Korean Hemorrhagic Fever (KHF) patients, but there has been rare reports of hypopituitarism in patients with KHF. Recently we have experienced a patient with hyponatremia who had recovered from KHF. He was admitted to our hospital due to nausea, vomiting, and epigastric discornfort. To determine the function of the anterior pituitary gland, hormonal levels of target galnds and pituitary gland were measured, and combined pituitary stimulation test was performed. ACTH, GH, and prolactin deficiency were confirmed by combined pituitary stimulation test in this patient. There was no evidence of hypothalamic or other pituitary diseases by brain MRI. Our experience shows that KHF can be a cause of hypopituitarism and these findings should alert physicians the possibility of hypopituitarisrn in patients who had recovered from KHF.
